[Mental disorders in pathological gamblers].

The study aimed at investigating mental disorders in pathological gamblers. A sample included 73 pathological gamblers (PG) and 77 age- and sex-matched controls. Severity of mental disorders was assessed using the SCL-90-R questionnaire. Most of PG were male (89%), mean age 26,8 +/- 6,3 years. Divorced were 27,4% and never married--20,5%. Most of PG (71,1%) had secondary school education. Alcohol-related problems were observed in 35,6% of PG and 52,1% had positive family history of alcohol-dependence. Mental disorders, mostly of anxiety-depressive and asthenia character, as well as obsessive-c

[Treating psychic traumas: a psychiatric emergency].

Interest for the psychopathological field of trauma has experienced a revival over the last fifteen years. Early and active treatment of victims is necessary to attenuate the psychopathological consequences of trauma. However, emergency psychiatry still rarely places a high value on it. This paper presents a case which contains in itself many aspects of psychological responses to psychologically traumatizing events. Trauma induced in this case, in particular, Dissociative Disorders (including a Dissociative Fugue), a Post-Traumatic Stress Disorder, Somatoform Disorders and Phobic Disorders. Th

Definition of a VR-based protocol to treat social phobia.

Social phobia is an anxiety disorder that is accessible to two forms of treatment yielding scientifically validated results: drugs and cognitive-behavioral therapies. Graded exposure to feared social situations is fundamental to obtain an improvement of the anxious symptoms. Traditionally, exposure therapies are done either in vivo or by imagining the situations. In vivo exposure is sometimes difficult to control and many patients have some difficulties in using imagination. Virtual reality (VR) seems to bring significant advantages. It allows exposures to numerous and varied situations. This

Solving panic disorder problems. When your patients' fears thwart their lives.

All in all, panic disorder with or without agoraphobia is a very satisfying illness to treat. Most patients enter treatment with considerable pain and disability and within a few weeks to months are free of panic attacks, no longer fearful of them, and no longer phobic. Panic disorder must be distinguished from generalized anxiety, and signs of agoraphobia must be sought during the initial diagnosis. Drug therapy is effective for panic disorder, while agoraphobia may respond to drug therapy or to a range of cognitive therapies.

The epidemiology of panic symptomatology and agoraphobic avoidance.

In a random community survey of 1,498 urban adults age 18 to 64 years who were interviewed using the Diagnostic Interview Schedule (DIS), the lifetime prevalence of panic disorder was 2.2% +/- 0.4%. This was higher in women (3.4% +/- 0.7%) than in men (0.9% +/- 0.6%), and in those under the age of 45 years. Lifetime prevalence for panic attacks was 7.8% +/- 0.7%. Panic attacks and panic disorder had a similar distribution by age and sex, with higher rates in women than men, and also in the under 45 age groups. The panic symptomatology reported by those subjects with panic attacks was similar t

Mental health status among Puerto Ricans, Mexican Americans, and non-Hispanic whites.

Compared mental health characteristics of island Puerto Ricans to three groups from the Los Angeles Epidemiologic Catchment Area Study: Mexican American immigrants, U.S.-born Mexican Americans, and Non-Hispanic whites. The Diagnostic Interview Schedule was used to obtain both diagnostic and symptom scale information about affective disorders, alcohol abuse/dependence, somatization, phobic disorder, and psychotic disorder. Mexican American immigrants had the fewest mental health problems of all groups. Puerto Ricans had more somatization disorder, but less affective and alcohol disorders than U

Mirtazapine versus fluoxetine in the treatment of panic disorder.

Mirtazapine is an antidepressant whose side effect profile differs from that of first-line agents (selective serotonin reuptake inhibitors) used in the treatment of panic disorder. The present study compared the effect of mirtazapine and fluoxetine in the treatment of panic disorder in a double-blind, randomized, flexible-dose trial conducted with outpatients. After a 1-week single-blind placebo run-in, 27 patients entered an 8-week double-blind phase in which they were randomly assigned to treatment with either mirtazapine or fluoxetine. Both groups improved significantly in all but one effic

Imipramine treatment of panic disorder with agoraphobia: the second time around.

The purpose of the present study was to assess and compare the therapeutic effects of imipramine during the initial treatment and retreatment of panic disorder with agoraphobia patients. Seven women with panic disorder with agoraphobia who had shown a marked and stable response to imipramine (121.4 +/- 41.8 mg/day) during their initial treatment of 24 weeks were retreated with the same dose (125 +/- 40.8 mg/day) of imipramine when they relapsed, on average 3 months following discontinuation of the drug. Assessments included operationalized criteria for response and relapse and plasma drug conc

Relationship of panic, anticipatory anxiety, agoraphobia and global improvement in panic disorder with agoraphobia treated with alprazolam and exposure.

In a controlled trial of alprazolam and exposure in 154 patients with panic disorder with agoraphobia, relations between panic, anticipatory anxiety, and phobic avoidance were examined. The three symptoms were independent of one another at baseline and improved largely independently during treatment; only early improvement in avoidance predicted global improvement after treatment. Global improvement was more related to reduction of avoidance than a decrease in panics. Panic was not a valuable outcome measure in panic disorder with agoraphobia.

[Adaptol in the treatment of anxiety disorders in children with school maladaptation].

We examined 336 children, aged 7-14 years, with signs of school maladaptation (SM). Anxiety disorders were found in 167 (49.7%), including generalized anxiety disorder - 87 children (25.9%), phobic disorder - 40 children (11.3%), anxiety disorder - 14 (4.2%), social anxiety disorder - 26 (7.7%). These indices differed significantly from those in the comparison group of children without SM. The children with generalized anxiety disorder were treated with adaptol (1000 mg/d during 30 days). The clinical and psychological examination revealed the high efficacy of this drug. Adaptol was well-toler

Perceived Marital Support and Incident Mental Illness: Evidence from the National Comorbidity Survey.

Social support in marriage may be associated with reduced risk for mental illness. Past studies are limited by short follow up and a focus on depression. A two-wave nationally representative survey in the United States (n = 2,503) is used to examine whether social support in marriage is associated with the onset of each of four clusters of disorders-internalizing, externalizing, phobic, and bipolar-10 years later. Results indicate that higher levels of perceived marital support were protective against internalizing, fear, and bipolar disorders, and against incident externalizing disorders for

[Chemotherapy-associated clinical dynamics of nosogenic reactions in breast and ovarian cancer model].

OBJECTIVE: To study the clinical features, dynamics and factors of nosogenic reactions (NR) development in patients with breast and ovarian cancers at the stage of chemotherapy. MATERIAL AND METHODS: The study involved 35 patients who underwent chemotherapy. Clinical-psychopathological and psychometric methods were used to assess the mental state. RESULTS: We distinguished 3 clinical types of nosogenic reactions: anxiety-phobic (n=14, 40%), anxiety-depression (n=13, 37%) and dissociative reaction (n=8, 23%). It was discovered that nosogenic reactions reflect the dynamics of psychopathologica

Contributions of diagnostic, cognitive, and somatovisceral information to the prediction of fear ratings in spider phobic and non-spider-fearful individuals.

BACKGROUND: Physiological responding is a key characteristic of fear responses. Yet, it is unknown whether the time-consuming measurement of somatovisceral responses ameliorates the prediction of individual fear responses beyond the accuracy reached by the consideration of diagnostic (e.g., phobic vs. non phobic) and cognitive (e.g., risk estimation) factors, which can be more easily assessed. METHOD: We applied a machine learning approach to data of an experiment, in which spider phobic and non-spider fearful participants (diagnostic factor) faced pictures of spiders. For each experimental t

International reliability of a diagnostic intake procedure for panic disorder.

Test-retest diagnostic reliability interviews using the Upjohn version of the Structured Clinical Interview for DSM-III (SCID) were conducted with 72 patients at 13 international sites of the Cross-National Collaborative Panic Study. Agreement on the diagnosis of panic disorder was very good. For the subtypes (uncomplicated, with limited phobic avoidance, and agoraphobia), agreement was fair to good.

Anxiety Symptom Severity and Long-term Risk for Suicidal Behavior in Depressive Disorders.

OBJECTIVE: We have previously shown that the severity of anxiety symptoms during major depressive episodes is associated in a linear fashion with the proportion of weeks in depressive episodes during a subsequent 25 years of follow-up. Here, we test whether the negative prognostic implications of anxiety comorbidity extend to higher likelihoods of suicidal behavior. METHODS: We used the NIMH Collaborative Depression Study (CDS) cohort to test both the severity of eight anxiety symptoms and the presence or absence of four anxiety disorders as risk factors for suicide attempts or completions ov

An examination of levels of agoraphobic severity in panic disorder.

Based on the results of previous studies, several factors believed to be related to the development of agoraphobia were simultaneously assessed in 195 panic disorder patients (57 males, 138 females) with various levels of DSM-III-R-defined agoraphobia: none, mild, moderate, or severe. The four groups of patients significantly differed from each other on self-reports of phobic avoidance, although all the groups reported a similar type of panic attack. The four groups also did not significantly differ on catastrophic panic cognitions (e.g. fear of dying), fear of anxiety symptoms (anxiety sensit

Family study of agoraphobia. Report of a pilot study.

A family study of agoraphobia (n = 20), panic disorder (n = 20), and nonanxious controls (n = 20) showed the morbidity risk for all anxiety disorders to be 32% among first-degree relatives of agoraphobics, 33% among relatives of patients with panic disorder and 15% among relatives of controls. Relatives of agoraphobics were also shown to be at higher risk for alcohol disorders. Female relatives were found to be at greater risk for anxiety disorders, reflecting their increased susceptibility to these illnesses, and male relatives were at greater risk for alcohol disorders. The increased risk fo
