Multiple lentigines confined to a resolving psoriatic plaque.

Development of nevus spilus-like lentigines in zones previously affected by plaques of psoriasis has been described. Most cases have appeared following phototherapy; then it has been suggested that the mechanism involved could be an abnormal reaction to UV light. However, cases of multiple lentigines arising within resolving psoriatic in patients who had not received phototherapy or photochemotherapy have been described as a result of possible post-inflammatory hyperpigmentation. We report the case of patient who developed nevus spilus-like lentigines following therapy with topical calcipotrio…

Treatment of childhood psoriasis.

Psoriasis is a common disease in children and adolescents. Because of the chronic course of the disease, appropriate choice of therapy in particular stage of the disease, so-called rotation therapy, is of paramount importance. This article provides a review of therapeutic options for childhood psoriasis. Local therapy for psoriasis in children consists of corticosteroid preparations, calcipotriol, tars and dithranol, local retinoids, and local immunomodulators. Phototherapy (narrow band UVB, photochemotherapy PUVA baths) is now a part of psoriasis therapy in children. Systemic therapy retinoid…

Nail psoriasis.

Psoriatic nail involvement is common and accompanies skin lesions on the body surface. The occurrence of nail psoriasis has been reported in … lt to treat and respond slowly to therapy. Corticosteroids, 5-fluorouracil, systemic agents such as photochemotherapy, oral retinoids, and methotrexate, and nail avulsion are among the therapeutic approaches utilized. We offer suggested measures whereby patients may benefit in the prevention and control of their nail psoriasis.

Efficacy of 1, alpha 25-dihidroxyvitamin D (Calcitriol) in the treatment of psoriasis vulgaris: an open study.

Several reports have appeared in the literature suggesting that Vitamin D metabolites and analogues may be useful for the treatment of psori … g and after the trial. Further trials with calcitriol as an adjuvant therapy to topical steroids or photochemotherapy, as well as the development of topical Vitamin D analogues may be the future of this novel therapeutic alternative for psoriasis.

Progressive pigmented purpuric dermatosis: effect of psoralens and ultraviolet A irradiation.

The administration of 8 - methoxypsoralen combined with UV-A exposure (PUVA) was found to be effective in a case of progressive pigmented pu … following the maintenance phase of therapy. Various theories concerning the mode of action of oral photochemotherapy in PPPD are discussed in the light of these observations. This is the first report of a case of PPPD which was successfully treated by PUVA.

CD30-positive primary cutaneous anaplastic large cell lymphoma with coexistent pseudocarcinomatous hyperplasia.

CD30-positive primary cutaneous anaplastic large cell lymphoma (C-ALCL) is an indolent type of cutaneous lymphoma with favourable clinical prognosis. Pseudocarcinomatous hyperplasia (PCH) is a rare benign epithelial condition that can resemble invasive squamous cell carcinoma both clinically and histopathologically. PCH predominantly occurs in CD30-positive lymphoproliferative disorders. We report a 75-year-old woman with PCH in a multifocal C-ALCL located on the scalp and right retroauricular area, which rapidly responded to treatment with psoralen ultraviolet A photochemotherapy. Comprehensi…

Photodynamic therapy as an alternative treatment for relapsed or refractory mycosis fungoides: A systemic review.

Mycosis fungoides is the most common cutaneous T-cell lymphoma. It is characterized by slow progress over years to decades, developing from patches to infiltrated plaques, and sometimes to tumors. Therapies such as localized chemotherapy, photochemotherapy and radiotherapy are often employed when lesions of refractory or relapsing mycosis fungoides are resistant to conventional therapies. However, these methods have acute or chronic side effects and toxicity, which may accumulate with repeated and protracted treatment cycles. Photodynamic therapy is a promising, well-tolerated option for the t…

Treatment of cutaneous mastocytosis.

Therapy of cutaneous mastocytosis is directed towards skin and systemic symptoms due to mediator release and targeted on skin lesions. Symptomatic therapy of cutaneous mastocytosis involves agents that inhibit the release of mediators or antagonize H1 and H2 receptors such as antihistamines ketodifen and Aspirin. Disodium cromoglycate has no effect of the cutaneous symptoms of cutaneous mastocytosis. Skin-targeted therapies that lead to a resolution of the lesions of cutaneous mastocytosis are psoralen-photochemotherapy and topical corticosteroid therapy either by occlusion or intralesional in…

Photocarcinogenicity of psoralens used in PUVA treatment: present status in mouse and man.

There is good evidence that 8-methoxypsoralen is photocarcinogenic in psoriatic patients undergoing long-term photochemotherapy (PUVA) in the U.S.A. However, this conclusion has not been supported by two major European studies which have indicated that PUVA is a tumour promoter of damage initiated by other agents. Variation in PUVA treatment protocols in the U.S.A. and Europe may partly account for the different conclusions. There is much interest in the therapeutic potential of monofunctional psoralens. It is hoped that these may reduce long-term risk. Monofunctional and cross-linking psorale…

An unexpected spectrum of p53 mutations from squamous cell carcinomas in psoriasis patients treated with PUVA.

Photochemotherapy employing 8-methoxypsoralen and long-wavelength ultraviolet radiation (UVA, 320-400 nm) is widely used in the treatment of psoriasis. The photoactivation of psoralens in skin cells leads to formation of DNA photoadducts which may be responsible, at least in part, for the efficacy of these photochemotherapies. However, mutations arising from these adducts may also lead to the well-characterized increased incidence of squamous cell carcinoma. Mutations in the p53 tumor suppressor gene have been detected in many human cancers. To determine whether p53 mutations occur in squamous…

[The practice guideline 'Photo(chemo)therapy and systemic therapy in severe chronic plaque-psoriasis'].

The guideline for the treatment of adults with moderate to severe chronic plaque-type psoriasis vulgaris is based on published scientific evidence. The efficacy of the treatment, its safety, the side effects, the patients' discomfort, the patients' preference and the costs of treatment and follow-up are listed as criteria that play a role in the choice of treatment. Photo(chemo)therapy is the treatment of choice for severe forms of psoriasis. Phototherapy with ultraviolet-B light (UVB), especially narrow-band UVB, is preferred above photochemotherapy with ultraviolet-A light and psoralenes (PU…

Psoriasis: current perspectives with an emphasis on treatment.

An individualized treatment regimen is necessary for each patient with psoriasis because of the diverse nature of the disease. The manifestation of psoriasis, the severity and extent of the lesions, and the medical history and lifestyle of the patient are important factors that determine the selection of treatment, but in general therapies with the fewest side effects are preferred. First-line topical treatments are corticosteroids, calcipotriene, and tazarotene. If topical treatments are unsuccessful, phototherapy with ultraviolet B or photochemotherapy with psoralens plus ultraviolet A (PUVA…

Difference in bioavailability between two brands of 8-methoxypsoralen and its impact on the clinical response in psoriatic patients.

1 Several 8-methoxypsoralen (8-MOP) preparations are now available for photochemotherapy of psoriasis. To assess differences in clinical response two preparations marketed in The Netherlands, Oxsoralen capsules (Elder) and Meladinine tablets (Basotherm), were given to eight psoriatic patients. Serum concentrations and the minimal phototoxic dose (MPD) as a measure for clinical response were determined. 2 Bioavailability and serum peak concentrations were significantly higher for Maladinine in all patients and the MPD value favoured Maladinine in 5 of the 8 patients. The mean time of the serum …

Hyperpigmented small spots induced by long-term PUVA therapy. A clinical, light and electron microscopic study.

Among 308 photochemotherapy (PUVA)-treated patients, 15 psoriatics and 1 case of mycosis fungoides developed persistent disseminated hyperpigmented small spots at the trunk and limbs as a side effect of the therapy. The histological, histochemical and electron microscopical studies performed on 5 of the patients revealed a greatly increased number of melanocytes in the macules, hyperactivity of the melanocytes and increased transfer of pigment to dermis and keratinocytes. Moreover, binucleated cells were found as well as multifarious signs of melanocytic damage in varying degrees. Similar alte…

Ocular findings in cutaneous T-cell lymphoma.

Seventeen patients with cutaneous T-cell lymphoma undergoing an investigative treatment regimen using extracorporeal photochemotherapy underwent ophthalmologic examinations to investigate the ocular findings in this disease. The most frequent finding was seborrheic blepharoconjunctivitis, which was noted in 13 of 17 patients. Other common lid findings included cicatricial ectropion, meibomianitis, chalazia, and madarosis. Corneal and conjunctival disease was also increased in this group of patients. The syndrome of cutaneous T-cell lymphoma has recently been redefined, and a discussion of the …

Granuloma annulare (GA) is a benign, granulomatous disease with several clinical manifestations, which include localized, generalized, perforating, subcutaneous, patch, papular, and linear forms. We report a case of papular GA of the dorsal aspects of the hands that arose after repeated, direct trauma to the site of subsequent involvement. Although multiple etiologies for GA have been proposed, which include ultraviolet light, arthropod bites, trauma, tuberculin skin tests, viral infections, and PUVA photochemotherapy, the underlying pathogenesis of the disorder remains unclear. However, owing…
