Epilepsy in CHAD: Realities, constraints, and prospects.

Epilepsy is a significant public health problem. More than 50 million people worldwide live with epilepsy, and over three-quarters of them are in low- and middle-income countries. The situation in Chad regarding people with epilepsy is challenging to assess, starting from the shortage of scientific data, the inadequacy of technical facilities, the lack of human resources, and the inadequacy of government action. The Ministry of Health and Chadian Society of Neurology are looking forward to prioritizing epilepsy as well as improving the living conditions of persons with this disease. The epilep

The case for adjuvant BRAF-targeted therapy versus adjuvant anti-PD-1 therapy for patients with resected, high-risk melanoma.

The development of highly effective BRAF-targeted therapy and immune checkpoint inhibition for patients with advanced metastatic melanoma has transformed the treatment of this disease. More recently, these advances have moved into the resected, high-risk stage II and III settings. For patients with resected, BRAF-mutant stage III melanoma, there are no head-to-head data to support the use of BRAF-targeted therapy (specifically the combination of dabrafenib and trametinib) with either single-agent nivolumab or pembrolizumab. Because the relapse-free and distant metastasis-free survivals are sim

Structural Analysis of the German Self-Report Questionnaire for Anxiety and Depression in People With Disorders of Intellectual Development (SAD-IE).

BACKGROUND: Anxiety disorders and depression are among the most common mental disorders in people with disorders of intellectual development (DID). As many symptoms are not directly observable, diagnoses should not be based solely on third-party assessment. The aim of the study was the development and structural analysis of a self-report questionnaire (SAD-IE) for anxiety and depression in people with DID. METHODS: Based on the specific diagnostic criteria of DM-ID-2, a trial version of the SAD-IE with 49 items was constructed in plain language. After contacting N = 233 institutions across al

Family Engagement With Pediatric Anesthetic Records: A Qualitative Study of Provider-Guardian Communication and Record Design.

BACKGROUND: In recent years, health systems worldwide have increasingly expanded patient and guardian access to clinical records. While adult patient perspectives on anesthetic records have been explored, little is known about how pediatric guardians interpret and engage with their child's anesthetic documentation. AIMS: To explore how guardians of pediatric patients engage with their child's anesthetic record and to identify elements they perceive as meaningful, confusing, or emotionally impactful. METHODS: We conducted a qualitative study using semi-structured video conference interviews b

Integrating orthotic and prosthetic solutions into disaster risk management: a multimethod study protocol.

INTRODUCTION: Disasters often cause large numbers of injuries, including fractures, lacerations and amputations, which overwhelm emergency services. Orthotic and prosthetic solutions can provide timely primary care, reduce the need for surgery and prevent complications. A structured framework for preparedness and response is required to integrate these solutions into disaster risk management. This study aims to develop such a framework to enhance rehabilitation and reduce secondary injuries. METHODS AND ANALYSIS: This multimethod protocol comprises three phases. First, a systematic scoping re

TARGET Study: Intravenous Dexamethasone Palmitate for Postoperative Pain Prevention: Protocol for a Multicenter, Randomised Controlled Study.

INTRODUCTION: Inadequate postoperative pain control still hinders recovery, fuels opioid dependence, and promotes chronic pain. Although perioperative systemic dexamethasone offers anti-inflammatory and opioid-sparing benefits, its clinical success is unpredictable, and concerns over systemic adverse events persist. To address these limitations, dexamethasone palmitate-a novel lipophilic prodrug engineered as a nanoparticle emulsion-utilizes enhanced permeability and retention (EPR) effects to concentrate selectively at inflammatory sites, promising superior local anti-inflammatory and analges

Kidney and cardiovascular-protective benefits of combination drug therapies in chronic kidney disease associated with type 2 diabetes.

Given the substantial burden of chronic kidney disease associated with type 2 diabetes, an aggressive approach to treatment is required. Despite the benefits of guideline-directed therapy, there remains a high residual risk of continuing progression of chronic kidney disease and of cardiovascular events. Historically, a linear approach to pharmacologic management of chronic kidney disease has been used, in which drugs are added, then adjusted, optimized, or stopped in a stepwise manner based on their efficacy, toxicity, effects on a patient's quality of life, and cost. However, there are disad

Effectiveness of manual diaphragmatic techniques in adults with non-specific low back pain: a systematic review and meta-analysis protocol.

INTRODUCTION: Non-specific low back pain (NSLBP) is one of the leading causes of disability worldwide. Emerging evidence suggests that altered diaphragmatic function may be associated with lumbar pain, impaired trunk stabilisation and functional disability. Manual diaphragmatic techniques have been proposed as an intervention to modulate diaphragmatic tension, mobility and neuromyofascial relationships; however, their effectiveness has not yet been synthesised using rigorous systematic review methods. METHODS AND ANALYSIS: This protocol follows Preferred Reporting Items for Systematic Review

The real-life experiences of people living with overweight and obesity: A psychosocial perspective.

Understanding the lived experience of obesity, just like any other chronic outcome, is essential to addressing deep routed inequalities and stigma associated with obesity, as well as creating more inclusive and effective policies and healthcare. The psychosocial aspects of obesity are vast and as such the lived experience of obesity differs between individuals and groups. However, there are consistent psychosocial issues identified within empirical studies as well as lived experience accounts that require consideration and potential adaption in the design and delivery of care including weight

Calpastatin, a calpain specific inhibitor, reduce seizures in a mouse model of temporal lobe epilepsy.

Epilepsy is a chronic condition characterized by unpredictable and recurrent spontaneous seizures. In a previous study, we reported that pharmacological inhibition of calpain prevented epileptogenesis in the rat pilocarpine model. In this study, we demonstrate that transgenic overexpression of calpastatin, the endogenous inhibitor of calpain, reduces calpain activation and lessens seizure burden in the mouse intrahippocampal kainate model. Blockade of calpain activation was evidenced by a reduction in the generation of spectrin breakdown products, a hallmark of calpain activation. CAST overexp

A rare case report of apixaban-induced lichenoid eruption.

UNLABELLED: With recent increase in the use of direct oral anticoagulants (DOACs), several new cases of adverse drug reactions (ADRs) have been identified in pharmacovigilance surveys. These ADRs can result in significant mortality and morbidity if not identified and treated promptly. It is important for physicians to recognize that immunologically mediated delayed hypersensitivity reactions, although rare in occurrence, can have significant impact on patient's quality of life. To the best of our knowledge, we report the first case of lichenoid eruption associated with apixaban. We further pro

ENLIVEN study: Pexidartinib for tenosynovial giant cell tumor (TGCT).

Pexidartinib is the first approved medication in the USA for people with tenosynovial giant cell tumor (TGCT). The drug was approved based on the ENLIVEN study, which looked at pexidartinib (brand name, Turalio™), a medication taken by mouth (orally) for people with TGCT (also known as giant cell tumor of the tendon sheath [GCTTS] and pigmented villonodular synovitis [PVNS]) who are not able to have surgery because of the location and/or the size of the tumor. The study showed that pexidartinib is effective in treating people with TGCT because it shrunk the size of their tumors and improved th
