The need for strategies to prevent complications from diabetic neuropathy (DPN) is well recognized. However, foot-ankle exercise programs show weak to moderate evidence, and barriers to their implementation persist, including broad and facilitated access to exercise programs, which guarantee for equity. In this paper, we report for the first time the effectiveness of a web-based foot-ankle exercise program aiming to improve DPN-related outcomes, gait biomechanics and functional outcomes. Sixty-two participants with DPN were randomly allocated into the control group (CG; n = 31), which received

Hyperexcitable stretch reflexes are often not present despite of other signs of spasticity in people with brain lesion. Here we looked for evidence that increased resistance to length change of the plantar flexor muscle-fascicles may contribute to a reduction in the stretch reflex response in adults with cerebral palsy (CP). A total of 17 neurologically intact (NI) adults (mean age 36.1; 12 female) and 13 ambulant adults with CP (7 unilateral; mean age 33.1; 5 female) participated in the study. Subjects were seated in a chair with the examined foot attached to a foot plate, which could be move

Paclitaxel-induced peripheral neuropathic pain (PIPNP) is a common and debilitating adverse effect of chemotherapy, with limited effective treatments. Electroacupuncture (EA) has demonstrated analgesic potential in various pain models, but its efficacy and mechanisms in PIPNP remain incompletely understood. This study aimed to evaluate the analgesic effect of EA in a murine model of PIPNP and investigate whether its mechanism involves modulation of the TLR4/P2X7-NLRP3 signaling pathway. A PIPNP model was established in male C57BL/6 J mice by intraperitoneal injection of paclitaxel (cumulative

Continuous plantar-pressure monitoring is important for objective gait analysis and early detection of abnormal loading; however, many existing solutions remain laboratory-bound (force plates and instrumented walkways) or rely on costly in-shoe multilayer sensor arrays. Here, we developed and optimized a fully screen-printed pressure-sensing insole based on carbon-polymer nanocomposite layers, with an emphasis on manufacturability and process control to bridge the gap between proof-of-concept force-sensitive resistor (FSR)-based insoles and scalable printed-electronics manufacturing workflows.

Charcot medial column and midfoot deformities are associated with rocker bottom foot, recurrent plantar ulceration, and consequent infection. The primary goal of surgical intervention is to realign and stabilize the plantar arch in a shoe-able, plantigrade alignment. Different fixation devices, including screws, plates, and external fixators, can be used to stabilize the Charcot foot; however, each of these methods has substantial disadvantages. To assess the effectiveness of rigid, minimally invasive fixation of the medial column and midfoot, 8 cases of solid intramedullary bolt fixation for

INTRODUCTION: Most frequent injuries of the Upper limb are a distal radius fracture (DRF). If they aren't handled well, they might cause problems for a long time. There are several techniques to treat this issue, but volar locking plate fixation has become increasingly prevalent since it may restore anatomical alignment, provide stable fixation, and allow for early movement. This study aimed to evaluate the functional and radiological results of distal end radius fractures treated with volar locking plate fixation.
MATERIALS AND METHODS: In this prospective analytical investigation, 60 patien

In this study, we investigated the effects of subacute agomelatine (40 and 80 mg/kg) administration on chronic hyperglycemia, metabolic parameters, and pain perception in streptozotocin-induced diabetic rats. Fasting blood glucose measurements and oral glucose tolerance tests were performed to evaluate the effect of agomelatine on glycemia, while metabolic parameters were monitored using metabolic cages. Potential effect of agomelatine on diabetes-induced mechanical and thermal allodynia was evaluated using dynamic plantar aesthesiometer and warm plate (38 °C) tests, respectively. Additionally

A long metatarsal and/or metatarsophalangeal joint dislocation associated with a digital contracture is a surgical challenge. Without appropriate surgical correction, the patient will be predisposed to numerous complications, including persistent subluxation or dislocation, recurrent metatarsalgia, dorsiflexory contracture of the digit, transfer lesions, and inadequate pain relief. The results of the present surgical treatment options have varied, with the most common complication being a floating toe. The purpose of our study was to introduce a decompression, shortening, lesser metatarsal ost

Distal third tibial fractures associated with anterior soft tissue compromise are a predictor of more complications and poor prognosis. The study aimed to introduce the treatment of such fractures through the posterolateral approach. From March 2020 and January 2022, 32 patients with distal third tibial fractures were plated through the posterolateral approach due to concurrent closed anterior soft tissue compromise. There were 30 male and 2 female patients with the mean age of 33 years (range, 20-53 years). The reduction quality of diaphyseal fractures was good (n=30) and acceptable (n=2). Th

BACKGROUND: A dorsal bunion may occur in nonambulatory adolescents with cerebral palsy (CP) and a Gross Motor Function Classification System (GMFCS) level of IV or V. The deformity can cause pain, skin breakdown, and difficulty wearing shoes and braces. A consensus on the biomechanics and surgical management of dorsal bunions in persons with severe CP has not been established.
METHODS: This retrospective cohort study included 23 nonambulatory adolescents with CP, GMFCS level IV or V, and symptomatic dorsal bunions requiring surgery. The median age at surgery was 17 years, and the median follo

OBJECTIVE: Intermittent claudication aggravates physical function and is associated with an increased risk of death in patients with peripheral arterial disease (PAD). Previous studies on kinetic parameters (joint moment and power) of lower limbs in these patients have largely focused on the decline in the ankle plantar flexor moment and power at self-selected (SS) walking speed, which may not be an optimal condition to induce claudication pain. In the present study, we investigated the abnormalities in joint kinetic parameters in patients with PAD at both SS and at fast walking speeds.

CONTEXT: Excessive quadriceps contraction with insufficient hamstrings muscle cocontraction has been shown to be a possible contributing factor for noncontact anterior cruciate ligament (ACL) injuries. Assessing the relationships among lower extremity internal moments may provide some insight into avoiding muscle contraction patterns that increase ACL injury risk.
OBJECTIVE: To examine the relationships of knee-extensor moment with ankle plantar-flexor and hip-extensor moments and to examine the relationship between knee moment and center of pressure as a measure of neuromuscular response to

OBJECTIVE: Biomechanical studies in diabetic neuropathy have clearly demonstrated abnormal foot pressures, but information on other aspects of gait is limited. This study aimed to investigate and describe the forces transmitted through the foot during walking in diabetic subjects with varying degrees of peripheral neuropathy and to determine if abnormalities in these forces might contribute to the risk of plantar ulceration.
RESEARCH DESIGN AND METHODS: Subjects from the following groups were included: healthy control subjects (C); diabetic control subjects (D); subjects with diabetic neuropa

Alphaviruses, such as Chikungunya and Mayaro, are emerging arthropod-borne pathogens that particularly affect developing countries. Alphavirus infection has great clinical relevance and must be identified and treated properly, as the disease caused by these viruses has a characteristic chronic phase with joint pain that can last for several months or even years, significantly impacting the quality of life of the infected subjects. Animal models of arthritis caused by Alphaviruses allow the study of inflammatory response, bone loss, and tissue damage, providing biological information similar to

BACKGROUND: Malunited fractures of the metatarsals seldom need correction because the adjacent joints proximally and distally are not axially loaded but they may cause significant pain when a subcapital fracture is malunited too far in a plantar direction.
METHOD: Even if a metatarsal head is malunited too dorsally the neighboring head signals transfer metatarsalgia. Therefore, reorientation osteotomy with the intraoperative help of a minidistractor and stable fixation with a small locking plate is needed. Painful nonunion, especially of the proximal fifth metatarsal needs improvement of the
