Role of omental flaps in thoracic surgery complications: a narrative review.

BACKGROUND AND OBJECTIVE: The greater omentum is a highly vascularized and immunologically active organ with recognized regenerative and antimicrobial properties. Its application in pleural space complications has gained renewed interest with the expansion of minimally invasive harvesting techniques. This review summarizes the current literature regarding the role of omental flaps in the management of post-lobectomy complications. METHODS: This narrative review was conducted using a comprehensive search of MEDLINE (Via PubMed), Scopus, and Cochrane Central Register of Controlled Trials from d

Assessment of Treatment Modalities in Patients With Keloids: A Vancouver Scale Comparison.

BACKGROUND: Various treatment modalities are used to treat keloids. However, a comprehensive assessment comparing the effectiveness of these modalities in terms of patient-reported outcomes is warranted. OBJECTIVE: To assess and compare the effectiveness of various treatment modalities for keloids. METHODS: We assessed treatment outcomes in a cohort of 250 patients diagnosed with keloids. The patients were categorized into four treatment groups: triamcinolone acetonide (TA), TA + cryotherapy, TA + pulsed dye laser (PDL), and TA + cryotherapy + PDL. The Vancouver Scar Scale was used to evalua

The effect of fatigue on asymmetry between lower limbs in functional performances in elite child taekwondo athletes.

BACKGROUND: Inter-limb asymmetry above a certain threshold in functional performance indicates increased injury risk in sports. Fatigue has been found to increase bilateral asymmetry in lower-limb jumping performance among high-school and adult athletes, whereas this impact has not been examined in child athletes. This study aimed to examine the effect of fatigue on inter-limb asymmetry in functional performances in elite Taekwondo athletes aged between 9 and 11 years. METHODS: Performance of single-leg jumps, Star Excursion Balance Test (SEBT), and muscle (hamstring and gastrocnemius) flexib

Stiffness of the human foot and evolution of the transverse arch.

The stiff human foot enables an efficient push-off when walking or running, and was critical for the evolution of bipedalism1-6. The uniquely arched morphology of the human midfoot is thought to stiffen it5-9, whereas other primates have flat feet that bend severely in the midfoot7,10,11. However, the relationship between midfoot geometry and stiffness remains debated in foot biomechanics12,13, podiatry14,15 and palaeontology4-6. These debates centre on the medial longitudinal arch5,6 and have not considered whether stiffness is affected by the second, transverse tarsal arch of the human foot1

Footwear for self-managing knee osteoarthritis symptoms: protocol for the Footstep randomised controlled trial.

BACKGROUND: Knee osteoarthritis (OA) is a leading cause of musculoskeletal pain and disability globally, and abnormal knee loading is central to disease pathogenesis. Clinical guidelines recommend clinicians provide advice regarding appropriate footwear for people with knee OA, yet there is little research comparing the effects of different footwear on knee OA symptoms. Research suggests that wearing flat flexible shoes is associated with lower knee joint loads compared to stable supportive shoe styles. This two-arm pragmatic, comparative effectiveness randomised controlled trial will compare

[Flexible endoscope in thoracic surgery: CITES or cVATS?].

Early pain and persistent parietal disorders remains a major unresolved problem in thoracic surgery. Thoracotomy and the use of multiple ports in most Video Assisted Thoracic Surgery (VATS) procedures are the major cause of this persistent pain. For the last decade, a few publications describing the use of either single incision VATS and cervical thoracic approaches have been reported without significant results in comparison with current used techniques. Intercostals compression during surgery and early after by intercostals chest tube placement, are probably the major cause of postoperative

Bending properties, porosity, and ash fraction of black bear (Ursus americanus) cortical bone are not compromised with aging despite annual periods of disuse.

In many species, including humans, disuse causes an imbalance in bone remodeling that leads to increased bone porosity as a result of increased bone resorption and decreased bone formation. However, black bears (Ursus americanus) may not develop disuse osteopenia, to the extent that other animals do, during long periods of disuse (i.e. hibernation) because they maintain osteoblastic bone formation during hibernation, even though bone resorption is increased during hibernation. Black bears may also have a mechanism to rapidly and completely recover the bone lost (by increased resorption during

In vitro low-speed side collisions cause injury to the lower cervical spine but do not damage alar ligaments.

Whether injuries to the alar ligaments could be responsible for complaints of patients having whiplash injury in the upper cervical spine is still controversially discussed. It is known that these ligaments protect the upper cervical spine against excessive lateral bending and axial rotation movements. The objective of the present in vitro study was therefore to examine whether the alar ligaments or any other structures of the cervical spine are damaged in side collisions. In a specially designed acceleration apparatus, six human osteoligamentous cervical spine specimens were subjected to incr

Dorsiflexion metatarsal osteotomy for treatment of recalcitrant diabetic neuropathic ulcers.

Twenty diabetic patients underwent 22 dorsiflexion metatarsal osteotomies for treatment of chronic persistent or recurrent neuropathic forefoot ulcers. Mean duration of nonoperative treatment was 13 months. The procedure consisted of irrigation and debridement of the ulcer followed by basilar closing wedge metatarsal osteotomy performed through a dorsal approach. At follow-up, complete ulcer healing was noted in 21 cases (95%) at an average of 40 days postoperatively. Complications occurred in 15 cases (68%). The main problems encountered postoperatively were acute Charcot disease (32%) and de

An impact analysis of a flexible bat using an iterative solver.

Although technology has now infiltrated and prompted evolution in most mass participation sports, the advances in bat technology in such sports as baseball and cricket have been relatively minor. In this study, we used a simple finite element modelling approach to try to shed new light upon the underlying mechanics of the bat-ball impact, with a view to the future optimization of bat design. The analysis of a flexible bat showed that the point of impact that produced the maximum post-impact ball velocity was a function of the bat's vibrational properties and was not necessarily at the centre o

Acylthiourea oligomers as promising reducing agents for dimethacrylate-based two-component dental materials.

OBJECTIVES: Currently used thiourea-based two-component dental materials may release bitter compounds if they are not properly cured. To address this issue, the objective of this study was to evaluate the potential of acylthiourea oligomers as reducing agents for the development of self-cure composites. METHODS: Acylthiourea oligomers ATUO1-3 were synthesized via cotelomerization of the acylthiourea methacrylate ATU1 with butyl methacrylate. They were characterized by 1H NMR spectroscopy and size exclusion chromatography. Self-cure composites based on the redox initiator system cumene hydrope

Peptide/Peptoid Hybrid Oligomers: The Influence of Hydrophobicity and Relative Side-Chain Length on Antibacterial Activity and Cell Selectivity.

Previous optimisation studies of peptide/peptoid hybrids typically comprise comparison of structurally related analogues displaying different oligomer length and diverse side chains. The present work concerns a systematically constructed series of 16 closely related 12-mer oligomers with an alternating cationic/hydrophobic design, representing a wide range of hydrophobicity and differences in relative side-chain lengths. The aim was to explore and rationalise the structure-activity relationships within a subclass of oligomers displaying variation of three structural features: (i) cationic side
