Variations in treatment of postmenopausal osteoporosis.

The purpose of this study was to determine trends and differences between various medical specialties with regard to the treatment of postmenopausal osteoporosis. The method used was a cross-sectional survey, and the authors' sample was composed of 116 physicians. There was an overall lack of consensus on choice of treatment for postmenopausal osteoporosis. Treatment varies with the individual physician without any strict adherence to a formal therapeutic protocol. Calcium was the treatment modality most widely used by all the specialties. Gynecologists and endocrinologists show the greatest u

Postmenopausal osteoporosis: the role of estrogens.

In 1980, the combined incidences of vertebral and hip fractures necessitating hospitalization of women living in Knox County, Tennessee nearly doubled each five years after the age of 50. These data led us to review the pathogenesis, diagnostic methods, and therapeutic measures used to control or minimize the occurrence of postmenopausal osteoporosis. Postmenopausal diminution in estrogen metabolism appears to be the most important cause of osteoporosis. Other contributory factors include an early menopause, low body weight, lack of sufficient exercise, poor nutrition, and excessive use of alc

Sodium excess in postmenopausal osteoporosis.

The sodium excess (Naes) which represents the nonexchangeable sodium compartment of bone was calculated from the total body sodium and total body chlorine of 95 women with vertebral crush fracture syndrome. No evidence was found for a subpopulation with increased or decreased Naes or Naes:TBCa ratio (TBCa is total body calcium which reflects total skeletal mass). Correlations in the osteoporotic women were found between the Naes and serum alkaline phosphatase (r = 0.31), PTH (r = 0.53), urinary hydroxyproline excretion (r = 0.42) and serum 250H vitamin D (r = -0.32). These data suggest that th

Problems related to postmenopausal women.

The most consistently troublesome symptom of the menopause is the hot flush. It can occur without warning or may be triggered off by emotion, exercise or change of environment or temperature. It is estimated that about 50% of women suffer from hot flushes, but only 5-10% of these seek medical advice. The cause is ascribed to excessive production of follicle-stimulating hormone. There is an increased sensitivity to the effect of locally released dilator substances in the peripheral vasculature.
