Why America's prisons are shrinking and racial gaps are closing.

Why have incarceration rates and racial disparities fallen in the USA? Using data on recorded crimes, arrests and prison admissions from 2000 to 2019, we decompose declines in imprisonment by race and offence category. Whereas the rise of mass incarceration primarily reflected increased punitiveness in policing and case processing, recent declines in imprisonment for violent and property offences can be attributed almost exclusively to reductions in recorded crime rates, particularly to declines in violence among Black Americans. For drug offences, however, the pattern appears different. There

Prescription patterns of benzodiazepines and Z-drugs in Berlin prisons.

BACKGROUND: Benzodiazepines and Z-drugs are commonly prescribed medications in psychiatric treatment. Prisoners are a unique patient population due to their elevated prevalence of substance abuse and psychiatric morbidity. Given the limited data about the medical treatment for prisoners, this study aims to provide an initial exploratory overview of the prescription patterns of benzodiazepines and Z-drugs in Berlin prisons. MATERIALS AND METHODS: A data set was compiled by extracting information from the documentation systems of all seven Berlin prisons. All inmates with a prescription for ben…

Resisting methadone: criminal subculture and methadone implementation in Moldovan prisons.

PURPOSE: Maintenance treatment with methadone (MMT) is globally recommended as a key strategy for HIV prevention and addiction treatment in prisons, yet implementation remains limited. Moldova offers a rare exception, where MMT has been available in prisons for over two decades, though uptake remains low. This study aims to examine how criminal subcultures, widespread in many Eastern European prisons, shape access to and perceptions of MMT. DESIGN/METHODOLOGY/APPROACH: A total of 35 in-depth qualitative interviews were conducted in two men's prisons near Chišinău with participants of varied s…

Best practice guidelines for viral hepatitis service delivery in prisons.

Viral hepatitis constitutes a major health burden among people living in prison globally. There is poor access to prison-based viral hepatitis services, and people living in prison are rarely prioritised for care, undermining global elimination efforts. This Review is a narrative summary of the first Global Guidelines for Viral Hepatitis Service Delivery in Prisons, which included a systematic review and an expert-led GRADE process to develop best practice recommendations for viral hepatitis management in prisons. 703 articles were included, underpinning the development of 30 recommendations a…

Playing "mental judo": Mapping staff compassion in Canadian federal prisons.

Prisons are inherently emotional environments where both staff and prisoners engage in a continuous process of emotion management while working and living in carceral spaces. This paper explores how Correctional Service of Canada (CSC) values and norms shape how predominantly nonuniformed staff manage compassion inside the prison environment. This includes when and to whom they are allowed to express compassion, when they need to hide or suppress the expression of compassion, and how expressing compassion toward prisoners can elicit feelings of disgust among some staff. We argue that, in the e…

Cervical Cancer Screening and Follow-Up Practices in U.S. Prisons.

INTRODUCTION: The objective of this study is to better understand cervical cancer screening and follow-up practices in U.S. prisons. METHODS: A 29-question survey examining cervical cancer screening practices, education, and facility/patient characteristics was disseminated to state-prison medical directors. RESULTS: A total of 70% (35/50) of state medical directors completed the survey between August 2021 and January 2022. All prison systems provided cervical cancer screening both at intake and specified intervals. A total of 36% provided colposcopy on site, and 9% performed excisional proc…

Excess mortality in U.S. prisons during the COVID-19 pandemic.

U.S. prisons were especially susceptible to COVID-19 infection and death; however, data limitations have precluded a national accounting of prison mortality (including but not limited to COVID-19 mortality) during the pandemic. Our analysis of mortality data collected from public records requests (supplemented with publicly available data) from 48 Departments of Corrections provides the most comprehensive understanding to date of in-custody mortality during 2020. We find that total mortality increased by 77% in 2020 relative to 2019, corresponding to 3.4 times the mortality increase in the gen…

Pooling Sputum Samples for Efficient Mass Tuberculosis Screening in Prisons.

BACKGROUND: Although systematic tuberculosis screening in high-risk groups is recommended by the World Health Organization (WHO), implementation in prisons has been limited due to resource constraints. Whether Xpert Ultra sputum pooling could be a sensitive and efficient approach to mass screening in prisons is unknown. METHODS: In total, 1280 sputum samples were collected from incarcerated individuals in Brazil during mass screening and tested using Xpert G4. We selected samples for mixing in pools of 4, 8, 12, and 16, which were then tested using Ultra. In each pool, a single positive sampl…

Ending Restrictive Housing in Prisons for People With Mental Disorders.

A recent federal investigation of prisons in Massachusetts has presented findings on the criminal legal system's continued failure to provide humane care for incarcerated individuals with mental disorders. The findings show that people in mental health crisis are placed in restrictive housing, where they lack monitoring and mental health resources-a practice that has led to frequent instances of self-harm and suicide. Since the investigation, Massachusetts has vowed to stop its use of restrictive housing. Skepticism remains about whether the state's decision will result in meaningful change in…

Needle and Syringe Programs in Prisons: Victoria's "Problematic" Policy Position.

With high rates of blood-borne virus infection in Australian prisons, a needle and syringe program (NSP) stands ready to deliver similar benefits to its community counterpart. Supplying sterile injecting equipment and safe-use guidance in prisons could improve prisoner health outcomes and the safety of the corrections system. Yet, despite the protestations of public health experts and the recommendations of State, national, and international bodies, Australian States and Territories refuse to implement prison-based NSPs, with serious consequences for prisoner health. This article focuses on th…

Shackling and pregnancy care policies in US prisons and jails.

OBJECTIVES: The number of incarcerated women in the United States has risen exponentially. Many are of childbearing age with 3-4% being pregnant at intake. Despite the need for comprehensive pregnancy-related health care in prisons and jails, there is no oversight that requires adherence to the established standards. The objective of this study was to assess prison and jail pregnancy policies and practices with an emphasis on restraint use and compliance with anti-shackling legislation. METHODS: We conducted a survey of 22 state prisons and six jails, including the five largest jails, from 20…

The results were also categorized according to age, sex, time spent in prison, background and academic levels. We also analysed the differences between results in healthcare quality according to socio-demographic variables. RESULTS: 84.87% of patients show satisfa…

Implementing prescribing safety indicators in prisons: A mixed methods study.

AIMS: To examine the prevalence of potentially hazardous prescribing in the prison setting using prescribing safety indicators (PSIs) and ex … gh intraprofessional collaboration. CONCLUSION: We successfully implemented a suite of PSIs into 2 prisons, identifying those with higher prevalence values as intervention targets. When appropriately resourced and integrated into staff workflow, PSI searches may support prescribing safety in prisons.
