ASIPP Guidelines for Sedation and Fasting Status of Patients Undergoing Interventional Pain Management Procedures.

Many of the patients undergoing interventional procedures have daily regimens of medications including analgesics, muscle relaxants, and other drugs that can have significant additive/synergistic effects during the perioperative period. Further, many patients also present with comorbid states, including obesity, cardiovascular, and pulmonary disease. Consequently, in the perioperative period, a significant number of patients have suffered permanent neurologic injury, hypoxic brain injury, and even death as a result of over sedation, hypoventilation, and spinal cord injury. In addition, physici

Administration of conscious sedation by a neuroradiology team during percutaneous vertebroplasty and spinal biopsy procedures.

INTRODUCTION: Percutaneous vertebroplasty, i.e. the consolidation of a vertebral body with polymethylmethacrylate, is a safe and effective image-guided technique increasingly used as a treatment option for different pathologic conditions, mainly vertebral body fractures secondary to osteoporosis, hemangiomas and metastasis. The procedure, although minimally invasive, could be painful and is better tolerated if a conscious sedation regimen is added to local anesthesia. An anesthesiologist usually performs the sedo/analgesia, but frequently, he is not available in our angiography unit, so we hav

Sedation in uncooperative children undergoing dental procedures: a comparative evaluation of midazolam, propofol and ketamine.

Dentists usually face a common problem dealing with pediatric patients due to their high levels of anxiety and fear, associated with dental procedures. Such children are usually managed by various pharmacological methods. The efficacy and safety of conscious sedation, using intravenous short acting group of drugs (midazolam, propofol and ketamine) in uncooperative children, requiring oral rehabilitation was thus evaluated in this study. A total of 30 uncooperative children, aged 3-6 years, belonging to ASA I, II category formed the study group. The efficacy of the three group of drugs was eval

Development of a sedation policy for upper GI endoscopy based on an audit of patients' perception of the procedure.

OBJECTIVE: To assess patients' perception of upper GI endoscopy, both with and without sedation, and based on this information to propose a sedation policy for the procedure. METHODS: A prospective audit of the experience of patients undergoing upper GI endoscopy both with and without sedation. RESULTS: Of the 500 patients in the study, 237 had endoscopy with sedation and 263 with local anaesthetic throat spray. Sedated patients found the procedure more tolerable, but most unsedated patients felt the extra discomfort was more than compensated for by other advantages. 87% of those patients se

Periprocedural rescue antiemetic administration during radiologic procedures performed under conscious sedation.

BACKGROUND: This study aimed to determine the incidence and clinical predictors of periprocedural rescue antiemetic administration during radiologic procedures performed under conscious sedation. METHODS: We performed a retrospective review of adult patients who underwent radiologic procedures under conscious sedation with midazolam and/or fentanyl between May 6, 2018, and December 31, 2024. Rescue antiemetics was defined as the use of an antiemetic during the procedure or recovery period. A modified Apfel score (excluding need for postprocedural opioids) was calculated. Logistic regression w

Sedation applications in pedodontics procedures: which one should we choose?: a retrospective analysis.

BACKGROUND: The use of sedation in the field of pediatric dentistry is becoming increasingly prevalent. While general anesthesia is perceived as a safer alternative, the appropriate use of sedatives, in conjunction with a well-trained team, can effectively mitigate the risks associated with sedation. Our study aims to identify safe drug combinations for prolonged sedation. METHODS: This study was conducted in the Operating Room of the Faculty of Dentistry at Akdeniz University. It encompassed 126 patients, classified as ASA 1-2, aged between 3 and 12 years, who underwent dental treatment with

Sedation and Analgesia for Intubation, LISA, and INSURE Procedures in Israeli NICUs: Caregivers' Practices and Perspectives.

Background/Objectives: Early pain exposure in newborns is linked to negative short- and long-term outcomes. Preterm infants often require endotracheal intubation for mechanical ventilation or brief laryngoscopy for surfactant administration via Less Invasive Surfactant Administration (LISA) or Intubation-Surfactant-Extubation (INSURE). While premedication before intubation is well-studied, data regarding premedication for LISA/INSURE are limited. We aimed to explore premedication practices for intubation and LISA/INSURE procedures across Neonatal Intensive Care Units (NICUs) in Israel. Methods

Remimazolam versus propofol for sedation in gastrointestinal endoscopic procedures: a systematic review and meta-analysis.

BACKGROUND: Propofol has a favourable efficacy profile in gastrointestinal endoscopic procedures, however adverse events remain frequent. Emerging evidence supports remimazolam use in gastrointestinal endoscopy. This systematic review and meta-analysis compares remimazolam and propofol, both combined with a short-acting opioid, for sedation of adults in gastrointestinal endoscopy. METHODS: We searched MEDLINE, Embase, and Cochrane databases for randomised controlled trials comparing efficacy-, safety-, and satisfaction-related outcomes between remimazolam and propofol, both combined with shor

Usefulness of deep sedation with intravenous dexmedetomidine and midazolam in cardiac catheterization procedures for pediatric patients.

BACKGROUND: Dexmedetomidine (DEX) is a highly selective alpha 2 receptor agonist that has the advantage of causing less respiratory depression than other sedative agents. We evaluated the add-on effects of DEX on sedation among pediatric patients who received midazolam and pentazocine during cardiac catheterization. METHODS: 120 cardiac catheterization procedures in 110 patients under deep sedation at Department of Pediatrics, Kanazawa University Hospital from January 2013 to August 2018: 63 procedures without DEX (i.e., non-DEX group) and 57 procedures with DEX (i.e., DEX group). Intravenous

Nasopharyngeal Tube Effects on Breathing during Sedation for Dental Procedures: A Randomized Controlled Trial.

WHAT WE ALREADY KNOW ABOUT THIS TOPIC: Dental procedures under sedation can cause hypoxic events and even death. However, the mechanism of such hypoxic events is not well understood. WHAT THIS ARTICLE TELLS US THAT IS NEW: Apnea and hypopnea occur frequently during dental procedures under sedation. The majority of the events are not detectable with pulse oximetry. Insertion of a nasal tube with small diameter does not reduce the incidence of apnea/hypopnea. BACKGROUND: Intravenous sedation is effective in patients undergoing dental procedures, but fatal hypoxemic events have been documented.

Cognitive aids: 'a must' for procedures performed by multidisciplinary sedation teams outside the operation room?

The human brain might not perform optimally during stressful situations. Cognitive aids can help in such situations to carry out all necessary treatment steps in a correct order. We present the case of a severe anaphylactic reaction during a percutaneous radiological intervention to drain an echinococcosis cyst on the radiology suite outside the operation room (OR), in which cognitive aids were successfully used to optimise patient care by a multidisciplinary team. Cognitive aids do not replace experience and skills of the individual caregivers, but can be invaluable tools for multidisciplinar

Deep sedation with methohexital or thiamylal with midazolam for invasive procedures in children with acute lymphoblastic leukemia.

Pediatric oncology/hematology patients, especially those with acute lymphoblastic leukemia (ALL), often undergo repeated painful invasive procedures. Deep sedation, mandatory for these procedures in young children, can reduce patient anxiety and get their compliance during procedures. This study assessed clinical experience of employing methohexital or thiamylal with midazolam as sedative for elective invasive procedures in children with ALL. Between November 1997 and March 2004, 20 out of 33 ALL children received deep sedation after evaluation, mainly because of relatively young age (mean age

Adaptive randomised controlled non-inferiority multicentre trial (the Ketodex Trial) on intranasal dexmedetomidine plus ketamine for procedural sedation in children: study protocol.

INTRODUCTION: Up to 40% of orthopaedic injuries in children require a closed reduction, almost always necessitating procedural sedation. Intravenous ketamine is the most commonly used sedative agent. However, intravenous insertion is painful and can be technically difficult in children. We hypothesise that a combination of intranasal dexmedetomidine plus intranasal ketamine (Ketodex) will be non-inferior to intravenous ketamine for effective sedation in children undergoing a closed reduction. METHODS AND ANALYSIS: This is a six-centre, four-arm, adaptive, randomised, blinded, controlled, non-…
