Investments in basic public infrastructure and their effects on economic growth in a developing country: The case of Cameroon.

The literature review conducted in this study indicates that there is no unanimity regarding the definition of infrastructure and its impact on growth. While some economists argue that focusing on logistical infrastructure, such as transportation, telecommunications, water, and energy, would be beneficial for governments, others emphasise the importance of social infrastructure, such as health and education. Given the diverse range of opinions, further investigation is required, particularly in the context of Cameroon's infrastructure. The objective of this article is to evaluate the impact of

Challenges and Opportunities for Tribal Waters: Addressing Disparities in Safe Public Drinking Water on the Crow Reservation in Montana, USA.

Disparities in access to safe public drinking water are increasingly being recognized as contributing to health disparities and environmental injustice for vulnerable communities in the United States. As the Co-Directors of the Apsaálooke Water and Wastewater Authority (AWWWA) for the Crow Tribe, with our academic partners, we present here the multiple and complex challenges we have addressed in improving and maintaining tribal water and wastewater infrastructure, including the identification of diverse funding sources for infrastructure construction, the need for many kinds of specialized exp

Assessing the feasibility of solar-powered digital health infrastructure in rural and peri-urban Kenya: a mixed-methods study.

BACKGROUND: Health systems in low- and middle-income countries (LMICs) face persistent infrastructure constraints, particularly unreliable electricity and limited digital connectivity, that hinder the effective implementation of digital health interventions. These challenges are especially pronounced in rural and peri-urban settings, where facilities rely on unstable power sources and fragmented information systems. Although digital health technologies can strengthen health systems and improve care delivery, their effectiveness depends on reliable infrastructure. Integrating renewable energy w

Developing Training in Response to High-Consequence Infectious Diseases and Preparedness Measures for the Future.

In recent history, outbreaks of high-consequence infectious diseases (HCIDs) have raised health security concerns among the global community. As HCIDs continue to emerge, public health systems around the world experience the burden of implementing adequate preparedness and response measures to ensure the safety and security of their populations. HCID outbreak response efforts have highlighted the need for specialized training in safety and infection prevention and control for frontline workers who may encounter ill patients. From Access to Outcomes: A Practical Approach to Strengthen Rural Health Systems.

Durable improvements in rural population health require stronger enabling conditions, including governance, workforce, payment, and data infrastructure, rather than a narrow focus on access to health care. We synthesized empirical evidence and health systems research published through 2025, emphasizing US studies of rural health outcomes, primary care, prevention, workforce, and payment models. Rural health systems are heterogeneous, and single interventions do not produce equal gains across communities. Evidence supports integrated approaches that combine community-wide prevention, team-based

From behavioural insight to system design: Reframing the role of psychology in public health.

Contemporary public health challenges, including non-communicable diseases, persistent inequalities, misinformation, and complex organisational systems, are fundamentally shaped by behavioural, social, and psychological processes. Although this is increasingly recognised, psychological science remains unevenly integrated into public health and is often confined to individual-level interventions rather than embedded in system design, governance, and policy. A more fundamental shift is required. Psychology should be understood not as an adjunct to public health but as one of its foundational dis

Translational medicine policy issues in infectious disease.

The European Academies Science Advisory Council has published a series of reports on infectious disease policy issues, analyzing priorities for building the science base as part of public health strategy. Among current challenges facing the European Union are the needs to tackle antibiotic resistance, promote vaccine innovation, prepare for the emergence of novel zoonoses, and integrate research approaches to human and animal health. The scientific community must help public policy-makers to address the organization, balance, and sustainability of research funding and infrastructure; encourage

The importance of establishing a national health security preparedness index.

Natural disasters, infectious disease epidemics, terrorism, and major events like the nuclear incident at Fukushima all pose major potential challenges to public health and security. Events such as the anthrax letters of 2001, Hurricanes Katrina, Irene, and Sandy, severe acute respiratory syndrome (SARS) and West Nile virus outbreaks, and the 2009 H1N1 influenza pandemic have demonstrated that public health, emergency management, and national security efforts are interconnected. These and other events have increased the national resolve and the resources committed to improving the national hea

Implementation factors shaping British Columbia's drug decriminalization pilot: A systematic review with narrative synthesis.

BACKGROUND: In January 2023, British Columbia (BC) became the first Canadian province to implement a legally sanctioned drug decriminalization policy, removing criminal penalties for adults possessing 2.5 g or less of opioids, cocaine, methamphetamine, and MDMA. Introduced as a three-year pilot, it aimed to reframe substance use as a public health issue, reduce stigma, and improve health and social service engagement. Criminal penalties were reintroduced for drug possession in most public spaces in May 2024, and the pilot ended in January 2026. Its termination has been interpreted as policy fa

Public health in a fracturing world: the erosion of the international rules-based order.

One of the greatest threats to public health today is not any single disease, environmental hazard, or conflict, but the erosion of the international rules-based order that enables countries to cooperate in addressing shared risks. Over the past decade, public health has faced increasingly interconnected challenges, including climate change, conflict, pollution, emerging infectious diseases, food and water insecurity, mass displacement, and misinformation. These threats are amplified by fragile governance, growing inequality, and declining trust in institutions. Public health depends fundament

Building the Foundations for Trustworthy AI in Pediatrics.

Artificial intelligence (AI) adoption in health care is rapidly accelerating, with pediatrics positioned to benefit from improvements in quality, efficiency, and access to care. Recent frameworks have articulated ethical commitments for trustworthy AI in pediatrics, but less attention has been paid to the foundational infrastructure required to achieve them. In this article, we examine the challenges that emerge at the intersection of AI and pediatrics: unique challenges that AI poses for pediatric care, including nonintuitive errors, impacts on clinician cognition, and new infrastructure requ

Public Health Under Attack: Continuity, Discontinuity, and History.

How unprecedented is the current backlash against the public health enterprise? In this article, the author explores previous attacks against public health practice in three exemplary domains: mass vaccination programs, air pollution control, and occupational safety. The author argues that in all three, public health was remarkably durable throughout the 20th century, and that most controversies over its powers-or even direct onslaughts from hostile elected officials-have failed to overturn long-standing practices or institutions, even if implementation may be altered for the worse. Once publi

Mpox in East Africa: Learning from COVID-19 and Ebola to Strengthen Public Health Responses.

The Africa Centers for Disease Control and Prevention declared mpox a Public Health Emergency of Continental Security (PHECS) in Africa. African public health systems have moved to mobilize a response against a backdrop of inherent significant challenges. With this commentary, we discuss how lessons from past public health emergencies, particularly COVID-19 and Ebola outbreaks, have prepared the region for improved disease surveillance, rapid response strategies, and effective public health communication and how these lessons can be applied to the mpox response, emphasizing the importance of s

Who Counts, Who's Accountable: Independent Scholarship and the Future of Carceral Mortality Oversight.

Deaths in jails and prisons are rising, yet the federal government lacks a reliable system to track or prevent them. This essay examines the Department of Justice's failure to implement the Death in Custody Reporting Act and produce accurate and timely data on deaths behind bars. We call for transferring responsibility for carceral mortality monitoring to public health systems with capacity to identify multilevel drivers of death and support interventions that promote prevention and accountability. Shifting this responsibility affirms the value of lives lost and moves surveillance from error-p

Infrastructural requirements for local implementation of safety policies: the discordance between top-down and bottom-up systems of action.

BACKGROUND: Safety promotion is planned and practised not only by public health organizations, but also by other welfare state agencies, private companies and non-governmental organizations. The term 'infrastructure' originally denoted the underlying resources needed for warfare, e.g. roads, industries, and an industrial workforce. Today, 'infrastructure' refers to the physical elements, organizations and people needed to run projects in different societal arenas. The aim of this study was to examine associations between infrastructure and local implementation of safety policies in injury prev
