Pyogenic flexor tenosynovitis complicated by necrotizing soft tissue infection in postpartum: A case report.

INTRODUCTION: Pregnancy and the immediate postpartum are periods of immune reconstitution inflammatory syndrome (IRIS), exposing patients to an increased risk of severe infections. Among these, necrotizing soft tissue infection (NSTI) is a rare but serious, life- and function-threatening condition. CASE REPORT: We report the case of a 31-year-old patient three weeks postpartum, admitted for acute right hand pain with septic signs. The initial clinical picture, typical of a pyogenic flexor tenosynovitis of the ring finger, rapidly evolved into NSTI of the palm. Despite urgent medical and surgi

[Examination on maternal morbidity and mortality during delivery and puerperium (author's transl)].

Our data show that it possible to reduce the maternal morbidity - mainly by reduction of the severe complications following caesarean sections. The higher maternal risk during and after operative delivery (forceps, caesarean section) should prompt the physician to analyse the indication and frequency of operative interventions within certain periods of time.

Factors Affecting Postpartum Infection: A Systematic Review.

BACKGROUND: Puerperal infection is used to describe any bacterial infection of the reproductive tract after delivery. Identifying the factors affecting postpartum infections can reduce the risk and complications of such factors and postpartum maternal mortality. OBJECTIVE: This structured study was designed to evaluate factors affecting postpartum infections. METHODS: In this study, after selecting Scopus, PubMed, SID, and Web of Science electronic databases, all observational studies (cohort and case-control) available and published in Farsi and English to investigate factors affecting post…

Postpartum invasive group A streptococcal disease in the modern era.

To describe the clinical features of individuals hospitalized for postpartum invasive group A Streptococcus (GAS) infection, a retrospective, population-based study of hospitalized patients in the state of Florida was conducted. Cases of postpartum invasive GAS infection (occurring within 42 days of delivery) were compared to women with other manifestations of invasive GAS disease with respect to their age at the time of admission. Four cases of postpartum invasive GAS infection were detected in this population, yielding a prevalence of 1.6% (4/257) of postpartum disease in this invasive GAS i

Pathogenic lower genital tract organisms in HIV-infected and uninfected women, and their association with postpartum infectious morbidity.

OBJECTIVES: To determine the prevalence of vaginal pathogens during pregnancy and their impact on postpartum infectious morbidity among antiretroviral-naïve HIV-infected, and HIV-uninfected, women. METHODS: Vaginal swabs were obtained during early labour by speculum examination prior to digital vaginal examination, and sent for microscopy and culture. Women were assessed for infectious complications within 24 - 72 hours of delivery, and up to 2 weeks postpartum. RESULTS: Laboratory results were available for 801 women who delivered vaginally (418 HIV infected and 383 uninfected). The baselin

Massive septic pelvic thrombophlebitis.

BACKGROUND: Septic pelvic thrombophlebitis is a major complication of endometritis. The thrombi commonly occur in the uterine and/or ovarian veins and may extend into the inferior vena cava. CASE: Following vaginal delivery, a 19-year-old woman, gravida 2, developed postpartum septic pelvic thrombophlebitis extending from the right ovarian vein up to the diaphragm and down to the femoral vein. The patient was treated successfully with heparin and antibiotics, and eventually was discharged on oral anticoagulants for an extended period. Follow-up revealed complete resolution of the thrombus. Postpartum maternal group B streptococcal meningitis.

Maternal group B streptococcal (GBS) meningitis is rare, with only four cases previously reported in the literature. In this review a fifth case of postpartum GBS meningitis is presented. The five cases are compared with 34 cases of GBS meningitis in nonparturient adults. All cases of maternal GBS meningitis followed a vaginal delivery. The mean age of patients who had GBS meningitis outside the perinatal setting was 55 years, and most of these individuals had associated illnesses or frank immunosuppression; the mortality rate in this group was 23%. In contrast, all five patients with maternal
