Cytokine and endothelial damage in pulsatile and nonpulsatile cardiopulmonary bypass.

Recently, several types of centrifugal pumps have been widely used as the main pumps for cardiopulmonary bypass (CPB). However, according to the results of our experimental studies, after cardiogenic shock, pulsatile flow was effective in maintaining the functions and microcirculations of end organs, especially those of the liver and kidney. To estimate the effectiveness of pulsatility during CPB, cytokine and endothelin and other metabolic parameters were measured in clinical pulsatile and nonpulsatile CPB cases. From March to May 1997, CPB was performed in 18 elective cases (14 ischemic and …

The Effect of CO2 Pressure and Flow Variation on Carbon Particles Spread During Pneumoperitoneum: An Experimental Study.

Background: A correlation between atypical recurrences and minimally-invasive surgery has been suggested in case of urothelial cancer; however, few data are available on the role of pneumoperitoneum in terms of gas flow and intra-abdominal pressure. The objective of the study is to analyze the impact of CO2 pneumoperitoneum variation on an inert material as surrogate of neoplastic cells. Material and Methods: We designed an experimental model mimicking pneumoperitoneum in three settings: sealed flow (no leakage), pulsatile flow (alternating efflux and influx), and continuous flow (AirSeal® ins…

Low pulsatility signals through the orbits.

BACKGROUND AND PURPOSE: Low pulsatility signals (LPS) on transcranial Doppler ultrasonography are detected (1) with arteriovenous malformations, (2) distal to hemodynamically significant stenosis, and (3) in venous structures. We describe focal LPS in the territory of the internal carotid artery siphon that do not represent any of the above conditions. METHODS: We performed retrospective and prospective reviews of transcranial Doppler studies on 3225 patients over 5 years. Clinical and radiological data of all patients with focal LPS were extracted. LPS was defined as a focal signal identifie

Acquired Von Willebrand Syndrome and Blood Pump Design.

The existence of acquired von Willebrand syndrome (AVWS) in patients with continuous flow left ventricular assist devices (LVADs) is well documented and has been verified by numerous investigators. AVWS has not been observed to occur in pulsatile devices such as the SynCardia total artificial heart (TAH), the HeartMate XVE, and the Thoratec pulsatile ventricular assist device (PVAD) used as a single pump. AVWS can also occur in patients with aortic stenosis, ventricular septal defect, mitral stenosis, and patent ductus arteriosus. It has been experimentally verified that supraphysiologic shear

Pulsatile tinnitus due to an aneurysmatic diverticulum of the jugular bulb treated with the Woven EndoBridge device.

Tinnitus is a common symptom with an incidence of 4-20%. Pulsatile tinnitus represents a minor subgroup of less than 10% of all patients suffering from tinnitus. Pulsatile tinnitus - different from permanent mostly idiopathic tinnitus is mainly caused by physical sources related to vascular pathologies resulting in a change in blood volume and pressure or an alteration of the vessel lumen. Differential diagnosis for pulsatile tinnitus include dural arteriovenous fistulas, glomus tumors of the jugular foramen and the middle ear or atherosclerotic diseases of the carotid artery in the petrous or

The effect of varying degrees of stenosis on transition to turbulence in oscillatory flows.

Many complications in physiology are associated with a deviation in flow in arteries due to a stenosis. The presence of stenosis may transition the flow to weak turbulence. The degree of stenosis as well as its configuration whether symmetric or non-symmetric to the parent artery influences whether the flow would stay laminar or transition to turbulence. Plenty of research efforts focus on investigating the role of varying degrees of stenosis in the onset of turbulence under steady and pulsatile flow conditions. None of the studies, however, have focused on investigating this under oscillatory…

How Irregular Geometry and Flow Waveform Affect Pulsating Arterial Mass Transfer.

Alzheimer's disease is a progressive degenerative condition that has various levels of effect on one's memory. It is thought to be caused by a buildup of protein in small fluid-filled spaces in the brain called perivascular spaces (PVS). The PVS often takes on the form of an annular region around arteries and is used as a protein-clearing system for the brain. To analyze the modes of mass transfer in the PVS, a digitized scan of a mouse brain PVS segment was meshed and used for computational fluid dynamics (CFD) studies. Tandem analyses were then carried out and compared between the mouse PVS

Physiologic and pathologic changes in patients with continuous-flow ventricular assist devices.

The clinical use of the newer continuous-flow pumps for mechanical circulatory support have resulted in superior outcomes including significantly reduced complication rates with improved durability over first generation pulsatile design pumps. However, as with all new technology, the newer LVADs have introduced a different set of management issues, as well as a unique risk profile into the mechanical circulatory support arena that were previously absent or unimportant with pulsatile LVADs. These include the effects of continuous flow on the systemic circulation and end-organ function, risk of

Prevalence of abnormal common femoral vein pulsatility on Doppler ultrasound in patients undergoing cardiac surgery and its association with adverse events: a prospective cohort study.

PURPOSE: Doppler ultrasound assessment of common femoral vein (CFV) flow could represent a simple, fast, and noninvasive technique to identify right ventricular dysfunction and venous congestion in patients undergoing cardiac surgery. Our primary objective was to determine the prevalence of abnormal CFV flow pulsatility before cardiac surgery. Secondary objectives included investigating its association with postoperative outcomes. METHODS: We conducted a single-centre prospective cohort study and recruited adult patients undergoing cardiac surgery with cardiopulmonary bypass. We performed pul

Arterial blood flow velocity waveforms of the pelvis and lower extremities in normal and growth-retarded fetuses.

Serial measurements of femoral artery flow velocity waveforms were made at specified intervals of 3 to 4 weeks from 15 to 20 weeks of gestational age to 35 weeks and 1- to 3-week intervals thereafter until delivery in 12 normal fetuses (group 1A) with pulsed Doppler ultrasonography. The pulsatility index of the femoral artery increased linearly with advancing gestation in all 12 fetuses; this was associated with reverse diastolic flow velocity waveforms. In 30 other normal fetuses (group 1B) whose gestational ages ranged between 16 and 38 weeks (mean +/- SD, 29.5 +/- 5.6 weeks), the femoral ar

Venous Outflow and Otosclerosis: An Unusual Case of Pulsatile Tinnitus.

We report resolution of right-sided pulsatile tinnitus in a 44-year-old male who underwent stapedectomy for fenestral otosclerosis. Initial workup revealed a mixed hearing loss and absent stapedial reflexes consistent with ossicular fixation. CT angiography demonstrated near complete stenosis of the left-sided transverse and sigmoid sinuses and dominant contralateral venous outflow. We hypothesized that the dominant right cerebral venous outflow tract created turbulent flow that was conducted to cochlea. Successful stapedectomy was performed, and the pulsatile tinnitus resolved. This case repo
