Bystander Acknowledgment Mitigates the Psychological and Physiological Pain of Racial Discrimination for Black Young Adults: A Randomized Controlled Trial.

Racism increases pain sensitization and contributes to racialized pain inequities; however, research has not tested interventions targeting racism to reduce pain. In this study, we examined whether White bystanders can act to mitigate racism's pain-sensitizing effects. To simulate racial exclusion in the laboratory, Black young adults (age 18-30; N = 92) were randomly assigned to be included or excluded by White players in a ball-tossing game (Cyberball). For half of the excluded participants, White bystanders acknowledged and apologized for the racial exclusion. Participants completed a cold …

The role of racial discrimination in medical care: A secondary analysis of the effect of culturally tailored, internet-delivered CBT for insomnia in Black women.

OBJECTIVES: Prior experiences of medical racism harm health outcomes through reduced trust in medical recommendations and ultimately reduced treatment uptake. Unfortunately, experiencing medical racism is common among Black women. Culturally tailoring interventions can increase patient trust and treatment engagement. This secondary analysis examines the role of medical racism as a moderator of intervention use and sleep outcomes among Black women randomized to a tailored or standard internet insomnia treatment. DESIGN: Secondary analysis of a randomized clinical trial. METHODS: In total, 218…

Addressing Adultification of Black Pediatric Patients in the Emergency Department: A Framework to Decrease Disparities.

Systematic racism and structural discrimination in the United States are factors that have negatively affected the health of Black individuals and families. One form of racism that often goes unrecognized and yet has a profound negative impact on the health and safety of Black children is adultification. Adultification occurs when children are perceived, or treated, as being older than they are. We implemented the Racism as a Root Cause framework to identify ways health care providers and health care systems can work to dismantle inequities and address the adultification of Black children in t…

Intersectional Stigma as a Fundamental Cause of Health Disparities: A case study of how drug use stigma intersecting with racism and xenophobia creates health inequities for Black and Hispanic persons who use drugs over time.

Recent evidence points to racial and ethnic disparities in drug-related deaths and health conditions. Informed by stigma, intersectionality, intersectional stigma, and fundamental cause theories, we aimed to explore whether intersectional stigma was a fundamental cause of health. We document key events and policies over time and find that when progress is made new mechanisms emerge that negatively affect health outcomes for Black and Hispanic persons. We then focus on intersectional stigma targeting Black and Hispanic persons who use drugs. We document that when a person, or group of people, o

Social and environmental adversity predict poor mental health in a Milwaukee, WI community sample.

Racism functions as a system of continued devaluation, disempowerment, and inequitable allotment driven by racial hierarchy. Operating at multiple levels - individual, interpersonal, community, and societal - racism perpetuates significant mental and physical health disparities. Though evidence evinces the impact of racism across domains, few studies have assessed factors rooted in structural racism simultaneously and how they relate together to predict outcomes. The current study examines how racism impacts the mental health (MH) of Black Americans adults across multiple levels. Risk and prot…

Stress and the Mental Health of Populations of Color: Advancing Our Understanding of Race-related Stressors.

This article provides an overview of research on race-related stressors that can affect the mental health of socially disadvantaged racial and ethnic populations. It begins by reviewing the research on self-reported discrimination and mental health. Although discrimination is the most studied aspect of racism, racism can also affect mental health through structural/institutional mechanisms and racism that is deeply embedded in the larger culture. Key priorities for research include more systematic attention to stress proliferation processes due to institutional racism, the assessment of stress…

Critical Reflections From the Black Women First Initiative.

In 2020, the Health Resources and Services Administration launched the first initiative focused on Black women in nearly 40 years of the HIV epidemic: the Black Women First initiative. A critical step toward advancing racial equity, the initiative addressed the health and social needs of diverse Black women in HIV care and treatment. In this essay, we posit that the disproportionate burden of HIV/AIDS experienced by Black women is rooted in racism, not race. Using critical reflexivity and the Public Health Critical Race praxis, we critically discuss the Black Women First initiative's framing a…

Migration status, discrimination, and recent sexually transmitted infection testing among South Asian gay, bisexual, and other queer men living in Canada.

BACKGROUND: South Asian gay, bisexual, and queer men (SA GBQM) living in Canada report that racism and heterosexism negatively impact their sexual health. There is a paucity of research examining associations between these forms of stigma, migration status, and sexually transmitted infection (STI) testing for this population. METHODS: We conducted a pooled analysis of Sex Now 2019 and 2021 - online, national, cross-sectional surveys of GBQM. We used modified Poisson regression models (adjusted for age, income, education, disability, and pre-exposure prophylaxis use for HIV) to examine the eff…

Policy solutions to eliminate racial and ethnic child health disparities in the USA.

Societal systems act individually and in combination to create and perpetuate structural racism through both policies and practices at the local, state, and federal levels, which, in turn, generate racial and ethnic health disparities. Both current and historical policy approaches across multiple sectors-including housing, employment, health insurance, immigration, and criminal legal-have the potential to affect child health equity. Such policies must be considered with a focus on structural racism to understand which have the potential to eliminate or at least attenuate disparities. Policy ef…

County-Level Associations Between Structural Inequalities in Social Determinants of Health and Stroke Prevalence Among U.S. Adults: An Ecologic Study.

INTRODUCTION: Women and racial minority groups bear a disproportionate burden of stroke. Stroke disparities have been partially explained by gender and race differences in individual-level SES and neighborhood-level education and residential segregation. This study explored whether structural sexism and structural racism, operationalized as structural inequalities between gender groups and between race-ethnic groups across multiple social determinants of health at the county level, could be contributors by investigating their associations with stroke prevalence. METHODS: County-level age-adju…

Substance use and experienced stigmatization among ethnic minority men who have sex with men in the United States.

Research has documented deleterious effects of racism among ethnic minorities and of homophobia among men who have sex with men (MSM). Less is known about the impact of multiple forms of stigmatization on ethnic minority MSM. This study examined substance use by African American, Asian/Pacific Islander and Latino MSM, and the associations of experienced racism and homophobia from various sources with polydrug use and stimulant drug use. Experienced racism within the general community was associated with higher levels of use; other forms of discrimination were either not associated with polydru…

Income inequality, drug-related arrests, and the health of people who inject drugs: Reflections on seventeen years of research.

This paper reviews and then discusses selected findings from a seventeen year study about the population prevalence of people who inject drugs (PWID) and of HIV prevalence and mortality among PWID in 96 large US metropolitan areas. Unlike most research, this study was conducted with the metropolitan area as the level of analysis. It found that metropolitan area measures of income inequality and of structural racism predicted all of these outcomes, and that rates of arrest for heroin and/or cocaine predicted HIV prevalence and mortality but did not predict changes in PWID population prevalence.…

Weathering the storm: Coping self-efficacy, racial trauma, and dysphoria among Black women facing racial stress.

BACKGROUND: In the United States, Black women face racial stressors that significantly undermine their well-being and contribute to poor mental health outcomes. OBJECTIVES: The current study examines how racial stressors (i.e., racial violence exposure through social media, vicarious racism, and interpersonal racism) are associated with racial trauma and dysphoria (i.e., depression, anxiety, and hostility) and the moderating role of emotion-focused coping self-efficacy (e.g., suppression of unpleasant thoughts and emotions) among Black women (Mage = 35, N = 283). DESIGN: The study employed a…

Community Resilience: A Dynamic Model for Public Health 3.0.

OBJECTIVE: To establish a model for Public Health 3.0 in order to define and measure community resilience (CR) as a method to measure equity, address structural racism, and improve population health. DESIGN: To develop the CR model, we conducted a literature review in medicine, psychology, early childhood development, neurobiology, and disaster preparedness and response and applied system dynamics modeling to analyze the complex interactions between public systems, policies, and community. MAIN OUTCOME MEASURES: The CR model focuses on community and population health outcomes associated with…

Racial discrimination, post traumatic stress, and gambling problems among urban Aboriginal adults in Canada.

Little is known about risk factors for problem gambling (PG) within the rapidly growing urban Aboriginal population in North America. Racial discrimination may be an important risk factor for PG given documented associations between racism and other forms of addictive behaviour. This study examined associations between racial discrimination and problem gambling among urban Aboriginal adults, and the extent to which this link was mediated by post traumatic stress. Data were collected via in-person surveys with a community-based sample of Aboriginal adults living in a mid-sized city in western C…

Social oppression, psychological vulnerability, and unprotected intercourse among young Black men who have sex with men.

OBJECTIVE: Young Black men who have sex with men (YBMSM) are at extraordinarily high risk for HIV infection. Given their dual minority identity, they experience multiple forms of social oppression-racism, homophobia, and poverty. This study tested a model for how these forces contribute to their sexual risk behavior. METHOD: YBMSM (n = 1,289) from 2 Texas cities completed a 1-time assessment of sexual behaviors and psychosocial variables. Structural equation modeling was used to characterize relationships among variables. RESULTS: Experiences of racism, homophobia, and socioeconomic distress…
