The concentration of TNF-alpha correlate with number of inflammatory cells and degree of vascularization in radicular cysts.

OBJECTIVE: To correlate values of tumor necrosis factor-alpha (TNF-alpha) depending on the count of inflammatory cells with degree of vascularization in cystic fluid of radicular cysts. MATERIAL AND METHODS: We investigated TNF-alpha concentration in 43 radicular cysts obtained from patients undergoing surgery, under local anaesthesia, and after aspiration of cystic fluid from non-ruptured cysts by enzyme-linked immunosorbent assay assay in respect of different clinical parameters as well as by histomorphometric analyses. RESULTS: Significantly higher concentration of TNF-alpha is associated…

Microvessel density and vascular endothelial growth factor (VEGF) expression in human radicular cysts.

PURPOSE: To assess vascular endothelial growth factor (VEGF) expression and microvessel density (MVD) by immunohistochemistry and to relate them to the inflammatory status in a sample of radicular cysts. METHODS: Specimens of 24 human radicular cysts were examined by immunohistochemistry using antibodies anti-VEGF and anti-CD34 and to evaluate vascular density. Integrity of the epithelium and inflammatory state of the connective tissues were evaluated and related with the immunohistochemical findings. A Spearman correlation test was utilized to compare the means of each parameter. RESULTS: V…

Radicular cysts arising from deciduous teeth. Review of the literature and report of 23 cases.

Radicular cysts arising from deciduous teeth are rare. 28 such cases have been recorded in the literature since 1898. In this study, we present 23 personally-observed cases of radicular cysts arising from deciduous teeth. The patients' ages ranged from 4 to 12 years with one exceptional case aged 19 years. The M:F ratio was 1.6:1. The mandible was affected more frequently than the maxilla and the deciduous molars were the teeth most commonly involved. 2 of the cysts presented multi-locular radiolucency; in 9 cases, buccal expansion was noticed and in 8 cases, permanent buds were displaced. Car…

Immunohistochemical expression of nuclear factor κB, matrix metalloproteinase 9, and endoglin (CD105) in odontogenic keratocysts, dentigerous cysts, and radicular cysts.

OBJECTIVE: The aim of this study was to compare the immunohistochemical expression of nuclear factor κB (NF-κB), matrix metalloproteinase 9 (MMP-9), and CD105 in odontogenic keratocysts (OKCs), dentigerous cysts (DCs), and radicular cysts (RCs). STUDY DESIGN: Twenty cases of OKCs, 20 DCs, and 20 RCs were analyzed. A labeling index (LI), which expresses the percentage of NF-κB-stained nuclei, was calculated for the analysis of NF-κB expression. Expression of MMP-9 in the epithelium and in the capsule of each lesion was scored as 0 (<10% stained cells), 1 (10%-50% stained cells), or 2 (>50% sta…

Rapidly Progressing Radicular Cyst in an Endodontically Treated Tooth: Diagnostic Challenges, Advanced Imaging, and Surgical Management.

Radicular cysts in teeth that have undergone root canal therapy (RCT) are uncommon but clinically important, often presenting unique challenges for diagnosis. Although rare, radicular cysts can progress rapidly, leading to symptoms, such as discomfort or swelling shortly after the initial treatment. In this case, a 29-year-old female experienced mild discomfort localized to the maxillary right first premolar, which had received RCT several months prior. The patient had no history of systemic conditions or chronic infections. Upon clinical examination, tenderness was noted in the periapical reg…

Pathways of inflammatory cellular exudate through radicular cyst epithelium: a light and scanning electron microscope study.

A feature of radicular cyst histology is the observation that polymorphonuclear leucocytes infiltrate the epithelium whereas chronic inflammatory cells do not and accumulate in the subepithelial connective tissue. Material for the study of this phenomenon consisted of radicular cysts fixed in formol saline. Observation under the SEM indicated that the lumenal surface of the radicular cyst is characterized by numerous interepithelial spaces through which polymorphonuclear cells reach the cyst cavity. The cells on the lumenal surface are associated with interepithelial spaces. Observations of th…

Management of infected radicular cyst associated with immature maxillary permanent lateral incisor: a conservative surgical approach.

Radicular cysts are the most common cystic lesions which affect the jaw. Traumatic dental injuries cause injury to the periodontal ligament and dental pulp often leads to pulpal necrosis. The necrosed pulp eventually becomes the nidus of infection and irritates the periapical epithelial cell remnants, which in turn develops into a cyst eventually. This case report presents the successful conservative surgical management of a large infected radicular cyst which was associated with traumatised, necrotic, permanent maxillary lateral incisor with open apex by Partsch II surgical procedure followed…

Platelet-rich fibrin combined with synthetic nanocrystalline hydroxy apatite granules in the management of radicular cyst.

Radicular cysts are inflammatory jaw cysts confined to the apices of teeth with infected and necrotic pulp. They arise from the epithelial residues in the periodontal ligament as a result of inflammation, following the death of pulp. The treatment of such lesions vary with regard to their sizes; the small cystic lesions heal after an endodontic therapy, but larger lesions, may require additional treatment. Apical surgery for radicular cysts generally involves apical root resection and sealing with endodontic material. This case report, describes the treatment of a cyst related to the maxillary…

Window decompression surgery for a large radicular cyst in a 7-year-old patient: A case report.

Radicular cysts are rarely present in the primary dentition because of the distinct biological cycle of primary teeth. Cyst formation in children may cause bony expansion and resorption, malposition, delayed eruption, enamel defects, or damage to the developing permanent successors. Various treatment modalities for the management of radicular cysts have been reported in the literature. In this case, the preferred treatment approach is window decompression surgery (WDS) combined with gap retainers. Long-term follow-up revealed good healing of the bony lesion. The present case involved an abnorm…

Iceberg lesion-A rare case of chronic osteomyelitis of the jaw with underlying radicular cyst: A case report.

Osteomyelitis is an inflammatory condition of the bones that often starts with an infection of the medullary cavity and swiftly extends to the haversian system and periosteum. If drainage and antimicrobials are ineffective, acute osteomyelitis may progress to chronic osteomyelitis. Here, we offer a unique case that analyzes the radiographic and clinical characteristics of osteomyelitis with underlying disease. Cysts and cyst-like lesions of the jaws must be diagnosed and evaluated using radiographic findings in conjunction with clinical complaints. This case study examines the circumstances th
