[Transhepatic access for radiofrequency ablation in a child].

Radiofrequency catheter ablation is safe and effective in children with refractory supraventricular tachycardia and poor response to pharmacological treatment. This procedure may be difficult or impossible to perform in patients with bilateral obstruction of the femoral venous access. In recent years, the transhepatic approach has been found to be safe in children. We report the case of a 19-month-old boy with permanent junctional reciprocating tachycardia who underwent transhepatic catheterization for radiofrequency ablation.

[Angiographic embolization for the treatment of uncontrollable epistaxis].

In the treatment of 8 cases of patients with uncontrollable epistaxis, we used the embolization following vascular angiographic localization. They were cured and no any serious complication. Embolization is the safe and effective method of stopping uncontrollable epistaxis and is the best method in the elder patients with other diseases, under the condition of anterior and posterior nasal packing being controversial. It is the key that before embolization we make angiography to arteries and find the bleeding vessels. The technique of embolization, superselective embolization and so on are all

Advances in pediatric interventional radiology.

The past year has seen important papers on the risk of acquired immunodeficiency syndrome in the invasive radiology laboratory with emphasis on awareness and prevention. Interventional catheterization techniques in congenital heart disease are reviewed. Valvoplasty of pulmonic stenosis has the greatest success and least complication rate. The majority of complications appear to be at the vascular access site and are more common in neonates and in procedures that are ultimately unsuccessful. Intrauterine valvoplasty of the aortic valve is reviewed as is angioplasty for a recurrent coarctation o

[The current aspects of treating residual choledocholithiasis].

Taking into consideration the seriousness of the problem residual calculosis of the common bile duct, the authors analyse the noninvasive methods of treatment during the past 20-30 years. Available data in the literature are adduced pointing to the importance of endoscopic papillotomy and extraction of the calculi. Indications and contraindications for performing this procedure are defined. The different methods of treatment are evaluated on the basis of a follow up of three patients with residual calculosis of the common bile duct, treated by noninvasive method and the experience of treatment

Intussusception during enteral nutrition: a case report.

We report a case of intussusception during enteral nutrition after esophagectomy. The case was a 60-year-old patient who underwent subtotal esophagectomy via the right thoraco-abdominal approach, reconstructed with esophagogastrostomy. He underwent tube feeding through gastrostomy after surgery. The tip of the feeding tube was emplaced in the jejunum. He complained of intermittent abdominal pain and the drainage volume through the gastric decompression tube was increased on the 2nd week from surgery. There was no abnormal finding on abdominal CT or or radiography of the digestive tract, and so

Odontoma of the middle ear: case report with 25-year follow-up.

We report the 25-year follow-up on the first reported case of odontoma in the middle ear. Diagnosis of odontoma had been made on the basis of radiography films that showed a middle ear mass with multiple toothlike areas of radiopacity. No clinical intervention was recommended. At 25-year follow-up, audiometry showed progressive mixed hearing loss on the affected side and mild ipsilateral sensorineural hearing loss. Computed tomography better characterized the discrete mass, which was shaped similar to a dental crown. To avoid jeopardizing cochlear and facial nerve function, no surgical interve

[Pericardial empyema due to oropharyngeal infection -- a rare entity].

Both the descending necrotizing mediastinitis (DNM) and the pericardial empyema are rare but life threatening situations, in which rapid aggressive surgical approach is important for the outcome. A 37-year-old man presented with reduced general health and new establishment of thoracal pain after an infection in the oropharyngeal region. The dissemination of the infection into the mediastinum (mediastinitis) diagnosed by clinic and radiography required surgical intervention. A pericardial empyema was detected intraoperatively. The patient was discharged home on the 14 (th) hospital day.

Occupational Radiation Exposures in Interventional Radiology and the Cardiac Catheterization Laboratory.

OBJECTIVE: To determine whether significant differences exist in occupational radiation exposures among clinicians, nurses, and technologists working in interventional radiology, potentially warranting role-specific protective measures. PATIENTS AND METHODS: De-identified radiation exposure records for 1,269 Mayo Clinic employees in radiology and cardiology were reviewed for 2021-2023. Annual exposures for participants meeting screening criteria were determined, with median imputation for missing data. Multiple linear regression compared the natural log of radiation doses between professional

Ultra-Low-Contrast PCI: A Structured Approach to Reducing Dependence on Contrast Vessel Opacification in PCI.

Since its inception, percutaneous coronary intervention (PCI) has relied upon vessel opacification with iodinated contrast to plan, guide, and assess the results of the procedure. Yet revisiting this fundamental concept is important in contemporary PCI practice, especially in patients with high-risk clinical or anatomical profiles. In addition to decreasing the probability of acute kidney injury during PCI, limiting the volume of iodinated contrast allows the operator to perform more thorough interventions by relying on intracoronary imaging and physiology, ultimately contributing to more comp

Successful contemporary reverse controlled antegrade and retrograde subintimal tracking without contrast medium: a case report.

BACKGROUND: Contrast-induced acute kidney injury is one of the common adverse events related to percutaneous coronary intervention and a predictor for worse outcome. In the setting of percutaneous coronary intervention for chronic total occlusion, large amounts of contrast medium, more than 200-400 mL, are generally injected. A higher dose of contrast medium causes contrast-induced acute kidney injury more frequently. Therefore, patients who undergo chronic total occlusion-percutaneous coronary intervention are at risk for contrast-induced acute kidney injury. CASE PRESENTATION: We present th

[Interventional neuroradiology: current options].

Modern interventional neuroradiology has a leading role in the treatment of ischemic and hemorrhagic stroke, and it provides more and more important treatment options for degenerative diseases of the vertebral column and the management of correlated pain. During the last decades interventional neuroradiology has played a primary role in the treatment of intracranial berry aneurysms due to the continuous technical improvements. Ongoing studies proved superiority of mechanical stent-thrombectomy in acute proximal occlusion of cerebral arteries. Less invasive neurointerventional methods, such as

[Cerebrospinal fluid leakage. Indications, technique and results of treatment with a blood patch].

BACKGROUND: In most cases cerebrospinal fluid (CSF) leaks are iatrogenic and caused by medical interventions, such as lumbar puncture, peridural anesthesia and surgical interventions on the spine, However, spontaneous cerebral hypotension is currently detected more frequently due to improvements in diagnostic possibilities but often the cause cannot be clarified with certainty. METHODS: There are various diagnostic tools for confirming the diagnosis and searching for the site of CSF leakage, such as postmyelography computed tomography (postmyelo-CT), indium(111) radioisotope cisternography an

Management of peripancreatic fluid collections following partial pancreatectomy: a comparison of percutaneous versus EUS-guided drainage.

BACKGROUND: Peripancreatic fluid collections (PFCs) occur in up to 30 % of patients following partial pancreatic resections. Traditionally, postoperative PFCs are managed via percutaneous drainage (PD). EUS-guided transgastric drainage has been shown to be effective for the management of PFCs secondary to acute pancreatitis. However, there are limited data on the use of EUS-guided drainage (EUSD) for the management of postoperative PFCs. OBJECTIVE: To compare the safety and efficacy of PD versus EUSD of PFCs in patients who have undergone partial pancreatic resections. DESIGN: Retrospective

Aberrant positioning of a central venous dialysis catheter to reveal a left-sided partial anomalous pulmonary venous connection.

We describe the identification of a rare, left-sided, partial anomalous pulmonary venous connection during routine central venous catheterization. To our knowledge, this is the first report in the literature to describe this anomaly in a hemodialysis patient. A young man had anomalous connection of the veins draining the upper lobe of the left lung and left innominate vein. Our case demonstrates the importance of routine fluoroscopy during insertion of central venous catheters to detect these anomalies and minimize complications.
