BACKGROUND: Free flap breast reconstruction (FBR) is a well-established option for breast reconstruction, with the internal mammary vessels (IMVs) representing the preferred recipient pedicle. Despite its high success rates, anastomotic thrombosis (AT) remains a challenging complication and few studies have investigated the quality of IMVs and risk factors associated with AT.
OBJECTIVES: To assess degenerative changes in IMVs and evaluate risk factors following FBR.
METHODS: During recipient vessel preparation for free flap breast reconstruction, 2-3 mm segments of the internal mammary arter

This article reports 2 cross-leg free composite tissue flaps for repairing the severe composite tissue defects in lower leg without suitable adjacent recipient vasculature for microvascular anastomosis. The osseous myocutaneous flap of ilium and tensor fascia lata pedicled with ascending branch of lateral femoral circumflex vessels and the osseous muscle flap of scapula and latissimus dorsi pedicled with subscapular vessels were performed, respectively, to reconstruct the bone and soft-tissue defects in the lower leg of 2 patients. Both donor vessels were the posterior tibial artery and great

Free vascularized fibula bone flap has been widely used in reconstruction of the mandible, long segment defect, congenital pseudarthroses, and osteomyelitis. Such applications stirred an interest in basic studies of bone biology, bone healing process, and incorporation of recipient bone defect. An experimental free vascularized fibula rat model is presented here for such investigations. We performed 16 angiograms and anatomic dissections in eight rats for collecting data on fibular length, blood supply, and the caliber of significant vessels. The fibula was harvested with part of the flexor ha

The use of an arteriovenous fistula (AVF) to afford vascular access for free tissue transfer is described in 11 consecutive patients from a 1 year period. The leg was the site of pathology in six cases, and a reversed saphenous AVF to the femoral vessels was created. In the remaining five cases, those in the head and neck and arm regions, the recipient vessels varied. The mean patient age was 37.7 years, and ten of 11 patients were male. Etiologies of defect were automobile accidents in six cases, neoplasm in three, and gunshot wound and electrical injury in one patient each. Mean ischemia tim…

OBJECTIVE: To describe the clinical application of a vascularized preputial free flap in the reconstruction of a large facial-labial defect.
STUDY DESIGN: Case report.
ANIMALS: An adult neutered male mongrel dog with a large left facial-labial defect.
METHODS: A preputial free flap was created by elevating the cranial part of the prepuce, using the ipsilateral caudal superficial epigastric vessels as a vascular pedicle. The flap was transferred to the facial-labial defect and the donor vessels anastomosed to the sublingual artery and vein, using standard microvascular technique.
RESULTS: T

Nasal alar defects lead to facial disfigurement, and nasal ala reconstruction is an important treatment option. The vascularized composite auricular flap based on the superficial temporal artery is an ideal option for a full-thickness nasal alar defect. However, the pedicle length and the discrepancy in artery diameter between the recipient vessel and flap pedicle continue to be major problems for free auricular composite tissue transfer. Considering that the angular artery is occasionally absent and the course of the infraorbital segment of the facial vein is constant, there are often no suit

BACKGROUND: While complications of deep inferior epigastric artery perforator flaps are known and well documented, a thorough literature review revealed no other reports of a patient developing a chyle leak following the use of the internal mammary vessels for recipient vessels in autologous breast reconstruction.
CASE: A 55-year-old woman underwent free autologous breast reconstruction. She developed a chyle leak during the postoperative period. This was verified through a computed tomography scan and fluid analysis demonstrating a high triglyceride count and the presence of chylomicrons. Th…

Organ transplantation can be associated with vascular torsions and angulations of both recipient and donor vessels. Such kinks and/or torsions of vessels can compromise the vascular integrity, obstruct inflow and/or outflow, and result in loss of the organ and/or body parts. On many occasions, mild angulations and torsions can be successfully addressed by repositioning the organ. In cases where the abnormal findings persist, maneuvers such as placing a fat pad to create a smoother curve, or even opening the peritoneum (in the case of kidney transplants) to allow for a better positioning of the

BACKGROUND: Complex and severe leg defects remain a persistent challenge in reconstructive surgery, especially when no feasible local or regional flaps are available. An operative solution is yet to be found. The descending branch of the lateral circumflex femoral artery of the injured leg was used as the recipient artery in a reverse-flow fashion in these difficult wounds.
METHODS: From March of 2005 to May of 2010, 11 male and four female patients with soft-tissue defects in the lower leg were treated. Causes of injury were industrial machinery injuries in six patients and traffic accidents

A 57-year-old Japanese female was considered for living donor liver transplantation (LDLT) due to end-stage liver cirrhosis caused by primary biliary cholangitis with portal vein thrombosis (PVT) formation. A 26-year-old daughter of the patient was selected as a living donor; however, a computed tomography examination revealed trifurcated-type portal vein anomaly (PVA). Preoperative liver volumetry showed that the right lobe graft was necessary for the recipient; therefore, reconstruction of the portal vein bifurcation during LDLT was necessary. We planned to extract the recipient's own hepati

BACKGROUND: Optical coherence tomography (OCT) is a high-resolution intravascular imaging technique used in adults. We tested the hypothesis that OCT could identify coronary abnormalities not seen by angiography in children with a history of Kawasaki disease (KD) and pediatric heart transplant (TX) recipients.
METHODS AND RESULTS: Patients with KD and TX recipients were evaluated between December 2012 and October 2013 with angiography and OCT (Ilumien System, LightLabs, St Jude Medical, Westford, MA). Modifications were made to the adult OCT protocol to adapt this technique for children. Seri

Chicken primordial germ cells (PGCs) are valuable for breeding, germplasm preservation, and genetic modifications. Here, using single-cell transcriptome analysis, we found that three transcriptionally distinct PGC subtypes were exist in chicken embryonic gonads PGCs. We further demonstrated that in vitro-cultured PGCs could integrate into recipient gonads via blood vessel injection. Additionally, the gene-modified PGCs could be re-established and integrated into recipient gonads after the PGCs were cultured for long-term use by a second blood-vessel injection. Together, these results provide e

Multidisciplinary care with vascular surgery and plastic surgery is essential for lower extremity free flap (LEFF) success in the chronic wound population with diabetes and peripheral vascular disease. There is a lack of understanding on performing targeted direct endovascular reperfusion on a vessel that will be used as the flap recipient. Our study compares outcomes of patients who received targeted revascularization (TR) to the recipient vessel for LEFF anastomosis versus nontargeted revascularization (NR) of arterial recipients prior to LEFF.LEFF patients who underwent preoperative endovas

PURPOSE: Retrospective reviews suggest that the progression of peripheral vascular disease (PVD) may be accelerated in heart transplant recipients. This study was undertaken to determine the incidence and to identify those risk factors that may be associated with the development or progression of PVD in these patients.
METHODS: Between January 1990 and December 1993 a prospective vascular screening protocol including abdominal ultrasonography, Doppler-derived ankel-brachial pressure indexes (ABI), and carotid artery duplex imaging was added to the routine preoperative and annual postoperative

Proximal lymphaticovenous anastomosis (LVA) achieves superior limb decompression; however, recipient veins in this region are often deep and disproportionately large in caliber relative to small lymphatic vessels (LVs). The resulting size mismatch frequently forces surgeons to forgo anastomoses with several LVs, undermining drainage, and no current technique reliably resolves this constraint. We developed the Lymphatico-Lymphatic Bridge (LLB), a sequential end-to-side method that uses an adjacent LV as an autologous conduit, linking otherwise unreachable LVs to a single recipient vein. The LLB
