The surgical anatomy of a horseshoe kidney (HK) is unique in many ways, ranging from its anomalous circulation, shared renal parenchyma between the right and left renal moieties, and its anterior renal pelvis, to the fact that it obscures access to the vena cava and aorta. While renal cell carcinomas (RCCs) are known to occur in HKs, the surgical approach to an RCC with tumour thrombus extending to the right atrium has not been reported in the literature. We report an unusual presentation of RCC and the technical aspects of our successful experience with managing RCC of a HK extending to the i

We measured blood viscosity in rats 24 h after induction of acute renal failure by glycerol or HgCl2 injections. The blood viscosity values of rats with acute renal failure were significantly higher than those of controls at any shear rates. The mean values of plasma fibrinogen in animals with glycerol - (302.5 +/- 35.2 mg/100 ml) or HgCl2 - (342.1 +/- 15.9 mg/100 ml) induced acute renal failure were significantly elevated compared to control levels (169.6 +/- 16.7 mg/100 ml). We believe that the increased blood viscosity in acute renal failure was primarily due to high concentration of plasma

A right pelvic kidney was observed in a patient, who presented with hypertension. On angiograms, the left kidney was normally positioned and had a single renal artery, whereas the right pelvic kidney received three arteries, which arose from bilateral common iliac arteries and from ipsilateral internal iliac artery. The renal arteries from the ipsilateral internal iliac artery and the contralateral common iliac artery supplied the medial half of the pelvic kidney. In the present case, the blood supply from both the right and left sides appeared to be related to the medial position of the right

A hemodynamic model of the pulmonary trunk stenosis was created in the experiment that involved twenty two pups to reveal the regularities of the structural changes in the different areas of the renal vascular bed. The animals were monitored from the six months up to two years of age. Specimens obtained from different parts of their kidneys were investigated histologically, histochemically, morphometrically and stereometrically. The material taken from ten dogs of the corresponding age was used for control. It was found that the establishment of pulmonary trunk stenosis and the impairment of t

The aim of this case report is to assess the potential role of intrarenal Doppler ultrasonography as a non-invasive method to evaluate intrarenal venous flow (IRVF) in acute heart failure (AHF) and concomitant renal dysfunction. We report a case of an 81-year-old woman with valvular heart disease (previous mitral valve replacement) that presented with acutely decompensated heart failure and concomitant worsening renal function (WRF). In addition to complete physical examination, laboratory analysis, and echocardiography, IRVF was assessed at baseline and 48 h after the administration of diuret

Renal artery stenosis may play a significant role in the pathogenesis of secondary hypertension, renal dysfunction, and flash pulmonary edema. Currently correction of renal arterial inflow stenosis is reserved for resistant hypertension patients who have failed maximal medical therapy, have worsening renal function and/or unexplained proximal congestive failure. With the recent advances in minimally invasive percutaneous stent placement techniques, open surgical revascularization has been largely replaced by renal artery stenting. The potential benefit of revascularization seemed intuitive; ho

A 60-year-old female patient presenting with anemia was found to have a left-sided renal tumor and a contralateral adrenal mass of 2 cm in diameter. Imaging studies for metastases were negative. Nephrectomy along with contralateral adrenalectomy was performed and histology disclosed renal cell carcinoma stage pT3a pNO G2 with solitary contralateral adrenal metastasis. In a survey of the literature, 24 previous cases of renal cancer with solitary contralateral adrenal metastasis were identified. The most probable biological pathway to explain this peculiar metastatic pattern is transpulmonal pa

This paper is aimed at evaluating the effect of blood purification on the mitochondrial functions of the kidney in jaundiced rats. At 1, 2, or 3 weeks after bile duct ligation and dissection, obstructive jaundiced rats were utilized for experiment. Cross circulation in 1 ml/min between jaundiced rats and normal rats was performed for 1 or 3 h. Immediately and 24 h after cross circulation, the rats were killed and mitochondrial respiratory functions of the kidney were assessed by measuring oxygen consumption. Mitochondrial function, especially ATP synthesis, showed significant improvement in ja

Collateral lumbar arterial blood supply to an autotransplanted kidney was angiographically documented in a patient with bilateral Wilms' tumor and postoperative transplant renal artery stenosis. During autotransplantation, most collateral pathways are interrupted. The development of a collateral circulation after autotransplantation may be postulated to begin with a vascular response to postoperative inflammation and adhesions. The small anastomotic pathways so formed may then dilate in response to ischemia resulting from renal artery stenosis.

The physiological role of inactive renin, especially the question of whether and how a conversion to active renin takes place in vivo, remains controversial. In order to show the dynamic alterations from inactive to active renin following acute ACE-inhibition, both forms of renin were investigated in both renal veins and the peripheral circulation of 20 patients with essential hypertension and 20 patients with renovascular hypertension before and 1 h after 25 mg of captopril. Active and inactive renin were determined indirectly as plasma renin activity (PRA, unit: ng/ml x h). In vitro activati

Presented herein is a clinical case report regarding surgical reconstruction of the thoracoabdominal aorta according to a new method aimed at reducing the time of ischemia of visceral organs. The procedure began with distal bypass of the iliac arteries, followed by sequential debranching of the left renal artery, superior mesenteric artery, and celiac trunk in the presence of natural blood circulation without aortic cross-clamping. And it was only at the stage of prosthetic repair of the right renal artery and intercostal arteries that the left ventricular bypass (pulmonary vein - branch of th

Renal atrophy due to the renal artery stenosis associated with aortitis syndrome was studied in seven patients. Severe hypertension, high plasma renin activity and normal overall renal functions were present in all. The luminal diameter of the stenosed renal artery seen on the angiogram was less than 1.5 mm at the maximum area of stenosis in all patients. The size of the kidney with renal artery stenosis was diminished, ranging from 8.0 to 14.0 cm in longitudinal diameter on the nephrogram. Histological findings of the small kidney revealed ischemic atrophy. Although the rate of progression of

Modification of proteins with hydrophilic polymers is an effective strategy for regulation of protein pharmacokinetics. However, conjugates of slowly or non-biodegradable materials, such as poly(ethylene glycol), are known to cause long-lasting cell vacuolization, in particular in renal epithelium. Conjugates of more degradable polymers, e.g., polysaccharides, have a significant risk of immunotoxicity. Polymers that combine complete degradability, long circulation in vivo, and low immuno and chemical toxicity would be most beneficial as protein conjugate components. This study explores new ful

AIMS/HYPOTHESIS: Ectopic lipid accumulation in renal tubules induces lipotoxicity and contributes to the progression of diabetic kidney disease (DKD). Apolipoprotein J (ApoJ), a glucose-regulated, liver-derived molecular chaperone, is implicated in systemic metabolic homeostasis. This study aimed to investigate the pathophysiological role of ApoJ in the development of DKD.
METHODS: Spearman's r analysis was used to evaluate the association between circulating ApoJ concentrations and renal function in a cohort of 201 individuals with type 2 diabetes mellitus. The pathways were identified by pr

During resection of certain complicated aortic abdominal aneurysms, the circulation of the renal artery may require separate perfusion. To achieve this in a simplified manner, a heparin bonded shunt has been used for continuous perfusion of the kidney. This has been used in patients with a horseshoe kidney undergoing aortic aneurysmectomy; for intraoperative perfusion of a previously inserted saphenous vein to renal artery graft for renovascular hypertension and in simultaneous correction of an aortic aneurysm and insertion of a renal artery graft for hypertension. In each situation, the techn

During phototherapy of jaundiced newborns, vasodilation occurs in the skin circulation compensated by vasoconstriction in the renal and mesenteric circulation. Furthermore, there is a slight decrease in cardiac systolic volume, and blood pressure, as well as an increase in heart rate and discrete changes in the heart rate variability (HRV). The primary change during phototherapy is the skin vasodilation mediated by multiple mechanisms: 1) Passive vasodilation induced by direct skin heating effect of the body surface and subcutaneous blood vessels, modified by myogenic autoregulation. 2) Active

OBJECTIVE: This study was undertaken to determine the effect of gadolinium arteriography on renal function and its diagnostic accuracy in patients with azotemia with suspected renovascular disease.
METHODS: Catheter-based digital subtraction arteriographic studies with gadolinium as the contrast agent were performed on 25 occasions in 21 consecutive patients with azotemia to evaluate renal arterial circulation. Gadolinium (gadodiamide, 287 mg/mL) was the only contrast used in these studies. Quantities of gadolinium administered ranged from 40 to 264 mL (mean +/- standard deviation, 124 +/- 74
