Oxygen is critical for neural metabolism, but under most physiological conditions, oxygen levels in the brain are far more than are required. Oxygen levels can be dynamically increased by increases in respiration rate that are tied to the arousal state of the brain and cognition, and not necessarily linked to exertion by the body. Why these changes in respiration occur when oxygen is already adequate has been a long-standing puzzle. In humans, performance on cognitive tasks can be affected by very high or very low oxygen levels, but whether the physiological changes in blood oxygenation produc…

Periodic breathing (PB) is a diseased condition of the cardiorespiratory system, and mathematically it is modelled as an oscillation. Modeling approaches replicate periodic oscillation in the minute ventilation due to a higher than normal gain of the feedback signals from the chemoreceptors coupled with a longer than normal latency in feedback, and do not consider the waxing-waning pattern of the oronasal airflow. In this work, a noted regulation model is extended by integrating respiratory mechanics and respiratory central pattern generator (rCPG) model, using modulation-demodulation1 hypothe

Central sleep apnea (CSA) in patients with heart failure (HF) occurs frequently and shows a serious influence on prognosis in this population. The key elements in the pathophysiology of CSA are respiratory instability with chronic hyperventilation, changes of arterial carbon dioxide pressure (pCO2) and elongated circulation time. The main manifestation of CSA in patients with HF is Cheyne-Stokes Respiration (CSR). The initial treatment is the optimization of HF therapy. However, many other options of the therapeutic management have been studied, particularly those based on positive airway pres…

Preeclampsia (PE) affects 5-8% of all pregnancies and is associated with significant maternal and fetal morbidity and mortality. Placental mitochondrial dysfunction has been reported in PE. MicroRNAs (miRNA) are small non-coding RNAs that regulate gene expression through mRNA degradation and translational repression. MiR-210 has been previously shown to be upregulated in placentas from pregnancies complicated by PE. We hypothesized that placental mitochondrial dysfunction during PE can be mediated by miR-210. Placentas were collected at term from normotensive pregnancies (CTRL) and those compl

This study aimed to investigate ventilatory correlates of conditioned fear responses. Respiratory, end-tidal carbon dioxide pressure (PetCO(2)) and heart rate changes were studied in a differential fear-conditioning paradigm. Forty-two participants viewed pictures of faces. One picture (CS+) was followed by a human scream (US) during the acquisition phase, but not in a subsequent extinction phase. Conditioning of PetCO(2) (decrease), respiratory cycle time (decrease) and inspiratory duty time (increase) was established and subsequently extinguished. When participants were clustered according t

The larynx is the "bottleneck" of the human airway. For this reason, the effects of stenosing laryngeal pathologies on the vital factor respiratory gas exchange are particularly critical.Internal stabilization is a prerequisite for recovery of the laryngeal respiratory function in severe forms of inspiratory collapse (laryngomalacia). Effective laser surgery techniques have been developed to this end in recent years.Glottis-dilating surgery in cases of bilateral vocal cord motion impairment is now moving in the direction of endoscopic laser cordotomy or cordectomy, whereas arytenoidectomy and

The effect of standard cardiac resynchronisation therapy (CRT) on the severity of Cheyne-Stokes respiration (CSR) in patients with congestive heart failure was studied. It was hypothesised that CRT, through its known beneficial effects on cardiac function, would stabilise the control of breathing and reduce CSR. Twenty-eight patients who were eligible for CRT and receiving optimised medical treatment for congestive heart failure were referred for overnight polysomnography, including monitoring of thoracic and abdominal movements to identify CSR and obstructive sleep apnoea events. Patients und…

The respiratory effects of acute spinal injury and paralysis are difficult to study. Urgent medical needs of human spinal cord injury victims usually preclude study, while induction of spinal cord lesions in awake animals is not feasible ethically. We utilized controlled, segmental infusion of epidural anesthetic in awake, highly trained, implanted canines to reversibly simulate the effects of thoracic and cervical (paraplegic and quadriplegic) spinal cord injury. We studied six animals, an average of 29 days after implantation with electromyogram and sonomicrometry transducers in transversus

Cheyne-Stokes respiration (CSR) during sleep is common in patients with severe congestive heart failure induces repetitive oxygen desaturation with arousals, and impairs sleep. This causes daytime symptoms and likely an increase in sympathetic activity. It has, therefore, been suggested that CSR is independently related to mortality. The major mechanisms behind CSR include reduced body stores of oxygen, a low apneic threshold for carbon dioxide, prolonged circulation time between the lung and the carotid body, and disturbance of respiratory control due to arousals. It is apparent that the main…

The actual therapy of choice for the common obstructive sleep-apnea syndrome is the application of continuous positive airway pressure (CPAP) via a nose-mask. This ventilation by positive pressure is explained in some detail. Mechanical ventilation for treatment of nocturnal respiratory failure in outpatients of various etiologies is covered in the companion paper by J. C. Chevrolet. Treatment of sleep disorders due to other pulmonary diseases such as bronchial asthma and others require treatment of the underlying disorder.

The relationship between vertical dentofacial morphology and respiration has been debated and investigated from various approaches. The purpose of this study was to use contemporary respirometric techniques to compare the breathing behavior of normal and long-faced adolescents. Sixteen normal and 32 long-faced subjects 11 to 17 years of age were chosen clinically and verified by means of a discriminant function. Vertical and anteroposterior facial form was assessed from lateral cephalometric radiographs according to the following measurement criteria: six skeletal angular, eight skeletal linea…

The purpose of this study was to determine the respective oral and nasal contributions to total respiration in patients scheduled for surgical corrections of nasal obstruction. The effect of anterior nares expansion and/or nasal decongestant administration on the nasal component of breathing was also examined in these patients. Although variability among subjects was demonstrated in the ratio of nasal respiration to total respiration, 25% of the "nasally-obstructed" patients were 100% nasal breathers and no patient had a nasal component less than 18% of total respiration. Great variability exi…

Acute colectasia may occur in patients under mechanical ventilation. Causative factors include haemodynamic changes, potassium loss, underlying pathology (chronic respiratory failure, cirrhosis) and especially morphine-like compounds used for sedation. Analysis of the results obtained with various treatments suggests that surgery is not justified: caecal perforation is extremely rare in a previous healthy colon; any surgical procedure is hazardous in this type of patient, and colectasia frequently regresses under appropriate medical treatment, even though mechanical ventilation is pursued.

We studied a maximum fall in VE (max delta VE) produced by 20 sec O2 breathing and its percentage of resting VE (reflex hypoxic drive) on 9 menstruating women, in order to examine menstrual phase effect on reflex hypoxic drive. During the luteal phase, resting PAO2 was higher by 4.8 mm Hg and PACO2 was lower by 3.0 mm Hg than during the follicular phase. In spite of these changes in the alveolar gases, reflex hypoxic drive was greater in the luteal phase (14.4%) than in the follicular phase (9.3%). Resting VE in the luteal phase was 7.8% greater than that in the follicular phase. Contribution

Intracerebroventricular (ICV) administration of the stable adenosine analogue 2-chloroadenosine (2CA) to hyperoxic halothane-anesthetized rats produced a dose-dependent depression of respiration largely as a result of a decrease in tidal volume. Similar changes were noted after another adenosine analogue, phenylisopropyladenosine (PIA). Higher doses shifted the minute ventilation-PaCO2 curve to the right and decreased its slope. Bradycardia and hypotension were produced at doses which altered respiration. Neonatal destruction of brain serotonin or dopamine-containing nerve terminals did not al…

Respiration was increased approximately 5-fold with 0.05 microM norepinephrine and to a maximum of 10-fold by 0.30 microM norepinephrine. Prazosin, an alpha-adrenergic blocking agent highly selective for alpha 1-type receptors, partially inhibited the response to norepinephrine (0.05 microM) by 20-25% at a concentration of 0.10-1 microM. In contrast, when the stimulus for respiration was provided by isoproterenol or 3-isobutyl-1-methylxanthine, prazosin was without effect up to a concentration of 10 microM. Yohimbine, an alpha-adrenergic blocking drug preferential for alpha 2-receptors, did no…
