Console and bedside surgeon fused robot-assisted thoracic surgery.

In the last decade, even thoracic surgery has seen an increase in the use of robotic surgical systems, and robot-assisted thoracic surgery (RATS) is considered one of the main issues. While RATS is associated with solo manipulative freedom and high-definition optical systems, several disadvantages, such as the lack of tactile sensation and difficult learning curves for the whole team, have been raised. Therefore, to overcome these issues, we developed a 'fusion surgery' approach combining a robotic procedure with manual maneuvers, where the table surgeon retracts the lung and staples the pulmo

Anaphylactic Shock to Intravenous Indocyanine Green During a Robotic Right Colectomy.

Intravenous indocyanine green (IV ICG) is regarded as a safe immunofluorescence agent used to assess bowel perfusion prior to creating bowel anastomoses and aids in ureter identification during intra-abdominal surgery. We report the first instance of anaphylactic shock to IV ICG after prior toleration of ICG via an intra-ureteral route. Shortly after administering IV ICG, our patient became hypotensive and hypoxic requiring chest compressions, vasoactive medications, and thoracostomy tubes prior to identifying the symptoms as an allergic reaction. Anaphylaxis is not a recognized side effect of

Robotic Myotomy and Partial Fundoplication for Achalasia.

Laparoscopic Heller myotomy is currently considered the standard definitive treatment of achalasia. With the advancements in technology, robotic Heller myotomy has emerged as an alternative approach to traditional laparoscopy due to three-dimensional (3D) visualization, fine motor control, and improved ergonomics provided by the robot. Although there is a lack of randomized controlled trials, robotic-assisted Heller myotomy seems to be associated with lower rates of intraoperative perforations compared to the laparoscopic approach. A robotic approach may also improve surgical outcomes by provi

FiMec tremor stabilization spoon: design and active stabilization control of two DoF robotic eating devices for hand tremor patients.

This article is about vibration-damping robotic eating devices designed for use by people who have difficulty in eating due to hand tremors due to neuromuscular system disorder. The robotic eating device has two degrees of freedom (DoF). It contains an active controller structure to absorb vibrations in the y- and z-directions. In the handle part of the robotic eating device, there are two DC motors placed on the y- and z-axis, a three-axis IMU inertia sensor, an embedded system board, and a power unit. To absorb the vibration measured from the IMU sensor, the position control of the two motor

Comparison of Global Evaluative Assessment of Robotic Surgery with objective performance indicators for the assessment of skill during robotic-assisted thoracic surgery.

BACKGROUND: The Global Evaluative Assessment of Robotic Skills is a popular but ultimately subjective assessment tool in robotic-assisted surgery. An alternative approach is to record system or console events or calculate instrument kinematics to derive objective performance indicators. The aim of this study was to compare these 2 approaches and correlate the Global Evaluative Assessment of Robotic Skills with different types of objective performance indicators during robotic-assisted lobectomy. METHODS: Video, system event, and kinematic data were recorded from the robotic surgical system du

Full subcostal subxiphoid robotic-assisted radical thymectomy: preclinical cadaveric study for optimizing patient positioning, table settings, and port configuration.

Robotic subxiphoid transthoracic thymectomy showed several surgical advantages. Nevertheless, the intercostal insertion of trocars may lead to nerve injury with enhanced post-operative pain. Moreover, the dissection of peri-diaphragmatic mediastinal fat may result challenging, in particular on left side, where the presence of the heart precludes the optimal visualization. We describe a preclinical cadaveric study of a novel full subcostal robotic-assisted technique to overcome these limitations. A total subcostal robotic-assisted radical thymectomy was evaluated on a cadaver model using the da

The Role of AirSeal in Robotic Urologic Surgery: A Systematic Review.

Introduction: The adoption of a valveless trocar system in robotic surgery has allowed for stable pneumoperitoneum and constant smoke evacuation. The reported benefits of this system are improved visualization, lower abdominal pressures resulting in a decrease in cardiopulmonary complications, ileus, and postoperative pain. We endeavored to perform a systematic review of the available literature on the clinical and systems-based outcomes of AirSeal™ during robotic urologic surgery. Materials and Methods: We performed this review according to the preferred reporting items for systematic reviews

Multiscale, multi-perspective imaging assisted robotic microinjection of 3D biological structures.

Microinjection is a widely used technique employed by biologists with applications in transgenesis, cryopreservation, mutagenesis, labeling/dye injection and in-vitro fertilization. However, microinjection is an extremely laborious manual procedure, which makes it a critical bottleneck in the field and thus ripe for automation. Here, we present a computer-guided robot that automates the targeted microinjection of Drosophila melanogaster and zebrafish (Danio rerio) embryos, two important model organisms in biological research. The robot uses a series of cameras to image an agar plate containing

A Review of Robotic and OCT-Aided Systems for Vitreoretinal Surgery.

The introduction of the intraocular vitrectomy instrument by Machemer et al. has led to remarkable advancements in vitreoretinal surgery enabling the limitations of human physiologic capabilities to be reached. To overcome the barriers of perception, tremor, and dexterity, robotic technologies have been investigated with current advancements nearing the feasibility for clinical use. There are four categories of robotic systems that have emerged through the research: (1) handheld instruments with intrinsic robotic assistance, (2) hand-on-hand robotic systems, (3) teleoperated robotic systems, a

Submandibular gland excision: From external surgery to robotic intraoral and extraoral approaches.

Submandibular gland excision is generally performed through a transcervical approach, a safe procedure that, however, carries few neurological, functional and aesthetic drawbacks. Intraoral approach to the submandibular gland has been described many years ago but it has been newly proposed in recent years thanks to endoscope-assisted and robot-assisted surgical procedures. The main purpose of intraoral approaches is to avoid cosmetic sequelae and to reduce the risk of marginal nerve injury although the recent introduction of retroauricular, postauricular, facelift and transhairline neck incisi

How to manage a partial detachment of the ureter during robotic radical prostatectomy?

The management of a ureteral orifice injury occurring during robotic radical prostatectomy (RARP) represents a challenge for urologists. Several techniques have been proposed to treat an intraoperative injury, but intraoperative positioning of a DJ stent represents the most common treatment in cases of a partial injury of the ureteral orifice. We present a technique to ensure the successful outcome in cases of a partial detachment of the ureter during RARP. When the orifice is identified after the incision of the bladder neck and it appears very close to the anastomosis line setting up a parti

Retroperitoneal access for robotic renal surgery.

INTRODUCTION AND OBJECTIVE: Retroperitoneal access for robotic renal surgery is an effective alternative to the commonly used transperitoneal approach. We describe our contemporary experience and technique for attaining retroperitoneal access. MATERIALS AND METHODS: We outline our institutional approach to retroperitoneal access for the instruction of urologists at the beginning of the learning curve. The patient is placed in the lateral decubitus position. The first incision is made just inferior to the tip of the twelfth rib as described by Hsu, et al. After the lumbodorsal fascia is traver

Transoral robotic surgery (TORS) for benign pharyngeal lesions.

Transoral robotic surgery (TORS) offers a minimally invasive approach to the pharynx with a magnified 3-dimensional view, wristed instruments with 7 degrees of freedom, and tremor filtration. TORS affords an excellent approach to benign lesions of the pharynx. This article is grouped into subsites of the nasopharynx, oropharynx, and hypopharynx, addressing patient setup, surgical technique, and postoperative management of each subsite. Although TORS has been described primarily for resection of malignant lesions of the pharynx, the setup, exposure, and anatomy described herein are identical fo

Comparison of Perioperative Complications of Early-Stage Endometrial Cancer Between Laparotomy, Laparoscopic, and Robotic-assisted Surgery using DPC Data: A Retrospective Cohort Study in Japan.

Early-stage endometrial cancer can be cured surgically using three techniques: laparotomy, laparoscopic surgery, and robotic-assisted surgery. In this retrospective study, we used 4 years of data from the Japanese Diagnosis Procedure Combination to analyze perioperative sequelae for each surgical procedure. Patients with early-stage endometrial cancer were classified into three groups: laparotomy, laparoscopic surgery, and robotic-assisted surgery. The number of robotic-assisted surgeries is increasing, but hospitals with low surgical volumes performed laparotomy at a rate of 57.6%, while hosp

Adoption of robot-assisted minimally invasive surgery across surgical specialties in France: a nationwide population-based study.

Robotic minimally invasive surgery has been used in France for nearly two decades, with monitoring enabled by the National Health Data System (SNDS) since March 2020. No French study has comprehensively described robotic procedures, including ENT, at a fine geographical scale. We aimed to assess the use of robotic and non-robotic minimally invasive surgery over time and across French departments (administrative subdivisions), describe patient and healthcare facility characteristics and investigate determinants of robotic surgery use. We identified adults undergoing robotic or non-robotic minim

Robotic surgery for submandibular gland resection through a trans-hairline approach: The first human series and comparison with applicable approaches.

BACKGROUND: Many submandibular gland diseases are treated by surgical intervention. Transcervical surgery results in an obvious cervical scar, whereas the retroauricular approach leaves a scar in the postauricular area. Therefore, robotic submandibular gland resection through a trans-hairline approach is developed to improve postoperative aesthetic outcomes. METHODS: From 2012 to 2017, 24 consecutive patients who met the inclusion criteria and had received submandibular gland resection through the trans-hairline robotic surgery were analyzed. RESULTS: All surgical procedures were successfull
