Feedback-controlled robotics-assisted treadmill exercise to assess and influence aerobic capacity early after stroke: a proof-of-concept study.

PURPOSE: The majority of post-stroke individuals suffer from low exercise capacity as a secondary reaction to immobility. The aim of this study was to prove the concept of feedback-controlled robotics-assisted treadmill exercise (RATE) to assess aerobic capacity and guide cardiovascular exercise in severely impaired individuals early after stroke. METHOD: Subjects underwent constant load and incremental exercise testing using a human-in-the-loop feedback system within a robotics-assisted exoskeleton (Lokomat, Hocoma AG, CH). Inclusion criteria were: stroke onset ≤8 weeks, stable medical condi…

Stepwise implementation of robotics and a certified enhanced recovery program significantly improves multiple outcome metrics in a liver surgery unit.

INTRODUCTION: Minimally-invasive surgery (MIS - including laparoscopy and robotics) and enhanced recovery after surgery (ERAS®) programs reduce complications and improve functional outcome for cancer patients. Their combined impact on liver surgery cohorts remains unclear. MATERIALS AND METHODS: This Austrian single centre cohort study assesses the stepwise implementation of laparoscopy, robotic-assisted-surgery (RAS), and an audited ERAS® certification. Three periods (17 months each; January 2020 to 06/2024) were compared: P1 (pre-RAS/pre-ERAS®), P2 (RAS/ERAS®-like), and P3 (RAS/ERAS®). Outc…

Sulphonated-graphene oxide nanocomposite membranes with PVA-ZnO nanostructures and mechanized agitation-enhanced pt coating for soft robotics bending actuator.

Ionic Polymer-Metal Composite (IPMC) actuators have garnered significant scientific attention in robotics and artificial muscles for their ability to operate at low voltage, high strain capacity, and lightweight construction. The lack of uniform bending in IPMC actuators undermines their control precision and restricts their range of potential applications. This study utilized the unique properties of nanoscale materials and Polyvinyl alcohol (PVA) to develop a membrane for soft robotic bending actuation. Subsequently, a platinum coating was applied to the membrane to mitigate the limitations …

Meniscus-Climbing System Inspired 3D Printed Fully Soft Robotics with Highly Flexible Three-Dimensional Locomotion at the Liquid-Air Interface.

Soft robotics locomotion at the liquid-air interface has become more and more important for an intelligent society. However, existing locomotion of soft robotics is limited to two dimensions. It remains a formidable challenge to realize three-dimensional locomotion (X, Y, and Z axes) at the liquid-air two-phase interface due to the unbalanced mechanical environment. Inspired by meniscus-climbing beetle larva Pyrrhalta, the mechanism of a three-phase (liquid-solid-air) contact line is here proposed to address the aforementioned challenge. A corresponding 3D printed fully soft robotics (named la…

Application of a robotics path planning algorithm to assess the risk of mobile bearing dislocation in lateral unicompartmental knee replacement.

Due to ligament laxity, bearing dislocation occurs in 1-6% of Oxford Domed Lateral (ODL) replacements with most dislocations occurring medially. Dislocations were studied using a previously built mechanical rig, however testing using the rig was inefficient. The aim of this study was to develop a better tool that was more reliable and efficient. An established robotics software package, the Open Motion Planning Library, was modified to accept the ODL components. Using a robotics path planning algorithm, the mobile bearing was allowed to find a way out from between the femoral and tibial compon…

Robotic Central Hepatectomy.

INTRODUCTION: Central hepatectomies for giant liver tumors are one of the most complex procedures in liver surgery. The use of minimally invasive techniques using the robotic platform for centrally located liver tumors is rarely reported.1-3 PATIENTS AND METHODS: We herein report a case of a 69-year-old female patient who presented with a solitary lesion from metastatic breast cancer in liver segments 4, 5, and 8. A primary systematic treatment was delivered before a surgical resection was considered by the multidisciplinary tumor conference in this selective case, although evidence for surviv

[Robotic Central Pancreatectomy].

Central pancreatectomy is an excellent alternative to left pancreatectomy for symptomatic benign or premalignant lesions of the pancreatic body or tail. A key advantage of this technique lies in the preservation of pancreatic parenchyma, resulting in a lower rate of postoperative diabetes mellitus. However, this procedure requires more complex reconstruction, which in turn is associated with an increased risk of morbidity.Insulinoma in the pancreatic body.Robot-assisted central pancreatectomy with pancreaticojejunostomy using a modified Blumgart technique.Central pancreatectomy is a generally

[Robotic esophageal surgery].

Robot-assisted minimally invasive esophagectomy (RAMIE) is increasingly becoming established as a standard procedure in surgical centers for esophagectomy in cases of cancer. To date, RAMIE has been shown to have fewer postoperative complications and at least equivalent oncological outcomes compared with open resection. Compared with classical minimally invasive resection, there seem to be fewer cases of postoperative pneumonia after RAMIE. In addition, a higher number of harvested lymph nodes could lead to better oncological long-term outcomes. The learning curve for this complex surgical pro

Robotic Mediastinal Surgery.

The robotic platform can be viewed as an advanced thoracoscopic instrument and can be utilized for any pathology amenable to thoracoscopic surgery. This ultimately comes down to surgeon comfort, but many have demonstrated the robotic approach to be useful in benign and malignant mediastinal disease in all compartments with at least equivalent-if not superior-outcomes compared to sternotomy for many metrics. There are various robotic approaches to the same compartments (such as with thymectomy), and no one robotic approach has proven superior to another. Here we describe our robotic approach to

Robotic Surgery for Tracheobronchomalacia.

Robotic tracheobronchoplasty (TBP) is a new surgical approach to treat tracheobronchomalacia (TBM), and is based on open TBP, which has undergone various technical iterations since the 1950s. The robotic approach to TBM may allow for more patients to undergo TBP, and in retrospective series has demonstrated equivalent outcomes compared to open TBP if not superior in terms of postoperative complications. Long-term data are eagerly awaited, and we describe our approach in this article.

Robotic Ureteral Reconstruction.

Robotically assisted laparoscopic techniques may be used for proximal and distal ureteral strictures. Distal strictures may be approached with ureteroneocystotomy, psoas hitch, and Boari flap. Ureteroureterostomy, buccal mucosa graft ureteroplasty, and appendiceal flap ureteroplasty are viable techniques for strictures anywhere along the ureter. Ileal ureteral substitution is reserved for more extensive disease, and autotransplantation is reserved for salvage situations.

Robotic Ear Surgery.

Robots under research and development for otology can be classified as collaborative (intervention is constrained by the robot but surgeon directly actuates the end-effector), teleoperated (surgeon remotely controls a tool with modification [eg, tremor reduction] by the robot), or autonomous (surgeon monitors the robot performing a task). Current clinical trials focus on more accurate stapes surgery, minimally invasive access to the cochlea, and less traumatic insertion of cochlear implant electrode arrays. Autonomous approaches to major aspects of surgical interventions (eg, mastoidectomy) wi
