The Incidence of Irritable Bowel Syndrome in Children Using the Rome III Criteria and the Effect of Trimebutine Treatment.

BACKGROUND/AIMS: Irritable bowel syndrome (IBS) is one of the most common functional gastrointestinal disorders and when compared to the vast knowledge pertaining to adults with IBS, very little is known about IBS in children and adolescents. We aimed to explore the prevalence of IBS, identify symptoms and contributing factors and also to examine the efficacy of trimebutine maleate in children and adolescents. METHODS: The study involved 345 children and adolescents (4-18 years) and parents were requested to fill in a questionnaire, Rome III criteria was used to diagnose IBS. To exclude organ…

Anxiety is associated with uninvestigated and functional dyspepsia (Rome III criteria) in a Swedish population-based study.

BACKGROUND & AIMS: The Rome III criteria for functional dyspepsia have been changed to include 2 distinct syndromes: postprandial distress syndrome and epigastric pain syndrome. We investigated risk factors for functional dyspepsia among the functional dyspepsia subgroups defined by the Rome III criteria. METHODS: We performed a cross-sectional population-based study in a primary care setting (the Kalixanda study). A random sample (n = 2860) of the adult population from 2 northern Swedish municipalities (n = 21,610) was surveyed using a validated postal questionnaire to assess gastrointestina…

Use of surrogate markers of inflammation and Rome criteria to distinguish organic from nonorganic intestinal disease.

BACKGROUND & AIMS: Differentiating symptoms of irritable bowel syndrome (IBS) from those of organic intestinal disease is a familiar problem for physicians. The aim of this study was to assess the sensitivity, specificity, and odds ratios (ORs) of fecal calprotectin, small intestinal permeability, Rome I criteria, and laboratory markers of inflammation (erythrocyte sedimentation rate [ESR], C-reactive protein [CRP], blood count) in distinguishing organic from nonorganic intestinal disease. METHODS: A total of 602 new referrals to a gastroenterology clinic who had symptoms suggestive of IBS or…

Additional investigations fail to alter the diagnosis of irritable bowel syndrome in subjects fulfilling the Rome criteria.

OBJECTIVE: Irritable bowel syndrome (IBS) is diagnosed by the presence of a constellation of symptoms fulfilling the Manning or Rome Criteria, after exclusion of organic disease. To exclude other diagnoses that might contribute to the abdominal pain or bowel symptoms experienced by subjects with IBS, numerous screening algorithms have been advocated, incorporating lactose hydrogen breath tests, thyroid function tests, fecal ova and parasite determination, and colonic endoscopy/radiography. The utility of these tests in uncovering alternative diagnoses, other than IBS, was examined in 1452 pati…

Distinct pain profiles across irritable bowel syndrome subtypes: results from the Rome Foundation Global Epidemiology Study.

Abdominal pain is the hallmark symptom of irritable bowel syndrome (IBS), yet its clinical expression varies considerably across subtypes. Despite this, few large-scale studies have compared pain features across all Rome IV-defined IBS subtypes. We aimed to delineate differences in abdominal pain patterns across IBS subtypes and evaluate the contribution of psychological comorbidities, extra-intestinal symptoms, and intermediate factors associated with psychological distress. Data were derived from the internet survey of the Rome Foundation Global Epidemiology Study, encompassing 54,127 partic…

Editorial: Clinical Implications of Diagnosing Irritable Bowel Syndrome: Do All Roads Need to Lead to Rome?

Irritable bowel syndrome (IBS) is one of the most common diagnoses made by healthcare providers. Yet the majority of patients with IBS are undiagnosed. The study by Sayuk et al. allows insight into the characteristics of different patient groups, e.g., with and without a formal diagnosis of diarrhea predominant IBS (IBS-D). We discuss the questions raised by this study regarding the importance of making a confident diagnosis, conveying it to patients and their implications for clinical practice.

Recurrent abdominal pain: symptom subtypes based on the Rome II Criteria for pediatric functional gastrointestinal disorders.

OBJECTIVES: Recurrent abdominal pain (RAP) is a common childhood complaint rarely associated with organic disease. Recently, the Pediatric Rome Criteria were developed to standardize the classification of pediatric functional gastrointestinal disorders (FGIDs) using a symptom-based approach. The authors tested the hypothesis that most patients with childhood RAP could be classified into one or more of the symptom subtypes defined by the Pediatric Rome Criteria. METHODS: Using a prospective longitudinal design, new patients with RAP (n = 114) were studied at a tertiary care children's medical …

All roads lead to Rome: N-acetylation of GBS Capsular Polysaccharides is linked to conjugate vaccine immunogenicity.

Streptococcus agalactiae or Group B Streptococcus (GBS) is a major causative agent of neonatal sepsis, meningitis, still births and postnatal complications in women. Capsular polysaccharides (CPS) on the bacterial surface are implicated in pathogen virulence by evasion of host cellular defense mechanisms and have in turn served as attractive vaccine candidates for maternal immunization. In this study, we probed the role of a key structural feature, N-acetylation of constituent sugars in GBS capsular polysaccharide, on product characteristics and preclinical immunogenicity. Loss of N-acetyl gro

Prevalence of psychiatric disorders in patients of irritable bowel syndrome fulfilling Rome IV criteria: An observational study.

BACKGROUND: Rome IV criteria for the diagnosis of irritable bowel syndrome have been introduced recently hence, there are very few studies conducted to know the prevalence of IBS using these criteria. Prevalence of psychiatric disorder vary depending on criteria used. In view of these facts, this study was conducted to estimate prevalence of psychiatric disorders in patients of IBS fulfilling Rome IV criteria. MATERIAL AND METHODS: Observational study at a tertiary care hospital in northern India. We included 100 patients aged ≥18 years who were diagnosed as irritable bowel syndrome fulfillin…

Association Between Rome Classification Among Hospitalized Patients With COPD Exacerbations and Short-Term and Intermediate-Term Outcomes.

BACKGROUND: Recently, the Rome proposal updated the definition of exacerbation of COPD (ECOPD). However, such severity grade has not yet demonstrated intermediate-term clinical relevance. RESEARCH QUESTION: What is the association between the Rome severity classification and short-term and intermediate-term clinical outcomes? STUDY DESIGN AND METHODS: We retrospectively grouped hospitalized patients with ECOPD according to the Rome severity classification (ie, mild, moderate, severe). Baseline, clinical, microbiologic, gas analysis, and laboratory variables were collected. In addition, data …

Irritable bowel syndrome based on Rome IV diagnostic criteria associates with non-motor symptoms of Parkinson's disease.

BACKGROUND: Recent concepts suggest that the neuropathological hallmark of Parkinson's disease (PD) may in part originate from the enteric nervous system. We evaluated the frequency of functional gastrointestinal disorders in PD patients using Rome IV criteria and correlated the clinical severity of PD. METHODS: PD patients and matched controls were recruited between January 2020 and December 2021. Rome IV criteria were used to diagnose constipation and irritable bowel syndrome (IBS). Severity of PD motor symptoms was evaluated using UPDRS part III scores and non-motor symptoms using Non-moto…
