Twenty-three patients with spontaneous rupture of a pulmonary cavity with a pyopneumothorax resulting from coccidioidomycosis are presented. Clinical and laboratory findings, medical and surgical treatment, and complications are detailed. Skin tests are not helpful in making a diagnosis. Although complement fixation titers were elevated in all patients and cultures were positive in 21, these laboratory tests should not delay surgical treatment. A family with hereditary ovalocytosis (HO) is described. The probands, 2 brothers, had splenic rupture after modest trauma as preenting symptoms. 7 members of the family had HO. The sister of the pobands had a moderately enlarged spleen. The other members proved normal on routine clinical examination.

Ninety-three incidents of uterine rupture are reviewed. There is a distinct difference in both the fetal and maternal outcome between the group with a previously scarred uterus and the group with no previous scarring. Rupture of the unscarred uterus is a more dramatic event. The most common etiologic factors appear to be oxytocin, cephalopelvic disproportion, grand multiparity, and abruptio placentae. Our investigations on premature rupture of the fetal membranes (PROM) show that prematurity represents a higher risk for the fetus than infection does. Therefore we tend to be expectant in case of PROM combined with prematurity. On the other hand latency between rupture of the membranes and onset of labor should not exceed 24 hours for a mature fetus (gestational age greater than 37 weeks, estimated weight greater than 2500 g, L/S-ratio greater than 2). Antibiotics are applied only in case of manifest infection and at the onset of labor following a latency longer than 24 hours.

Hepatic rupture as a late complication of toxemic pregnancy is a rare yet lethal condition requiring rapid recognition and surgical management. Five cases of occult or 'spontaneous' rupture of the spleen are described. Pathological examination of the spleen showed changes of infectious mononucleosis in 2 cases, Hodgkin's disease in 1, amyloidosis in 1 and in the remaining spleen no underlying disease process was seen. From 1964 to 1974 14 patients affected with a delayed rupture of the spleen, were operated in the Surgical Clinic of the University of Mainz. 12 of these 14 patients had a polytrauma. Head-, brain- and thorax injuries were prevalent. Because of the risk of a delayed spleen rupture each blunt trauma has to be hospitalized and observed under clinical conditions. Non-traumatic spontaneous rupture of the otherwise normal bladder is a rare occurrence. The literature is reviewed and 4 new cases are reported. Experimental data are presented and a theory is reviewed regarding the frequent intraperitoneal location of the rupture. The frequent paucity of symptoms compared to the high mortality rate is stressed.
