OBJECTIVES: To examine health inequalities faced by individuals living with mental illness in rural Wyoming and to explore how the Clubhouse model helps address these challenges.
METHODS: This qualitative secondary analysis used semi-structured interview data from a prior study on mental health stigma in rural Wyoming. Sixteen current Clubhouse members (n = 12) and staff (n = 4 ) were recruited from the only Clubhouse in Wyoming between August and September 2024. Data were analyzed using Braun and Clarke's reflexive thematic analysis in NVivo.
RESULTS: Two major themes emerged: contributors

BACKGROUND/OBJECTIVES: Limited data exist on care delivery, engagement, and clinical outcomes among rural populations accessing telehealth mental health services, particularly within integrated psychotherapy-and-psychiatry models. This retrospective observational 12-week cohort study aimed to examine access, engagement, and preliminary clinical outcomes for rural and non-rural patients receiving services through a national outpatient telehealth mental health program. Engagement and clinical outcomes were expected to be comparable among rural and non-rural patients.
METHODS: Descriptive and in

BACKGROUND: The mental health of the older adult in western rural areas is an area of growing concern. Mental health, as defined by the World Health Organization, encompasses emotional, psychological, and social well-being, affecting how individuals think, feel, and act. In this study, we operationalize mental health through four validated dimensions: depression (emotional well-being), anxiety (psychological stability), loneliness (social connectedness), and life satisfaction (overall subjective well-being), which together provide a comprehensive assessment of mental health status. BACKGROUND: Rural individuals with atrial fibrillation (AF) experience challenges to anticoagulation adherence and self-management of the condition. We tested an intervention to improve anticoagulation adherence, quality of life, and health care utilization in rural individuals with AF.
METHODS: We randomized rural patients with AF receiving anticoagulation to receive a smartphone-based relational agent (for disease education and adherence guidance) and a heart rate and rhythm monitor for 4 months or a smartphone-based health education app. Adherence was determined with 12-month proportion of

BACKGROUND: Health workers have traditionally delivered health promotion and education to rural communities in the Global South in paper leaflet formats or orally. With the rise of digital technologies, health promotion and education can be provided in innovative and more effective formats, which are believed to have a higher impact on disease prevention and treatment.
OBJECTIVE: The aim of this tutorial is to illustrate how a multi- and interdisciplinary approach can be applied in the design process of digital health messages for use in the Global South.
METHODS: The multi- and interdiscipl

INTRODUCTION: Facing the challenges of aging populations, increasing chronic diseases prevalence and health system fragmentation, there have been several pilots of integrated health systems in China. But little is known about their structure, mechanism and effectiveness. The aim of this paper is to analyze health system integration and develop recommendations for achieving integration.
METHOD: Huangzhong and Hualong counties in Qinghai province were studied as study sites, with only Huangzhong having implemented health system integration. Questionnaires, interviews, and health insurance recor

The good quality laboratory services in developing countries are often limited to major urban centers. As a result, many commercially available high-quality diagnostic tests for infectious diseases are neither accessible nor affordable to patients in the rural areas. Health facilities in rural areas are compromised and this limits the usability and performance of the best medical diagnostic technologies in rural areas as they are designed for air-conditioned laboratories, refrigerated storage of chemicals, a constant supply of calibrators and reagents, stable electrical power, highly trained p

The Donald W. Reynolds Institute on Aging at the University of Arkansas for Medical Sciences in Little Rock is addressing one of the most pressing policy issues facing the United States: how to care for the burgeoning number of older adults. In 2001, the Institute created the Arkansas Aging Initiative, which established seven satellite centers on aging across the state using $1.3 to $2 million dollars annually from the state's portion of the Master Tobacco Settlement. These centers on aging assist the state's population of older adults, many of whom reside in rural areas, live in poverty, and

In spite of the fact that feeble levels of participation have long been identified as a major constraint to the successful long-term implementation of community-based health insurance (CBI) in low-income countries, evidence on determinants of enrolment in CBI is still lacking. The application of econometric modelling has provided a partial answer to the question, but on its own it has proved to be insufficient to guide policy making. This paper aims to fill this gap in knowledge using qualitative research methods. In-depth interviews with 32 household heads were conducted in the Nouna Health D

AIMS: To investigate the epidemiology of illness among young infants at remote health clinics in a rural developing country, and to determine risk factors for mortality that might be used as triggers for emergency treatment or referral.
METHODS: Multi-site 12 month observational study of consecutive presentations of infants less than 2 months, and an investigation of neonates who died in one district without accessing health care.
RESULTS: Forty per cent of 511 young infant presentations occurred in the first week of life and most of these in the first 24 hours. Twenty five deaths were recor

This inter-disciplinary study compares the health status of school-age children in Ghana, both enrolled and non-enrolled, and examines these results within a wider socio-economic and socio-cultural context including kinship and livelihood. Children matched for age and sex who were not enrolled in Primary School were significantly shorter and more stunted than enrolled children were, and 70% of all Primary school-age children were anaemic. Young children from farming communities were significantly more undernourished than children from fishing communities. Adolescent non-enrolled boys were more

Access to health care has been considered central to understanding older rural adults' utilization of health services. Access has been measured in terms of distance to care, use of care, and barriers to care; because older persons' relevant perceptions have received little attention, the validity of the typical access constructs is questionable. During a phenomenological study of their experiences of living alone at home, eight older rural widows shared perceptions about the locales where they received primary care and specialty care. A facet of their life-worlds, "staying close to shore," was
