Estimation of distal arm joint angles from EMG and shoulder orientation for transhumeral prostheses.

In this paper, we quantify the extent to which shoulder orientation, upper-arm electromyography (EMG), and forearm EMG are predictors of distal arm joint angles during reaching in eight subjects without disability as well as three subjects with a unilateral transhumeral amputation and targeted reinnervation. Prior studies have shown that shoulder orientation and upper-arm EMG, taken separately, are predictors of both elbow flexion/extension and forearm pronation/supination. We show that, for eight subjects without disability, shoulder orientation and upper-arm EMG together are a significantly

A larger critical shoulder angle requires more rotator cuff activity to preserve joint stability.

Shoulders with rotator cuff tears (RCT) tears are associated with significantly larger critical shoulder angles (CSA) (RCT CSA = 38.2°) than shoulders without RCT (CSA = 32.9°). We hypothesized that larger CSAs increase the ratio of glenohumeral joint shear to joint compression forces, requiring substantially increased compensatory supraspinatus loads to stabilize the arm in abduction. A previously established three dimensional (3D) finite element (FE) model was used. Two acromion shapes mimicked the mean CSA of 38.2° found in patients with RCT and that of a normal CSA (32.9°). In a first step

Associations among shoulder strength, glenohumeral joint motion, and clinical outcome after rotator cuff repair.

Rotator cuff tears are a common condition causing pain and disability, but the relationships among clinical measures of shoulder function and measures of glenohumeral joint (GHJ) function are not well known. In the study reported here, dynamic in vivo GHJ motion was measured during abduction from biplane radiographs in 22 rotator cuff repair (RCR) patients and 36 control subjects. Isometric shoulder strength was measured and clinical outcomes were assessed using the Western Ontario Rotator Cuff (WORC) Index. Associations among WORC, GHJ motion, and several shoulder strength ratios were assesse

Shoulder function after arthroscopic anterior stabilization of the glenohumeral joint using an absorbable tac.

Among the advantages of arthroscopic stabilization could be less loss of range of motion and, with that, better functional outcome. In each of 24 shoulders, arthroscopic anterior repair of instability was performed through use of cannulated resorbable tacs (Suretacs), and the clinical outcome, especially the function, was examined in 20 shoulders after a mean follow-up of 3.4 years. The study included a retrospective analysis of 10 shoulders with documented recurrent anterior dislocations, 3 shoulders with posttraumatic subluxations, and 7 shoulders with painful apprehension and unequivocally

In vivo assessment of the subacromial space during shoulder elevation following acromioclavicular joint reconstruction.

High-grade disruptions to the acromioclavicular joint (ACJ) significantly impair scapular function, which may compromise the subacromial space and increase the risk of impingement related pathology. Research has traditionally focused on the minimum distance between the humeral head and undersurface of the acromion, which may not accurately address the space above the supraspinatus footprint. This study aimed to quantify the subacromial space between the anterior acromion and the supraspinatus footprint, as well as the general humeral-acromial spacing during muscle-driven shoulder motion, in in

Case Report: Salmonella Infection of a Prosthetic Shoulder Joint Requiring Long-term Antibiotic Suppression.

Salmonella infection most frequently presents as gastroenteritis in developed countries, while septic arthritis represents a much rarer occurrence. Salmonella septic arthritis most commonly occurs in the hip joint, while existing reports of Salmonella infection of the shoulder joint are limited to the pediatric population. Here we report a case of an immunocompetent elderly male presenting with Salmonella septic arthritis to the left shoulder joint status post total shoulder arthroplasty three years prior. The definitive source of infection remains unclear, though potential etiologies include …

Shoulder Joint Hybrid Assistive Limb Treatment for Chronic Stroke Patients with Upper Limb Dysfunction.

Upper extremity dysfunction after stroke affects quality of life. Focusing on the shoulder joint, we investigated the safety and effectiveness of rehabilitation using a shoulder joint hybrid assistive limb (HAL). Eight patients with chronic stroke and upper extremity functional disability were enrolled and used a shoulder joint HAL, which assisted shoulder movement based on the user's intention, through myoelectric activation of the shoulder flexor. Ten training sessions of 30-40 min each were performed to assist voluntary movement of upper limb elevation on the affected side through triggerin…

Severe acromioclavicular joint osteoarthritis is associated with acromial stress fractures after reverse shoulder arthroplasty.

BACKGROUND: Little is known about the role of disorders of the acromioclavicular joint (ACJ) and how they relate to complications after reverse shoulder arthroplasty (RSA). The purpose of this study is to compare the severity of ACJ osteoarthritis in patients undergoing RSA with and without postoperative acromial and scapular spine fractures. METHODS: A retrospective review was performed to identify all patients who underwent primary RSA between 1/1/2007 and 10/31/2019 with a postoperative acromial or scapular spine stress fracture from a single institution. Patients who underwent RSA with a

Pseudomonas aeruginosa infection of shoulder joint after latissimus dorsi tendon transfer: A case report.

Infection rates of arthroscopic procedures have been consistently reported at approximately 1% and are even less common in shoulder arthroscopy (0.3%). We are unaware of any prior reports of infection associated with an arthroscopic-assisted latissimus dorsi transfer and report on a 60-year-old male who experienced this event. At the 2-month follow-up, he reported an infection of the shoulder joint, characterized by a fistula on the portal scar. Laboratory tests revealed a Pseudomonas aeruginosa infection which was treated with arthroscopic irrigation and debridement of the shoulder joint foll…

Functional integrity of the shoulder joint and pectoralis major following subpectoral implant breast reconstruction.

Subpectoral implants for breast reconstruction after mastectomy requires the surgical disinsertion of the sternocostal fiber region of the pectoralis major. This technique is associated with significant shoulder strength and range of motion deficits, but it is unknown how it affects the underlying integrity of the shoulder joint or pectoralis major. The aim of this study was to characterize the long-term effects of this reconstruction approach on shoulder joint stiffness and pectoralis major material properties. Robot-assisted measures of shoulder strength and stiffness and ultrasound shear wa…

Electromyographic analysis of shoulder joint function of the biceps brachii muscle during isometric contraction.

Surface electromyography was performed for both heads of the biceps brachii in 11 healthy men while the muscles were under 30% maximum isometric shoulder flexion and abduction. Elbow related biceps activity was minimized by using a brace locked in neutral forearm rotation. Electromyographic activity was normalized as a percentage of maximal muscle contraction during 24 shoulder motions. Electromyographic activity was detected in all motions examined, suggesting that the biceps muscle acts as a flexor and an abductor of the shoulder. Both heads of the biceps muscle had higher activities during

Shoulder joint rupture in juvenile rheumatoid arthritis producing bicipital masses and a hemorrhagic sign.

Multiple episodes of shoulder joint rupture leading to the development of bicipital masses occurred in a 6-yr-old boy with the systemic form of juvenile rheumatoid arthritis. Observations made during the rapid evolution of the masses included early physical findings in the proximal upper arm which suggested a malignancy. An unusual hemorrhagic sign appeared in the cease of the elbow. A sequence of events is postulated to explain the course of development of these masses.

Incidence, risk factors, characteristics and shoulder joint function status of heterotopic ossification following surgical treatment of clavicle fracture or acromioclavicular joint dislocation.

BACKGROUND: Heterotopic ossification (HO) around the shoulder is common in patients with clavicle fracture or acromioclavicular joint (ACJ) dislocation. Periarticular HO sometimes leads to poor joint function. HO of the elbow and hip requires active prevention. However, studies on the incidence, risk factors, characteristics, and shoulder joint function of HO around the shoulder after surgical treatment of clavicle fracture or ACJ dislocation are still lacking. STUDY HYPOTHESIS: ACJ dislocation, clavicular hook plate fixation and excessive screw length are independent high-risk factors for po…

Evaluation of acid production as a metabolic phenotype in Cutibacterium acnes shoulder periprosthetic joint infections.

BACKGROUND: Cutibacterium acnes (C. acnes) is the most frequently isolated organism in shoulder periprosthetic joint infection, yet distinguishing contamination from infection remains challenging. Current diagnostic frameworks rely on culture positivity and consensus criteria but provide limited insight into bacterial metabolic activity. Since C. acnes produces short-chain fatty acids influencing host inflammation, acid production may represent a relevant metabolic phenotype beyond simple biomass. METHODS: An exploratory cohort of 40 clinical C. acnes isolates were selected from a larger repo

On the use of inertial measurement units and musculoskeletal modeling in the calculation of muscle and joint loading at the shoulder.

Inertial measurement units (IMUs) are wearable motion sensing devices that support low-cost human kinematics measurement outside of the laboratory; however, the impact of IMU motion measurement errors on estimates of upper limb muscle and joint force using musculoskeletal modeling remains poorly understood. The aims of this study were to measure upper limb kinematics using IMUs and optical motion analysis, and evaluate the differences in shoulder musculoskeletal model estimates of muscle and joint forces when using IMU-based motion analysis and optical motion analysis data. Thirty healthy adul
