UNLABELLED: During revision total shoulder arthroplasty, bone grafting severe glenoid defects without concomitant reinsertion of a glenoid prosthesis may be the only viable reconstructive option. However, the fate of these grafts is unknown. We questioned the durability and subsidence of the graft and the associated clinical outcomes in patients who have this procedure. We retrospectively reviewed 11 patients with severe glenoid deficiencies from aseptic loosening of a glenoid component who underwent conversion of a total shoulder arthroplasty to a humeral head replacement and glenoid bone gra

BACKGROUND: Normal function of the upper limb is seldom restored after limb-sparing surgery for tumors of the proximal humerus. The literature suggests superior shoulder function is achieved in the short term with reverse total shoulder arthroplasty compared to other techniques when performed for conditions with rotator cuff deficiency. It is unclear whether this superiority is maintained when reverse total shoulder arthroplasty is performed for tumors.
QUESTIONS/PURPOSES: When performed for tumors, we determined whether reverse total shoulder arthroplasty restores function and improves motio

INTRODUCTION: Reverse shoulder arthroplasty (RSA) is a key tool in the orthopedic trauma surgeon's arsenal, especially when faced with a proximal humerus fracture in older patients. However, few studies have focused on the glenoid side of RSA in this indication as the implant is placed in a generally healthy scapula.
HYPOTHESIS: Glenoid implants for RSA after trauma are well positioned and do not often cause complications.
MATERIAL AND METHODS: Retrospective multicenter study of 513 patients who underwent RSA because of a proximal humerus fracture. The mean follow-up was 55 months. Radiograp

BACKGROUND: A leading cause of anatomic total shoulder arthroplasty failure continues to be glenoid component loosening. Historically, metal-backed glenoids (MBGs) have shown decreased performance compared with polyethylene-backed glenoid (PEG) components. Previous studies have shown that MBG components have higher revision rates and shorter time to prosthetic failure compared with PEG components. However, the negative outcomes of many of these studies may have been linked to an examination of older generations of MBG implants, and do not account for newer technology. Thus, our study aims to a

Bone deficiencies of either the humeral head or glenoid fossa may cause recurrent shoulder instability following soft tissue stabilization procedures. The engaging Hill-Sachs lesion, a major risk factor for instability, has been identified in a majority of patients with recurrent anterior instability. Guidance for surgical management of large humeral head deficiency presents few available options, with even fewer clinical data to support any one technique. Anteroinferior glenoid deficiency has also been a well-documented source of recurrent instability. The Latarjet coracoid transfer procedure

Periprosthetic joint infection (PJI) is one of the most frequent reasons for painful shoulder arthroplasties and revision surgery of shoulder arthroplasties. Cutibacterium acnes (Propionibacterium acnes) is one of the microorganisms that most often causes the infection. However, this slow growing microorganism is difficult to detect. This paper presents an overview of different diagnostic test to detect a periprosthetic shoulder infection. This includes nonspecific diagnostic tests and specific tests (with identifying the responsible microorganism). The aspiration can combine different specifi

Arthroscopy may be used to diagnose symptomatic glenoid loosening following anatomic total shoulder arthroplasty (aTSA), and arthroscopic glenoid component removal may suffice as treatment for some patients. Arthroscopy may be used to diagnose shoulder prosthetic joint infection (PJI) following aTSA and reverse shoulder arthroplasty (RSA). Tissues are biopsied from multiple sites within the joint and the subdeltoid or subacromial space, and they are cultured for a minimum of 14 days. Postoperative adhesions arising after prosthetic shoulder arthroplasty (PSA) may be released arthroscopically.

A 72-year-old man with a history of right reverse shoulder arthroplasty presented with a 1-month history of erythema, pain and drainage from the right shoulder. Arthrocentesis was performed and synovial fluid gram stain revealed gram-positive rods. Clinical diagnosis of prosthetic shoulder joint infection was made. Orthopaedic surgeons performed irrigation and debridement with resection of the right shoulder prothesis and implantation of an antimicrobial spacer. Operative cultures grew Actinomyces neuii The patient was treated with 6 weeks of ceftriaxone with improvement in both clinical sympt

INTRODUCTION: Management of periprosthetic shoulder infections (PSIs) still remains challenging. We conducted a retrospective case study to assess the outcomes of definitive articulating antibiotic spacer implantation in a cohort of elderly, low-demanding patients. We hypothesized that in patients with low functional demands seeking pain relief with chronic PSIs, treatment with a definitive articulating antibiotic spacer would lead to satisfying results concerning eradication of the infection, improvement of pain, and improving shoulder function.

BACKGROUND: The control of shoulder-level disarticulation prostheses is significantly more difficult than that of prostheses for more distal amputations. Amputees have significant difficulties coordinating the separate functions of prosthetic shoulder, elbow, wrist, and hand/hook components. The user must lock one joint at a particular position in space before subsequently moving a different joint.
METHODS: A patient with bilateral humeral disarticulations after an electrical injury underwent a novel nerve transfer procedure designed to improve the control of a myoelectric prosthesis. The med

BACKGROUND: Different surgical procedures have been proposed for the treatment of Distal Radioulnar Joint (DRUJ) arthrosis and other conditions. This study aimed to introduce a new design of DRUJ prosthesis based on the Sauvé-Kapandji procedure followed by the evaluation of its short-term results. Darrach and Sauvé-Kapandji techniques are two well-known salvage procedures. Various implant designs have been proposed for DRUJ substitution to avoid the disadvantages of these procedures.
METHODS: Before and after the insertion of the intraosseous DRUJ prosthesis in five patients, indices, such as

The authors report the case of a 46-year old woman who, 7 years after bilateral implantation of a silicone breast prosthesis, presented with a seropositive rheumatoid arthritis involving the shoulders, wrists, metacarpophalangeal and interphalangeal joints of both hands, as well as the knees and ankles. Six months after the onset of the arthritis, she developed a Raynaud's syndrome and a lacrymal and salivary dry syndrome with unilateral axillary lymphadenopathy. No regression followed the removal of the breast prostheses, one of which was found broken.

PURPOSE: The reverse shoulder prost hesis (rTSA) is now implanted by the same percentage of anatomic shoulder prosthesis in the USA. Scapular notching and loss of extrarotation have been underlined as complication at long-term follow-up due to the Grammont design. The current trend to reduce those limits is to position both components lateralized. As the role of the subscapularis tendon in this new rTSA design is unclear, the purpose of this study is to quantify rTSA outcomes in patients with or without subscapularis tendon suture.
METHODS: The surgery was performed by the same orthopaedic su…

BACKGROUND: Reverse shoulder arthroplasty (rTSA) has become increasingly common for inflammatory arthritis (IA) due to concerns regarding rotator cuff failure and glenoid loosening in anatomic total shoulder arthroplasty (aTSA). Comparative data between these procedures in IA, however, remains limited. This study aims to compare functional outcomes and revision-free survival between aTSA and rTSA in IA using data from the New Zealand Joint Registry.
METHODS: All primary aTSA and rTSA procedures performed for IA between 2000 and 2022 were identified in the New Zealand Joint Registry. Demograph

PURPOSE: Proximal humerus fractures can be treated non-operatively with excellent results in the vast majority of cases. Surgical intervention is indicated for more complex fracture patterns, utilizing internal fixation or arthroplasty techniques. The aim of this study was to evaluate the long-term functional outcomes of patients who underwent shoulder hemiarthroplasty due to proximal humerus fracture.
METHODS: A retrospective chart review of patients who underwent shoulder hemiarthroplasty due to proximal humerus fracture in the University Hospital of Heraklion, utilizing the Aequalis fractu
