A 27-year-old man with a personality disorder, who did not have keratoconus previously, recently presented with a clinical picture resembling acute corneal hydrops OS three years after silicone oil injection for a complicated retinal detachment. Although self-induced trauma could not be excluded completely, this case might represent a newly described late corneal complication of intracameral silicone oil.

The risk associated with silicone oil removal after complex vitreoretinal surgery is unclear. Therefore, a cohort of 87 consecutive cases of silicone oil removal were analyzed. Eyes with attached retina before silicone oil removal with a follow-up of at least 5 months were included into the study. Profound hypotony associated with loss of vision complicating bilateral chronic uveitis was managed in five consecutive patients with vitrectomy and silicone oil injection. In seventeen patients presenting funnel chest a subcutaneous implantation of silicone prostheses was performed. In nine patients seromas were aspirated postoperatively. Five patients were reoperated, one for correction of a displacement of the prosthesis, one for size reduction and one for implant removal, because of various complaints not related to the prosthesis. At the final evaluation sixteen out of seventeen patients expressed satisfaction with the result.

Twenty-two patients with congenital and acquired defects, rehabilitated with silicone ectoprostheses, were followed up for 6 mo to 4 yr. The simplicity of the procedure, its good results and the early physical and social rehabilitation attained, favor its use when adequately indicated.

A contact lens made from silicone rubber has been fitted successfully for extended wear in children. The lens has been used primarily with aphakic children, but it has also been tolerated by children with anisometropia, high myopia, and keratoconus. The lens can be handled like a hard contact lens and can be fitted by a variety of methods. The eye shows excellent tolerance of the lens and complications or failures to wear the lens have been due primarily to discomfort. After about a year of wear the lens surface can no longer be cleaned.
