Facial reanimation after skull base trauma.

Three basic types of procedures are used by the author to reanimate the face paralyzed by interruption of the facial nerve at the skull base. In order of choice they are direct nerve repair or grafting; hypoglossal-facial nerve anastomosis; and regional facial reanimation (brow lift, implantation of a gold weight or spring in the eyelid, tightening of the lower eyelid, and temporalis muscle transposition to reanimate the mouth). The indications for and expected results of each approach are presented with special emphasis on the usefulness of regional reanimation.

Skull base surgery in composite resection.

We extended the limits of standard jaw and neck dissection in large tumors of the oropharynx with extension to the soft and hard palate. Resection of the primary tumor with adequate margins and the lymphatics of the infratemporal fossa is required for tumor control in these extensive lesions. Splitting the lip in the midline and developing a large cervicofacial flap gives an excellent exposure to the region. The resection includes the hemimandible and the soft tissues of the intratemporal fossa. The internal carotid artery is followed to the skull base and all structures medial to this vessel,…

Basilar skull fractures in three horses.

Of three horses with basilar skull fractures, two died within 48 hours. The remaining horse was euthanatized because of a locomotion deficit. Clinical signs included epistaxis followed by ataxia. In each case, the basi-occipital bone and ventral portion of the calvarium were involved in the fracture. Diagnosis was based on clinical signs or radiographic appearance of guttural pouches, or both.

Cryptococcal meningitis complicating remote skull fracture.

A patient in whom cryptococcal meningitis complicated a nine year old depressed frontal skull fracture, an association which has not been reported previously, is recorded. This is also the first case of cryptococcal meningitis recognised in Ethiopia.

Skull Base Metastases: A Pictorial Review.

Compared to other sites of skeletal metastases, skull base metastases are relatively rare with varying incidence reported in the literature. The most frequently implicated cancers are breast, prostate and lung carcinomas and lymphoma. While some lesions are asymptomatic, the clinical presentation may be with pain or cranial neuropathy. Adequate imaging techniques and knowledge of skull base anatomy are essential. Imaging features of metastases at various locations at the skull base will be described, highlighting important anatomical landmarks and clinical implications.

[Intraoperative Neuromonitoring for Skull Base Surgery].

Intraoperative neuromonitoring techniques can be used to evaluate neural functions during surgical procedures. For removal of tumors in the skull base region, we perform them for both structural mapping of the compressed cranial nerves and continuous monitoring of their preservation. The goal is to eliminate the critical irreversible damage to the neurological structures and prevent postoperative neurologic deficits. Recording techniques have been developed and neuromonitoring equipment used in the operation room with the intravenous anesthesia technique. For skull base surgery, in particular,…

Imaging of Developmental Skull Base Abnormalities.

The skull base is a critical structure in the craniofacial region, supporting the brain and vital facial structures in addition to serving as a passageway for important structures entering and exiting the cranial cavity. This paper will review and highlight some of the embryology, developmental anatomy, including ossification, and related abnormalities of the anterior, central and posterior skull base using illustrative cases and tables. Pathologies such as dermoids/epidermoids, cephaloceles, nasal gliomas, glioneuronal heterotopias, various notochordal remnants, persistent craniopharyngeal ca…

Current management of skull base osteomyelitis.

PURPOSE OF REVIEW: Skull base osteomyelitis (SBO) is a life-threatening condition. Due to an aging and increasingly multimorbid population, clinicians are more often challenged with this disease. Yet, there is no consensus on the optimal diagnostic and follow-up management. This review should aid clinicians in decision-making for their patients. RECENT FINDINGS: Treatment-resistant otalgia or headache is suspicious of SBO. Pseudomonas aeruginosa remains the most common pathogen but clinicians are challenged with increasing rates of sterile or fungal cultures due to previously applied antibiot…

Anterior Skull Base Glomangioma-Induced Osteomalacia.

Background  There have been studies trying to characterize Fibromuscular Dysplasia (FMD); however, most of them are based in mainly non-Hispanic sample. The objective of this study is to better understand the epidemiology and clinical characteristics of craniocervical FMD in the Hispanic population. Methods  We retrospectively reviewed the cerebral angiograms performed in our center in a period of 3.5 years under any indication looking for angiographic patterns of FMD. Exclusion criteria consisted of cases in which the first angiogram was done when the patients were younger than 18 years. Pati

Basilar skull fractures: the petrous bone.

UNLABELLED: Basilar skullfractures: the petrous bone. OBJECTIVES: to provide suggestions for the management of three of the most dangerous or important lesions (internal carotid artery lesions, cerebrospinal fluid leaks and facial nerve paralysis) associated with the petrous part of basilar skull fractures, thereby trying to assess categories of evidence and determine strengths of recommendation. METHODOLOGY: A PubMed-based literature review was carried out, as well as a consultation of online sources as encountered in the literature review. Also, a non-systematic search of chapters of well-…

[Recurring post-traumatic growing skull fracture].

INTRODUCTION: Growing skull fracture, also known as post-traumatic bone absorption or leptomeningeal cyst, is a rare complication of traumatic brain injuries and occurs almost exclusively in children under 3 years of age. CASE REPORT: We report the case of a 6-month-old child who presented, two months after an apparently unimportant traumatic skull injury, persistence of left temporoparietooccipital cephalohaematoma with no other signs. A transfontanellar ultrasonography scan revealed a bone defect with brain herniation, and computerised tomography and magnetic resonance imaging also confirme…

Skull base embryology: a multidisciplinary review.

INTRODUCTION: The skull base represents a central and complex bone structure of the skull and forms the floor of the cranial cavity on which the brain lies. Anatomical knowledge of this particular region is important for understanding several pathologic conditions as well as for planning surgical procedures. Embryology of the cranial base is of great interest due to its pronounced impact on the development of adjacent regions including the brain, neck, and craniofacial skeleton. MATERIALS AND METHODS: Information from human and comparative anatomy, anthropology, embryology, surgery, and compu…

Skull base osteomyelitis following a mastoidectomy.

Skull base osteomyelitis is a rare but life-threatening condition that is characterized by osteitis of the temporal bone and skull base. Although skull base osteomyelitis is recognized as a rare complication of malignant external otitis or middle ear infection, it may also occur following a mastoidectomy. We present a case of an 81-year-old woman who suffered severe otalgia with normal-looking EAC and tympanic membrane that developed six weeks following a canal wall up mastoidectomy. Otalgia was the only abnormal feature to suspect occurrence of skull base osteomyelitis.

Skull base meningiomas mimicking otitis media.

INTRODUCTION: Meningiomas are slow-growing, benign tumours originating from the arachnoid villi of the meninges. They account for 13 to 26 p … reported. AIM: To describe three patients in whom chronic otitis media was the presenting sign of skull base meningioma. METHOD: Case presentations.
