A new CBCT-based classification of posterior extramural ethmoid cells.

PURPOSE: The study aimed to evaluate the prevalence of extramural expansion of posterior ethmoid cells using Cone beam computed tomography ( … ial relationship with surrounding structures. Posterior extramural ethmoid cells may pneumatize the sphenoid bone to such an extent that they can be radiologically mistaken for the sphenoid sinus. Accurate differentiation between these structures is crucial, as inflammatory processes may develop independently within either the sphenoid sinus or the sphenoethmoi …

[The ethmoido-maxillary plate and paranasal sinuses--a study by HRCT].

The ethmoido-maxillary plate is a thin layer of bone separating the maxillary sinus from the ethmoidal cells or sphenoidal sinus. The plate was studied using axial HRCT images obtained from various otological lesions. One hundred forty-one adults were included in this study. The ethmoido-maxillary plate was first classified into four groups of configurations, namely, straight or near-straight, anterior-concave, posterior-concave and ant. and post. concave forms. The incidences of these forms were 55%, 23%, 13%, and 9%, respectively. A symmetrical configuration between the two sides was seen in

Skull fractures by glass bottles tested on cadaveric heads.

Head trauma is frequently related to the misuse of drinking vessels as weapons. Forensic reports usually evaluate these blunt injuries as having occurred in scenarios where the alcohol intake is high. Fatal consequences are seen in blows with glass bottles aiming at the head. To prove the outcome that a glass bottle thrown to the head could cause, three intact human cadaver heads were impacted with 1-liter glass bottles at 9.5 m/s using a drop-tower. The impact location covered the left temporal bone, sphenoid bone, and zygomatic arch. The contact between the head and the bottle was produced a…

[Extradural en bloc removal and in situ replacement of the anterior clinoid process].

Extradural removal of the anterior clinoid process (ACP) is useful and essential for approaching aneurysmal and tumor lesions in and around the cavernous sinus. A safe, rapid and less invasive technique is beneficial for this basic skull base surgery. We developed a new technique by sharply cutting the ACP together with the part of the sphenoid ridge bone followed by complete replacement. A series of patients with either basilar top or internal carotid artery aneurysms underwent the present technique. After frontotemporal craniotomy, the lateral frontal and anterior middle cranial fossae are e

Occult malformations of the skull base.

Occult malformations of the skull base are rare anomalies. They are often not detected until they give rise to complications such as meningitis or cerebrospinal fluid rhinorrhea. We used high-resolution computed tomography, fluorescein endoscopy, cisternography, magnetic resonance imaging, and duraplasty to diagnose occult malformations of the skull base in 4 patients. The four patients had had between 4 and 6 attacks of meningitis. Cerebrospinal fluid rhinorrhea was confirmed in 3 cases. The following occult malformations were found: (1) an encephalocele of the glabella/cribriform plate and a

[An atypical form of Recklinghausen's disease].

The authors report the case a young girl who presented ptosis of the left eye lid and in whom other ophthalmic findings (Lisch's nodules, larger wind sphenoid bone hypoplasia) led to the diagnosis of Von Recklinghausen's disease. They recall the importance of such manifestations for this diagnosis.

Endoscopic management of a traumatic meningo-encephalocele through a planum sphenoidale defect: case report.

A 37-year-old man involved in a motor vehicle accident was admitted to our hospital with disturbed consciousness. Computed tomography (CT) showed an acute, right-sided epidural hematoma and pneumocephalus. Emergency evacuation of the acute epidural hematoma was carried out, and his condition gradually recovered. However, cerebrospinal fluid (CSF) rhinorrhea continued despite conservative treatment. Three-dimensional reconstructed CT revealed a bone defect, which was approximately 20 mm in size, in the planum sphenoidale and tuberculum sellae, and magnetic resonance imaging demonstrated a traum

Anterior skull base surgery for benign and malignant disease.

Teamwork between the head and neck surgeon and the neurosurgeon utilizing the craniofacial resection technique greatly extends the frontiers of surgery for tumors of the anterior half of the skull base. Tumors for which this technique may be used include benign and malignant tumors arising in the frontal bone or frontal sinus, nasal vault, ethmoid, maxilla, sphenoid or orbit. Selected nasopharyngeal lesions such as angiofibromas with anterior or middle fossa extensions and sphenoid or clival chordomas may also be approached in this manner. This report evaluates the surgical techniques currentl

Endoscopic endonasal repair of a temporal lobe meningoencephalocele in the pterygoid fossa: A case report and literature review.

INTRODUCTION: Meningoceles are defined as herniation of the meninges through a skull base defect. If brain tissue is herniated as well, then it is called meningoencephalocele. The most common locations of meningoceles are the cribriform plate and the lateral recess of the sphenoid sinus, with the most common presentation being CSF leakage. We present a case of temporal lobe meningocele that herniated through a defect in the greater wing of the sphenoid bone to the pterygoid fossa and that was diagnosed incidentally while evaluating the patient for seizures. METHODOLOGY: Case report and litera…

Angiofibroma Localized in the Sphenoid Sinus.

Juvenile nasopharyngeal angiofibroma is the most common benign tumor of the nasopharynx with complaints of unilateral nasal obstruction and recurrent nosebleeds in the young male population. Despite being a benign tumor, it can be aggressively destructive in surrounding tissues and bones by acting locally. The gold standard treatment method is the surgical excision of the tumor. This case report is a case of angiofibroma, a 32-year-old asymptomatic male patient with no evidence of clinical signs and endoscopic examination, which is recognized as a localized vascular mass lesion in the right sp

[Differential diagnosis of superior orbital fissure syndrome].

Paresis of cranial nerves III, IV, VI and the ophthalmic nerve is termed fissura-orbitalis-superior syndrome. It is caused by affections of the surrounding bones, vascular lesions in the region of the cavernous sinus, tumours of sella and sphenoid bones and diseases of the orbital cavity. Cranial computed tomography (with and without contrast enhancement) is a reliable diagnostic method which may be supported by angiography of NMR in individual cases.

Polydactyly in neurofibromatosis type I: a potential clue to diagnosis.

Neurofibromatosis type 1 is a genetic disorder characterized by variable phenotypic manifestations. The diagnostic criteria, 25 established in 1987, are broad to encompass these pleiotropic findings. Included are the specific osseous manifestations of 26 sphenoid dysplasia and dysplasia or thinning of the cortex of long bones. This review highlights recent evidence on the role of 27 neurofibromin in bone development and suggests consideration for additional diagnostic criteria.

Endoscopic transphenoidal approach for fibrous dysplasia of clivus, tuberculum sellae and sphenoid sinus; report of three cases.

AIM: Fibrous dysplasia is a benign disease in which medullary bone is replaced by fibro-osseous tissue, and causes distortion and overgrowth of the involved bone and represents about 3% of all bone tumors. There is variability in the manifestation of Fibrous Dysplasia lesions with imaging techniques due to their proportional variations of fibrous to osseous tissue. Radiological differential diagnoses include meningioma, chordoma on MRI and Paget disease on CT imagings. MATERIAL AND METHODS: We report three cases of monostotic fibrous dysplasia, affecting clivus, tuberculum sellae, and sphenoi
