Relationship of the optic nerve to the posterior paranasal sinuses: a CT anatomic study.

PURPOSE: To delineate the relationship between the optic nerves and the posterior paranasal sinuses using CT data. METHODS: Direct coronal sinus CT scans of 150 consecutive patients with chronic inflammatory sinus disease were reviewed by two radiologists. Axial oblique reconstructions along the course of the optic nerve were obtained for the first 100 patients. The direct relationship between the optic nerve and the posterior ethmoid and sphenoidal sinuses was recorded, as were identations into the sinus wall, course of the nerve through the sinus region, pneumatization of the anterior clino

Benign pleomorphic adenoma of the soft palate metastasizing to the sphenoid sinus.

A benign pleomorphic adenoma is the most common tumor of the salivary glands. This tumor has the potential to metastasize to bone, the head and neck region, visceral organs and skin. Although there a few reports about a benign pleomorphic adenoma metastasizing to the maxillary sinus in the paranasal sinuses, there are no reports about a metastatic benign pleomorphic adenoma in the sphenoid sinus. We report here on a case of a benign pleomorphic adenoma of the soft palate that metastasized to the sphenoid sinus, and we briefly review the relevant clinical literature.

Catastrophic Allergic Fungal Sinusitis: A Report of Two Cases.

BACKGROUND: Allergic fungal rhinosinusitis (AFRS) is a common condition in which sinusitis develops as an IgE-mediated response to common sinonasal fungal organisms. If that response leads to blockage of sinus ostia, bone expansion and erosion by expansive cysts containing dense inspissated debris may occur with the potential for critical neurovascular compression including damage to the anterior visual pathway. METHODS: Review of clinical and imaging features of 2 patients who sustained catastrophic clinical outcomes. RESULTS: The first patient had pansinusitis with massive mucocele-like cy

Investigation of sinonasal microbiome spatial organization in chronic rhinosinusitis.

BACKGROUND: Chronic rhinosinusitis (CRS) is a multifactorial inflammatory airway disorder in which bacteria are implicated in the initiation and/or sustenance of disease in some patients. The sinuses are colonized by bacteria even in health, and the potential for sinus-specific niches harboring unique microbial consortia raises questions for clinical and research investigation. The objective was to determine the degree to which resident upper airways microbiota differ between individuals and anatomic sites, in order to determine the optimal site of microbial sampling for study in CRS. METHODS

Microbiological and Radiological Characteristics of Paranasal Sinus Fungus Ball: a retrospective analysis of 235 cases.

Paranasal sinus Fungus Ball (FB) is the most common non-invasive mycotic rhinosinusitis. It most frequently affects the maxillary and sphenoidal sinuses and the treatment of choice is Endoscopic Sinus Surgery (ESS). Although this pathology has been widely investigated throughout the years, some questions still remain unanswered. This study concentrates on assessing radiological and microbiological characteristics by examining a large number of cases treated in our centre. 235 cases of FB that underwent ESS in Fondazione I.R.C.C.S. Policlinico San Matteo di Pavia in the period comprised between

[Incidence of non-tumor anomalies of the facial sinuses on x-ray computed tomography of the brain of asymptomatic patients. Clinical and radiologic correlations between involvements of ethmoid cells and other sinus cavities].

Two hundred ninety five consecutive brain CT examinations of asymptomatic adults were studied prospectively for detection of nontumoral paranasal sinus abnormalities. CT alterations were found in 104 cases (35.3%), localized to the ethmoid cells in 83 cases, the maxillary sinuses in 56 cases, the sphenoid sinuses in 19 cases, and the frontal sinuses in 18 cases. The relationships between ethmoid cells disease and alterations of the large paranasal cavities are analysed, and the association with previous sinusitis or rhinitis is evaluated. Frontal sinus disease is associated in more than two-th

Multiple unilateral traumatic carotid-cavernous sphenoid sinus fistulas with associated massive epistaxis: a consequence of parkour.

BACKGROUND: Traumatic carotid-cavernous fistulas (CCFs) present the clinician with diagnostic and surgical challenges. Extension of a CCF into the sphenoid sinus presents additional management difficulties. Endovascular interventions using various thrombogenic materials such as balloons, coils, or liquids are effective treatment strategies. Ideally, these techniques are used to obliterate the fistula while maintaining the patency of the parent artery. CASE REPORT: We present a rare case of traumatic carotid-cavernous sphenoid sinus fistulas complicated by multiple tears in the internal caroti

Cutaneous malignant melanoma metastatic to the sphenoid sinus.

While metastases to the paranasal sinuses are unusual, metastases to the sphenoid sinus are exceedingly rare, especially metastases of a cutaneous melanoma to the mucosa. We report the case of a 38-year-old man who presented with a solitary sphenoid sinus metastasis that had spread from a cutaneous malignant melanoma. The metastasis was removed via a wide, endoscopically assisted sphenoidotomy. The patient was further treated with external radiotherapy, and at 8 months of follow-up he was free of disease. However, he experienced a recurrence at 3 years that proved to be fatal. We review the pa

Extraoral anatomy in CBCT – a literature review. Part 3: Retromaxillary region.

In this review about extraoral anatomy as depicted by cone beam computed tomography, the retromaxillary region is discussed. A medium-sized (6 x 6 cm) or large (≥ 8 x 8 cm) field of view of the maxilla will inevitably depict the retromaxillary region that can be considered a «transition» zone between the viscerocranium and the neurocranium. Major structures of the region include the sphenoid bone and the pterygopalatine fossae. The sphenoid bone is a single but complex bone located between the maxilla and the brain. It is composed of a central body, bilateral greater and lesser wings, and pter

Identification of Onodi cell and new classification of sphenoid sinus for endoscopic sinus surgery.

BACKGROUND: There is no effective classification method for the opening of the sphenoid sinus. The objective of this study was to examine the effectiveness of identification of the Onodi cell and classification of the sphenoid sinus using sagittal computed tomography (CT) for sphenoidotomy. METHODS: CT images of the sinuses of surgical patients (n = 261; 522 sides) were studied. Using sagittal CT, the relationships between the lateral side of the anterior wall of the sphenoid sinus and the optic nerve, and between the middle of the anterior wall of the sphenoid sinus and the skull base or pit

Sphenoid esthesioneuroblastoma arising from the hindmost olfactory filament.

Esthesioneuroblastoma (ENB), or olfactory neuroblastoma, is a rare malignant neoplasm arising from the olfactory neuroepithelium. Typically, ENBs are found in the olfactory cleft with extension to the ethmoid sinuses or anterior skull base. Here we report a case of ENB located in the sphenoid sinus, which had been considered as an ectopic ENB. However, endoscopic resection revealed the continuity of the tumor with the hindmost olfactory filament. The present case suggests that an ENB in the sphenoid sinus was not ectopic, but arose from the normal olfactory neuroepithelium. This continuity of

The Unusual Presentation of a Myxoma Within the Sphenoid Sinus: Case Report and Review of the Literature.

BACKGROUND: We describe a rare case of a sphenoid sinus myxoma that was resected via an endoscopic endonasal skull base approach. We review the literature regarding these rare tumors of the paranasal sinuses. CASE DESCRIPTION: A 72-year-old woman was diagnosed with an incidental sphenoid sinus tumor and left sphenoid wing meningioma during a workup for left-sided proptosis and diplopia. Biopsies of the sphenoid wing and sphenoid sinus tumors were obtained. After undergoing surgical resection of the meningioma, the patient then underwent definitive resection of the sphenoid sinus myxoma via en

Pituitary apoplexy presenting with epistaxis.

A 42-year-old woman developed headache and epistaxis followed by fever, stiff neck, and loss of vision of the right eye. The diagnosis of simple epistaxis was changed to mucormycosis, then to bacterial meningitis and then to sphenoid sinusitis, before the correct diagnosis of pituitary apoplexy was established by CT scan. Epistaxis is yet another confusing symptom of pituitary apoplexy.

The completely opacified frontal or sphenoid sinus: a marker of more severe disease in chronic rhinosinusitis?

OBJECTIVE: Determine whether complete opacification of the sphenoid or frontal sinus is associated with increased clinical severity of chronic rhinosinusitis (CRS). METHODS: Adult patients undergoing evaluation for CRS prospectively completed the rhinosinusitis symptom inventory (RSI) and underwent computed tomography of the paranasal sinuses. A cohort with at least one completely opacified frontal sinus was identified. To each patient in this opacified cohort, a control patient without complete frontal sinus opacification was matched with corresponding Lund score. Symptom scores for headache

Pneumosinus dilatans: a sign of intracranial meningioma.

BACKGROUND: Pneumosinus dilatans affecting the sphenoid and posterior ethmoid sinuses has been reported in association with spheno-orbital meningiomas and is believed by some authors be a sign of an adjacent meningioma. METHODS: We report the case of a 57-year-old man who developed progressive neurologic signs and symptoms consistent with a frontal lobe lesion. Neuroimaging studies revealed a large partially cystic mass at the base of the anterior cranial fossa that appeared to be invading the left frontal lobe and that was associated with pneumosinus dilatans of the adjacent left frontal sin

Retrobulbar optic neuropathy associated with sphenoid sinus mucormycosis.

Because fungi usually spread from the paranasal sinuses to the orbital apex in invasive fungal sinusitis (IFS), IFS often presents as an orbital apex syndrome (OAS) characterized by dysfunction of cranial nerves II, III, IV, V1, and VI. We report a case of sphenoid sinus mucormycosis that presented as isolated retrobulbar optic neuropathy. A 94-year-old woman presented with acute blindness in the right eye. Examination revealed the absence of light perception and pupillary reflex in the right eye. Head MRI showed a mass in the right sphenoid sinus, which was contiguous with the right optic ner

In-office functional endoscopic sinus surgery for chronic rhinosinusitis utilizing balloon catheter dilation technology.

PURPOSE OF REVIEW: Balloon catheter dilation (BCD) technology was introduced in 2005 as a device employed to dilate maxillary, sphenoid, and frontal sinus outflow tracts in patients with chronic sinus disease. With the evolution of the technology, BCD has been utilized in the office setting. The purpose of this review is to discuss the safety, tolerability, and technical success of this change of venue and to consider the possible significant cost savings in the current healthcare environment. RECENT FINDINGS: Recent studies have shown that BCD technology can be safely and successfully used i
