Spinal cord injuries at birth: a multicenter review of nine cases.

OBJECTIVE: To report a multicenter study of nine cases of children presenting with a birth-related spinal injury. METHODS: The medical charts of nine patients identified by a questionnaire sent to the members of the French Society of Pediatric Orthopedics (SOFOP) were reviewed. RESULTS: The pregnancy was uneventful in all cases. The fetal presentation was cephalic in three cases, a breech presentation in four cases, and a face presentation in two cases. The lesion involved the cervical spine in six cases. Three patients presenting upper cervical injuries died before the age of six years. The

Recognition and effective management of autonomic dysreflexia in spinal cord injuries.

Autonomic dysreflexia is a potentially life-threatening hypertensive medical emergency that occurs most often in spinal cord-injured individuals with spinal lesions at or above the mid-thoracic spinal cord level. It is a condition that remains poorly recognised outside of spinal cord injury centres, which may result in adverse outcomes including mortality from potentially delayed diagnosis and treatment. Acute autonomic dysreflexia is characterised by severe paroxysmal hypertension associated with throbbing headaches, profuse sweating, nasal stuffiness, flushing of the skin above the level of

Olfactory ensheathing cells and repair of brain and spinal cord injuries.

Cells generated in cultures of primary tissue from the olfactory nerves can be transplanted into areas of spinal cord damage where they induce growth of severed nerve fibers and return of lost functions. These cells are of two types, both essential for the reparative effect. The precise lineage relationships are not fully understood. The limitation of numbers of cells obtainable from human samples currently restricts their use to repair of small lesions.

[Progress in understanding spinal cord injuries. Clinical aspects, imaging, pathological correlations].

Close inspection of MR images in all stages of SCI can reveal alterations which are important for our understanding of the changes which occur in SCI and may be crucial for planning surgical intervention. Importantly also, these observations may assist in the evaluation of novel therapies in SCI, such as cellular transplantation. It is hopeful that MR strategies which are currently in routine use in the brain, such as diffusion weighted imaging, perfusion studies, spectroscopy, and magnetization transfer can be adopted for use in the spine [8]. Because of the small size of the cord, the magnet

Bowel management outcomes in individuals with long-term spinal cord injuries.

Outcomes of bowel management were evaluated in 1993 among 221 British long-term spinal cord injury survivors who were followed-up as part of a longitudinal study of aging with spinal cord injury. Representing a population-based sample followed up by two regional spinal cord injury treatment centres - Stoke Mandeville Hospital in Aylesbury and the District General Hospital in Southport, England - all had been injured at least 20 years when the study was begun in 1990; all underwent comprehensive assessments and extensive interviewing at their centres in 1990 and 1993. When assessed in 1993, 42%

Recurrence of initial pressure ulcer in persons with spinal cord injuries.

Whether treated with surgery or by conservative, nonsurgical measures, pressure ulcers recur in 5% to 91% of spinal cord injured (SCI) patients. Factors other than the surgical technique used or the standard conservative management provided may be responsible. A retrospective study of 176 SCI patients with a history of one or more pressure ulcers was conducted at the Department of Veterans Affairs Medical Center at Castle Point, N.Y. Approximately 35% of patients who received either surgical or nonsurgical treatment had a recurrence. Patients who smoked and patients with diabetes or cardiovasc

Spinal cord injuries in Iceland 1973-1989. A follow up study.

All traumatic SCI patients in Iceland come to the Neurosurgical Department of Borgarspítalinn, the City Hospital in Reykjavík for their initial treatment. From 1973-1989, 79 (93%) were rehabilitated at Borgarspítalinn. The purpose of this study was to look at the most common complications and the social functions of SCI patients after discharge from the Rehabilitation Department. A questionnaire was sent to 59 of the 79 patients. They represent almost the whole population of the spinal cord injured population in Iceland. Forty-five patients (76.3%) answered the questionnaire, 69% male, 31% fem

[Trauma of the spine and spinal cord combined with other injuries].

From analysis of 304 clinical cases with trauma of the spine and spinal cord combined with various extravertebral injuries the authors discuss the specific features of the clinical course and diagnosis of the condition and the surgical tactics in the management of this severe contingent of patients. They substantiate the use of emergency instrumental methods for recognizing injuries to the viscera and brain as well as the tendency to conduct early operations for fractures of the spine and limbs whenever indicated.

Postoperative management and acute rehabilitation of patients with spinal cord injuries.

The mortality and morbidity rates of patients with spinal cord injury have decreased greatly. Prior to World War II, few patients survived their acute injuries. Now even severely involved respiratory-dependent patients may return to live in the community. Advanced surgical procedures, when appropriately applied, are decreasing the time of immobility and preventing late spinal deformities. Exciting new developments in basic science research such as the use of thyrotropin-releasing hormone and functional electrical stimulation may alter the whole course of recovery for patients within the forese

Temporal course of motor recovery after Brown-Sequard spinal cord injuries.

Recovery of voluntary motor function after incomplete spinal cord injuries is attributed to a variety of physiological mechanisms, such as resolution of conduction block in injured axons, and neuroplasticity mechanisms in spared axons. To better understand these recovery mechanisms, we have examined motor recovery in one type of incomplete cord injury, the Brown-Sequard Syndrome. This syndrome is observed in patients with unilateral injury of the spinal cord and is manifested as asymmetric weakness and pain/temperature sensory loss contralateral to the weakest extremity. We have followed the c…

Treatment of femoral shaft fractures associated with acute spinal cord injuries.

Twenty-seven femoral shaft fractures in 23 patients with acute spinal cord injuries were reviewed for evaluation of the outcome of operative versus nonoperative treatment. Three groups were identified: 11 nonoperative, eight early operative, and eight delayed operative. Patients treated initially by nonoperative methods developed five impending nonunions (31%), which subsequently were treated by open reduction and internal fixation. One femur in each of the operative groups developed a refracture after early removal of metal fixation devices. In the delayed operative group, four patients (50%)…

Contributing Factors to Endothelial Dysfunction in Individuals with Spinal Cord Injuries.

BACKGROUND: Patients with spinal cord injuries (SCIs) are at a greater risk for the development of cardiovascular diseases (CVDs) than able-bodied individuals due to the high risk of endothelial dysfunction. SUMMARY: For instance, patients with SCIs lose autonomic control of the heart and vasculature, which results in severe fluctuations in blood pressure. These oscillations between hypotension and hypertension have been shown to damage blood vessel endothelial cells and may contribute to the development of atherosclerosis. Furthermore, the loss of skeletal muscle control results in skeletal …

[Movement analysis in spinal cord injuries : Assistance in clinical decision making].

For motor incomplete spinal cord injured patients, improvement of walking function is an important aim in the rehabilitation program. In specialized treatment centers, the 6‑minute walking test, the 10-meter walking test or the timed-up-and-go test are used as an assessment tool to determine walking ability, but these tests are not able to assess the quality of gait. Marker-based movement analysis can be used as a reliable method to evaluate the gait pattern. This allows an objective assessment of gait quality over time or can be used to support therapy planning. The benefit of such an analysi
