[Non-tropical sprue and chronic inflammatory rectal stenosis in a patient with abuse of ergotamine-containing suppositories].

A 51 year old Yugoslavian patient was admitted to our hospital in reduced general condition with distinct hypocalcemia, osteomalacia, and with rectum stenosis. Our investigations led to the diagnosis of a malabsorption syndrome due to non-tropical sprue. The most likely cause of the rectum stenosis is an abuse of ergotamine-containing suppositories for several years. A gluten-free diet and the interruption of the use of the suppositories improved her general condition remarkably.

Faecal free fatty acids in tropical sprue and their possible role in the production of diarrhoea by inhibition of ATPases.

Faecal excretion of fatty acids is increased in patients with tropical sprue because of unabsorbed dietary fatty acids. The excretion of fatty acids correlates well with faecal wet weight. In vitro unsaturated fatty acids inhibited Na K-ATPase and Mg-ATPase isolated from basolateral membranes of enterocytes and colonocytes. These findings are a possible explanation for the observed abnormalities in water and electrolyte absorption by the colon in patients with tropical sprue and steatorrhoea.

Pathogenesis of tropical sprue: a pilot study of antroduodenal manometry, duodenocaecal transit time & fat-induced ileal brake.

BACKGROUND & OBJECTIVES: Small intestinal bacterial overgrowth (SIBO) due to ileal brake-induced hypomotility may cause tropical sprue (TS). We evaluated effect of infusion of fat or placebo in duodenum randomly in patients with TS and healthy controls on antroduodenal manometry (ADM) and mediators of ileal brake, and duodenocaecal transit time (DCTT). METHODS: ADM and DCTT (lactulose hydrogen breath test, HBT) were evaluated with placebo and fat in eight controls and 13 patients with TS (diagnostic criteria: tests showing malabsorption of two unrelated substances, abnormal duodenal histology

Tropical sprue: two cases report and review of literature. Has anything changed in the past 40 years?

BACKGROUND: Tropical sprue is an acquired condition seldom reported in travelers from tropical countries including Thailand. Only one case report from Thailand was reported in 1969. CASE REPORT: The authors report two cases of American residents living in Thailand and Vietnam who had been suffering from chronic diarrhea, weight loss, being easily fatigued and nutritional deficiency and who dramatically responded after treatment for tropical sprue. However a relapsed episode was seen in one case and somewhat permanent damage was detected in another case. CONCLUSION: Tropical sprue is currentl
