Sputum inflammation predicts exacerbations after cessation of inhaled corticosteroids in COPD.

INTRODUCTION: The GOLD guidelines advocate not to institute inhaled corticosteroids (ICS) in patients with mild-to-moderate COPD. However, m … smography, and health status assessment were performed in 68 COPD patients using ICS. Additionally, sputum cell differentials, supernatant leukotriene B(4), eosinophilic cationic protein, and myeloperoxidase, serum C-reactive protein and soluble intracellular adhesion molecule, and urinary desmosine were assessed. Sputum was also analysed for mRNA levels …

Sputum colour: a useful clinical tool in non-cystic fibrosis bronchiectasis.

This study explored the utility of sputum colour in clinically stable patients with bronchiectasis. Interpretation of sputum colour between the doctor and the patient was reliable (intraclass correlation coefficient 0.83 (95% confidence interval 0.76-0.89). Sputum colour predicted bacterial colonisation (5% in mucoid sputum; 43.5% in mucopurulent sputum; 86.4% in purulent sputum; p<0.0001). On multivariate logistic regression analysis, independent factors associated with purulent sputum were bacterial colonisation, varicose or cystic bronchiectasis, forced expiratory volume in 1 s <80% predict…

Assessing airway inflammation in clinical practice - experience with spontaneous sputum analysis.

BACKGROUND: The assessment of airway inflammation for the diagnosis of asthma or COPD is still uncommon in pneumology-specialized general practices. In this respect, the measurement of exhaled nitric oxide (NO), as a fast and simple methodology, is increasingly used. The indirect assessment of airway inflammation, however, does have its limits and therefore there will always be a need for methods enabling a direct evaluation of airway inflammatory cell composition. Sampling of spontaneous sputum is a well-known, simple, economic and non-invasive method to derive a qualitative cytology of airwa…

Influenza vaccination: changes in exhaled nitric oxide levels and sputum cytology.

OBJECTIVE: Influenza vaccination is routinely recommended for patients with chronic lung disease, but has been reported to cause a small increase in airway reactivity. The use of simple, non-invasive methods of assessing changes in airway inflammation could potentially allow improved understanding of the cellular mechanisms underlying such changes. METHODOLOGY: We studied a group of 44 hospital workers before and after routine influenza vaccination, using spirometry, and exhaled nitric oxide (eNO) as a marker of pulmonary inflammation (Group A). In addition, methacholine challenge and sputum …

Benefits of three consecutive sputum inductions using nebulized racemic salbutamol versus hypertonic saline in smear-negative or sputum-scarce pulmonary tuberculosis patients: a prospective randomized, double-blind study.

UNLABELLED: Although, sputum induction (SI) with hypertonic saline increases the diagnostic yield for suspected pulmonary tuberculosis (PTB) in patients with smear-negative microscopy, but, it carries adverse effects (AEs), particularly bronchospasm. This study assessed and compared the diagnostic yields and safety profiles of three consecutive SI procedures using nebulized salbutamol vs conventional 3% sodium chloride (NaCl) in patients with suspected PTB and initially negative sputum smears. A randomized, double-blind study was conducted at Songklanagarind Hospital, Thailand, between 2016 an…

Home sputum collection for Aspergillus fumigatus detection in adults with cystic fibrosis.

BACKGROUND: Elexacaftor/tezacaftor/ivacaftor (ETI) has impacted the ability for people with cystic fibrosis (PwCF) to spontaneously expectorate sputum, leading to lower respiratory sampling rates and infection detection challenges. Home sampling may permit a potential strategy for fungal detection in PwCF. METHODS: We conducted a prospective decentralised cohort study of PwCF to test the feasibility of home sputum collection and ambient temperature transport for Aspergillus fumigatus (Af) detection. Participants collected and shipped weekly sputum samples from home to the laboratory for funga…

Targeted sputum sequencing for rapid and broad drug resistance of Mycobacterium tuberculosis.

PURPOSE: Rapid detection of drug resistance in Mycobacterium tuberculosis (Mtb) from clinical samples facilitates the timely provision of optimal treatment regimens for tuberculosis (TB) patients. METHODS: In November, 2023, the WHO released its second catalogue of resistance-conferring mutations in Mtb. Utilizing this information, we developed a single 17-plex PCR assay covering 16 key resistance genes and modified thermo-protection buffer to amplify 30 kbp DNA directly from sputum samples for nanopore sequencing. We implemented our protocol using rapid barcoding for sequencing with both a F…

Diagnostic yield of induced sputum and Bronchoalveolar lavage in suspected pulmonary tuberculosis.

Aim of this study was to compare the diagnostic yield of induced sputum (IS) and bronchoalveolar lavage (BAL) in patients with suspected pulmonary tuberculosis (PTB) and negative sputum smears. We enrolled 215 patients who underwent both IS and BAL after two negative spontaneous sputum samples. PTB was confirmed by culture or molecular test in 26 patients (12.1%). IS detected 10 cases (38.5%) of all PTB, while BAL detected 22 cases (84.6%) of all PTB. IS had a sensitivity of 38.46% and a specificity of 100%, while BAL had a sensitivity of 84.62% and a specificity of 100%. BAL had a higher diag…

Viral Cytopathic Changes in Sputum Cytology in a Patient with Known Human Metapneumovirus.

INTRODUCTION: Viral cytopathic changes seen in sputum cytology have been described in association with infection by viruses such as cytomegalovirus (CMV), herpes simplex virus (HSV), adenovirus, and even measles. However, viral cytopathic changes due to human metapneumovirus (hMPV) have not yet been well described in cytology. hMPV is a relatively new entity, discovered in 2001. It is known to cause upper and lower respiratory tract infections in children, the elderly, and immunocompromised patients. CASE PRESENTATION: We describe the viral cytopathic changes seen in sputum in a 63-year-old m…

Effects of postage on recovery of pathogens from cystic fibrosis sputum samples.

BACKGROUND: Regular surveillance microbiology of sputum is used in cystic fibrosis (CF) to monitor for new pathogens and target treatments. A move to remote clinics has meant greater reliance on samples collected at home and posted back. The impact of delays and sample disruption caused by posting has not been systematically assessed but could have significant implications for CF microbiology. METHODS: Sputum samples collected from adult CF patients were mixed, split, and either processed immediately or posted back to laboratory. Processing involved a further split into aliquots for culture-d…

Blood and Sputum Eosinophils of COPD Patients Are Differently Polarized than in Asthma.

Different eosinophil subpopulations have been identified in asthma and other eosinophilic disorders. However, there is a paucity of data on eosinophil subpopulations in patients with chronic obstructive pulmonary disease (COPD). The aim of this study was to compare eosinophil phenotypes in blood and induced sputum in patients with COPD, asthma and controls. Stable patients with mild-to-moderate COPD (n = 15) and asthma (n = 14) with documented blood eosinophilia ≥100 cells/µL in the year prior to the study and the control group (n = 11) were included to the study. The blood and sputum eosinoph…

Are sputum eosinophils associated with a different phenotype in COPD patients? A retrospective study.

Sputum eosinophilia in Chronic Obstructive Pulmonary Disease (COPD) patients seems to be associated with a better response to inhaled corticosteroids (ICS). To verify if this feature could identify a specific subpopulation of COPD patients, we retrospectively compared functional and inflammatory parameters of 110 COPD patients according to the presence of sputum eosinophilia (>2%). Patient with eosinophilia were characterized by lower dyspnea score, lower functional impairment and lower ICS use, suggesting that airway eosinophilia may be associated to a lower COPD severity and some functional …

The profile of respiratory pathogens in induced sputum of elderly and non-elderly asthmatics.

INTRODUCTION: Respiratory pathogens are thought to be involved in the pathogenesis and exacerbations of asthma at all ages; however, little is known about the airway microbiome in the elderly. AIM OF THE STUDY: To identify respiratory pathogens in the induced sputum (IS) of elderly asthmatics, and to determine the association between pathogens and the markers of asthma activity. MATERIAL AND METHODS: Twenty-nine subjects with stable asthma, 15 above 65 years of age and 14 aged 30-49 years, underwent clinical evaluation, fractional exhaled nitric oxide measurement, and sputum induction. Patho…
