Human monocytes promote Th1 and Th17 responses to Streptococcus pneumoniae.

Streptococcus pneumoniae is a leading cause of bacterial pneumonia, meningitis, and sepsis in children. Human immunity to pneumococcal infections has been assumed to depend on anticapsular antibodies. However, recent findings from murine models suggest that alternative mechanisms, dependent on T helper cells, are also involved. Although the immunological events in which T helper cells contribute to acquired immunity have been studied in mice, little is known about how these responses are generated in humans. Therefore, we examined bacterial and host factors involved in the induction of Th1 and

A rare case of sepsis in newborn: Streptococcus pneumoniae septicemia.

BACKGROUND: Streptococcus pneumoniae is a rare cause of sepsis in the newborn. CASE: The term baby was admitted on complaint of dyspnea, and antibiotherapy was begun after samples for hemocultures were obtained with the suspicion of sepsis according to the clinical and laboratory data. S. pneumoniae was demonstrated in the vaginal culture of the mother of the patient whose lumbar punction and chest roentgenogram were normal but hemoculture revealed the propagation of S. pneumoniae. The patient, treated with antibiotherapy for 14 days, was discharged without any complications. CONCLUSION: In

[A case of association of atypical localisations of Streptococcus pneumoniae].

Streptococcus pneumoniae can be responsible for spinal, joints, pulmonary or infrequently cutaneous septic localisations. The association between different septic localisation is extremely rare with this microorganism. We describe the first case of concomitant spinal, joints and cutaneous septic localisations in a critically ill patient with S. pneumoniae septicaemia. This observation illustrates that heterogeneity of clinical features depends on the pathogen characteristic and its interaction with the host.

Purpura fulminans due to Streptococcus pneumoniae sepsis following gastric bypass.

An older female underwent bariatric surgery which was followed by a significant weight loss and diarrhea, from which C. difficile was isolated just before her hospitalization. Less than 48 hours after admission, she became febrile, developed deep venous thrombosis of the leg and a pulmonary embolus. Blood cultures grew out Streptococcus pneumoniae and the patient developed purpura fulminans. There was convincing laboratory evidence for disseminated intravascular coagulation and a marked depletion of proteins C and S as well as antithrombin. Treatment with ceftriaxone and drotrecogin alfa toget

Clinical isolates of Streptococcus pneumoniae that exhibit tolerance of vancomycin.

The ability of Streptococcus pneumoniae to escape lysis and killing by vancomycin, a property termed "tolerance," has recently been noted in a laboratory strain of the species. Vancomycin tolerance in clinical isolates represents a potential new health risk. We determined the prevalence of vancomycin and penicillin tolerance among 116 clinical isolates of pneumococci by monitoring lysis and viability after exposure to the respective antibiotic for 4 hours. Eight percent of the strains were tolerant to penicillin and 3% were tolerant to vancomycin. The 3 vancomycin-tolerant isolates also had a

Epidemiology of invasive Streptococcus pneumoniae infections in adults in Finland.

Laboratory-based surveillance of invasive pneumococcal infections in adults in Finland from 1983 to 1992 identified 862 episodes of pneumococcal bacteraemia and 97 episodes of meningitis. The overall incidence of invasive pneumococcal infections was 9.1 per 100,000 for all adults per year, but 27.1, 35.8, and 44.5 per 100,000 in those aged 65 years or over, 75 years or over, and 85 years or over, respectively. Most (99.7%) of the pneumococcal strains were sensitive to penicillin. Ninety-five percent of the strains belonged to serogroups/types present in the 23-valent pneumococcal polysaccharid

Recurrent Streptococcus pneumoniae sepsis in children with sickle cell disease.

Streptococcus pneumoniae sepsis is the most common invasive infection among patients with sickle cell disease. The risk of a recurrent episode of sepsis and subsequent death in those patients who have had a previous septic event is much higher. Patients with sickle disease who have had pneumococcal sepsis should continue penicillin prophylaxis indefinitely and should not be candidates for out-patient management of febrile episodes.

Streptococcus pneumoniae in human immunodeficiency virus type 1-infected children.

The purpose of this study was to characterize systemic Streptococcus pneumoniae disease in human immunodeficiency virus type 1 (HIV-1)-infected children. All cases of bacteremia and meningitis caused by S. pneumoniae among children less than 18 years old were collected by review of the Microbiology Laboratory records at the Bellevue Hospital Center during the period August 1, 1978, through July 31, 1993. There were 31 bouts of systemic S. pneumoniae disease in 19 of 235 HIV-1-infected children cared for by the Pediatric Infectious Disease staff and 116 bouts in 113 children not known to be HIV

[Three interesting pediatric cases with penicillin-resistant Streptococcus pneumoniae infection].

Since penicillin-resistant Streptococcus pneumoniae first recognized in 1967, the rate of penicillin-resistant strains has been increasing worldwide. There have been up to 50% from pediatric specimens in Japan. We reported three pediatric cases with penicillin G resistant Streptococcus pneumoniae infection to show some important clue from these cases for clinical practice against resistant pneumococcal infection. The first case was a typical acute mastoiditis, although we have experienced only masked mastoiditis recently. The second case was meningitis with septicemia, which did not show any a

Immunologic responses of children to serious infections with Streptococcus pneumoniae.

Antibody responses (as measured by radioimmunoassay), alterations in serum levels of complement, and the presence of circulating immune complexes (as measured by the fluid-phase C1Q-binding assay, the fluid-phase conglutinin assay, and the activation of C1 were evaluated in 15 children after meningitis and/or bacteremia caused by Streptococcus pneumoniae. The ages of the children ranged from two months to 16 years; the duration of follow-up ranged from 18 to 189 days (mean, 78 days). Increases in levels of homotypic antibody were found in only three (25%) of the 12 children in whom this respon

Host defense against Streptococcus pneumoniae: the role of the spleen.

The asplenic patient has an increased susceptibility to bacterial septicemia. Streptococcus pneumoniae is responsible for approximately 50% of the episodes. Although the incidence of septicemia varies among patients of different ages and with different underlying diseases, half of the patients with overwhelming post-splenectomy infection die. Increased susceptibility to overwhelming infection with blood-borne bacteria is due primarily to the absence of a major phagocytic mass with a large blood flow. In the nonimmune host the spleen plays an important role in the phagocytosis and clearance of

Alterations in Nutrient Availability in the Lungs During Streptococcus pneumoniae-Induced Pneumonia.

Streptococcus pneumoniae is a leading cause of pneumonia. Importantly, the extent and impact of changes in the infected airway on bacterial nutrient availability and gene expression are not known. Utilizing untargeted UPLC-ESI-MS/MS metabolomics, we comprehensively characterized the metabolic landscape in the airway across early, mid, and severe stages of pneumococcal pneumonia. This revealed that dynamic shifts in metabolites occurred during pneumonia, with an initial influx of metabolites at the early stage, followed by declines as the disease progressed. Specific host metabolic perturbation

Phage Therapy against Streptococcus pneumoniae: Modern Tool to Control Pneumonia.

Phage therapy is important for treatment of drug-resistant pathogens as compared to antibiotics in this modern era. Since 1966, bacteriophages have been used as antibacterial agents and played a very crucial role in the expansion of molecular biology. Bacteriophages have been used to treat infection in Western medicine along with antibiotics. Antibacterial agents against the antibiotic resistance strains have been discovered. The lytic bacteriophage used for treatment in conventional phage therapy have shown hopeful results in human clinical cases. In animal models and in vitro studies, phages

Use of genomics to design a diagnostic assay to discriminate between Streptococcus pneumoniae and Streptococcus pseudopneumoniae.

Distinuishing the species of mitis group streptococci is challenging due to ambiguous phenotypic characteristics and high degree of genetic similarity. This has been particularly true for resolving atypical Streptococcus pneumoniae and Streptococcus pseudopneumoniae. We used phylogenetic clustering to demonstrate specific and separate clades for both S. pneumoniae and S. pseudopneumoniae genomes. The genomes that clustered within these defined clades were used to extract species-specific genes from the pan-genome. The S. pneumoniae marker was detected in 8027 out of 8051 (>99.7 %) S. pneumonia
