Effect of IL-6 receptor inhibition on infarct volume after endovascular treatment for ischaemic stroke: a phase 2, randomised, placebo-controlled trial.

BACKGROUND: Tocilizumab, an interleukin-6 receptor inhibitor, is a promising cytoprotective agent selected by the Stroke Preclinical Assessment Network. It showed a protective effect on infarct volume and functional outcomes in animal stroke models. METHODS: In this investigator-initiated, multicentre, randomised, double-blind, placebo-controlled trial, patients with acute ischaemic stroke undergoing EVT were recruited. Eligible patients were randomly assigned (1:1) to receive tocilizumab or placebo treatment. Both patients and investigators were blinded to the treatment assignments. A single

Goal-directed fluid therapy using stroke volume variation does not result in pulmonary fluid overload in thoracic surgery requiring one-lung ventilation.

Background. Goal-directed fluid therapy (GDT) guided by functional parameters of preload, such as stroke volume variation (SVV), seems to optimize hemodynamics and possibly improves clinical outcome. However, this strategy is believed to be rather fluid aggressive, and, furthermore, during surgery requiring thoracotomy, the ability of SVV to predict volume responsiveness has raised some controversy. So far it is not known whether GDT is associated with pulmonary fluid overload and a deleterious reduction in pulmonary function in thoracic surgery requiring one-lung-ventilation (OLV). Therefore,…

Assessment of regional cerebral blood volume in acute human stroke by use of single-slice dynamic susceptibility contrast-enhanced magnetic resonance imaging.

BACKGROUND AND PURPOSE: The purpose of this study was to evaluate the clinical usefulness of dynamic susceptibility contrast-enhanced MRI (DSC-MRI) in acute cerebral ischemia. METHODS: During bolus injection of gadolinium-diethylenetriamine pentaacetic acid, a series of rapid T2*-weighted images was recorded from one slice. Concentration-time curves and images of regional cerebral blood volume (rCBV) were calculated from this data set. DSC-MRI, MR angiography, conventional spin-echo MRI (SE-MRI), and CT were performed in 11 patients within 6 hours after stroke onset and before thrombolytic or

Surface and volume three-dimensional displays of Tc-99m HMPAO brain SPECT images in stroke patients by a three-headed gamma camera.

Twenty patients with strokes were studied to evaluate volume and surface three-dimensional (3D) displays in Tc-99m HMPAO brain SPECT imaging using a triple-head gamma camera interfaced with a 64-bit super computer. In each patient, after an intravenous injection of 20 to 30 mCi of Tc-99m HMPAO, a first-pass cerebral flow study and brain SPECT images at 30 to 60 minutes were obtained. Although the cerebral lesion was more clearly delineated with surface 3D and volume 3D, crossed cerebellar diaschisis in seven patients was more clearly exhibited by volume 3D rather than surface 3D imaging. Multi-scale DWI-based Radiomics for predicting infarct volume evolution in acute ischemic stroke beyond the therapeutic time window: A dual-center study.

OBJECTIVES: This study examined whether radiomics features from multi-scale diffusion-weighted imaging (DWI) could predict infarct volume reduction during the subacute phase in acute ischemic stroke (AIS) patients beyond the therapeutic window, and whether predicted volume evolution correlates with clinical neurological function. METHODS: Seventy-two AIS patients from Center 1 and 29 from Center 2 were retrospectively analyzed. Core infarct volumes of interest were delineated on baseline DWI, with volume reassessed on follow-up fluid-attenuated inversion recovery imaging. Four volumes of inte

Quantitative Evaluation of Ischemic Core Volume in GE's CT Perfusion Imaging Analysis Software and Its Relationship to Alberta Stroke Program Early CT Score.

OBJECTIVE: Computed tomography (CT) and magnetic resonance imaging of cerebral perfusion are useful in determining the indication of mechanical thrombectomy (MT) for acute ischemic stroke. RAPID (iSchemaView, Menlo Park, CA, USA) is the most common software for analyzing brain perfusion images worldwide, but various other software are also available. The optimal threshold value for each software is different, and each has its characteristics. This study investigated the relationship between the quantitative evaluation of ischemic core volume (ICV) and the Alberta Stroke Program Early CT Score

Predictive value of follow-up infarct volume on functional outcomes in middle cerebral artery M2 segment vessel occlusion stroke treated with mechanical thrombectomy.

BACKGROUND: Medium vessel occlusion (MeVO) strokes, particularly affecting the M2 segment of the middle cerebral artery, represent a critical proportion of acute ischemic strokes, posing significant challenges in management and outcome prediction. The efficacy of mechanical thrombectomy (MT) in MeVO stroke may warrant reliable predictors of functional outcomes. This study aimed to investigate the prognostic value of follow-up infarct volume (FIV) for predicting 90-day functional outcomes in MeVO stroke patients undergoing MT. METHODS: This multicenter, retrospective cohort study analyzed data

Predicting Futile Recanalization in Acute Ischemic Stroke Patients Undergoing Endovascular Thrombectomy: The Role of White Blood Cell Count to Mean Platelet Volume Ratio.

BACKGROUND: Approximately half of AIS patients have an unfavorable outcome even after complete reperfusion. White blood cell (WBC) count to mean platelet volume (MPV) ratio (WMR) may be a promising predictive factor for futile recanalization. This study aimed to determine the predictive value of WMR in identifying individuals at higher risk of futile recanalization. METHODS: In this retrospective cohort study, 296 patients who achieved complete reperfusion after endovascular treatment (EVT) were included in the analysis. WBC count and MPV were collected at admission. Multivariable logistic re

Assessment of Exercise Stroke Volume and Its Prediction From Oxygen Pulse in Paralympic Athletes With Locomotor Impairments: Cardiac Long-Term Adaptations Are Possible.

The determinants of cardiac output (CO) during exercise, i.e., stroke volume (SV) and heart rate (HR), could differ in Paralympic athletes (PAthl) with spinal cord injury (SCI) with respect to PAthl with locomotor impairments caused by different health conditions (HCs). The purposes of the present study were the comparisons of two groups of PAthl, one with SCI and the other with either amputation (AMP) or post poliomyelitis syndrome (PM), assessing the (1) peak cardiorespiratory responses and determinants (SV and HR) of CO during maximal and submaximal arm cranking exercise (ACE), respectively…

Testing of a 50 cc stroke volume completely implantable artificial heart: expanding chronic mechanical circulatory support to women, adolescents, and small stature men.

The development of a completely implanted total artificial heart at our institution has progressed to successful in vivo and in vitro testing of a device that is nearing clinical testing. This system consists of a 70 cc stroke volume pump originally designed to be used in men of average stature. Implantation of this system remains limited by patient size; hence, many women and adolescent patients will likely be precluded from support because of their smaller stature. A system similar in design, but with a 50 cc stroke volume pump has been developed. The first in vivo study of this device has b…

The clot burden score, the Boston Acute Stroke Imaging Scale, the cerebral blood volume ASPECTS, and two novel imaging parameters in the prediction of clinical outcome of ischemic stroke patients receiving intravenous thrombolytic therapy.

INTRODUCTION: Recently two classification methods based on the location and the extent of thrombosis detected with CT angiography have been introduced: the Boston Acute Stroke Imaging Scale (BASIS) and the clot burden score (CBS). We studied the performance of BASIS and CBS in predicting good clinical outcome (mRS ≤ 2 at 90 days) in an acute (< 3 h) stroke cohort treated with intravenous thrombolytic therapy. METHODS: Eighty-three consecutive patients who underwent multimodal CT were analyzed. Binary logistic regression model was used to assess how BASIS, CBS, and cerebral blood volume (CBV)
