A Computational Study of Dynamic Obstruction in Type B Aortic Dissection.

A serious complication in aortic dissection is dynamic obstruction of the true lumen (TL). Dynamic obstruction results in malperfusion, a blockage of blood flow to a vital organ. Clinical data reveal that increases in central blood pressure promote dynamic obstruction. However, the mechanisms by which high pressures result in TL collapse are underexplored and poorly understood. Here, we developed a computational model to investigate biomechanical and hemodynamical factors involved in Dynamic obstruction. We hypothesize that relatively small pressure gradient between TL and false lumen (FL) are

Dise in osa: valuable tool or an overestimated technique? a systematized review.

PURPOSE: Drug-Induced Sleep Endoscopy (DISE) is increasingly used to evaluate upper airway dynamics in obstructive sleep apnea patients (OSA). It enables real-time visualization of collapse and may support treatment selection; however, its true clinical impact and limitations remain controversial. This study aims to summarize the current evidence regarding the real role of DISE in OSA management. METHODS: A structured literature review of the literature was conducted, focusing on key aspects of DISE, including its diagnostic utility, comparison with awake endoscopy, pharmacological agents use

Patterns of Reimperialization and Postimperial strategic cultures. The cases of Liberal Spain (1833-1868) and Post-Soviet Russia (1991-2025).

This article explores the structural patterns of post-imperial conflict through a comparative analysis of Liberal Spain (1833-1868) and post-Soviet Russia (1991-2025). It argues that both powers, following the abrupt collapse of their empires, developed strategic cultures oriented toward reimperialization-defined as the attempt to reassert influence over former imperial domains through cultural, diplomatic, or military means. Drawing on theories from historical sociology, international relations, and strategic studies, the article introduces the concepts of "levels of imperiality" and "degrees

Evaluation of the Patient with Nasal Obstruction.

Patients often present with a complaint of nasal blockage, either primarily, or in conjunction with aesthetic concerns. The evaluation of the patient with nasal obstruction involves a comprehensive history and a detailed physical examination. The nose is an organ in which form and function are inseparable, and as such, examination of the patient with nasal obstruction must focus not only the internal structures that may cause obstruction of the nasal airway, but also the external structure of the nose as it impacts nasal breathing. Detailed facial analysis and a systematic nasal examination wi

Activation of ALDH2 with Low Concentration of Ethanol Attenuates Myocardial Ischemia/Reperfusion Injury in Diabetes Rat Model.

The aim of this paper is to observe the change of mitochondrial aldehyde dehydrogenase 2 (ALDH2) when diabetes mellitus (DM) rat heart was subjected to ischemia/reperfusion (I/R) intervention and analyze its underlying mechanisms. DM rat hearts were subjected to 30 min regional ischemia and 120 min reperfusion in vitro and pretreated with ALDH2 activator ethanol (EtOH); cardiomyocyte in high glucose (HG) condition was pretreated with ALDH2 activator Alda-1. In control I/R group, myocardial tissue structure collapse appeared. Compared with control I/R group, left ventricular parameters, SOD act

Flatfoot: New diagnostic modalities.

Adult acquired flatfoot deformity, recently renamed Progressive Collapsing Foot Deformity (PCFD), is challenging to diagnose and treat due to the still poorly understood nature of its pathogenesis, which involves a complex interaction between soft tissues and bony structures. Long regarded as being primarily linked to posterior tibial tendon dysfunction, PCFD is now considered a multifactorial deformity (osseous dysplasia, joint malposition, tendon muscle imbalance, etc.), with many aspects yet to be explored. This study aims to provide an update on this pathology by addressing the following fiv

Central venous stenosis caused by traction of the innominate vein due to a tuberculosis-destroyed lung.

We report a case of central venous stenosis due to a structural deformity caused by a tuberculosis-destroyed lung in a 65-year-old woman. The patient presented with left facial edema. She had a history of pulmonary tuberculosis, and the chest X-ray revealed a collapsed left lung. Angiography showed leftward deviation of the innominate vein leading to kinking and stenosis of the internal jugular vein. Stent insertion improved her facial edema.

A Case Report of Tracheobronchomalacia Requiring Stenting and Extracorporeal Membrane Oxygenation Treatment.

BACKGROUND: Tracheobronchomalacia (TBM) is characterized by the excessive expiratory collapse of the central airway due to structural weakness or membranous redundancy of the tracheal and bronchial cartilages. TBM may present with anatomical and physiological compromise of the airways. Aggressive treatments for severe respiratory failure include mechanical ventilation and extracorporeal membrane oxygenation (ECMO). Interventional approaches include short-term silicone stent trials to predict surgical benefits and definitive procedures, such as tracheobronchoplasty. CASE PRESENTATION: An Asian

The buckling-condensation mechanism driving gas vesicle collapse.

Gas vesicles (GVs) are proteinaceous cylindrical shells found within bacteria or archea growing in aqueous environments and are composed primarily of two proteins, gas vesicle protein A and C (GvpA and GvpC). GVs exhibit strong performance as next-generation ultrasound contrast agents due to their gas-filled interior, tunable collapse pressure, stability in vivo and functionalizable exterior. However, the exact mechanism leading to GV collapse remains inconclusive, which leads to difficulty in predicting collapse pressures for different species of GVs and in extending favorable nonlinear respo

Lung Volume Reduction Coils as a Novel Bronchoscopic Treatment for Emphysema.

Chronic obstructive pulmonary disease remains a major cause of morbidity and mortality worldwide. Despite regular advances in pharmacology, there remains great potential for addressing structural deficiencies, especially in emphysema. The loss of alveolar attachments to small bronchial tubes results in diffuse loss of elastic recoil and airway collapse during exhalation. This appears physiologically as hyperinflation of lung volumes with flattened diaphragms and significantly elevated residual volumes (RV) on pulmonary function testing (predicted RV > 175%).

Perforation of the hard palate associated with cocaine abuse.

Cocaine abuse is associated with serious systemic complications. Snorting cocaine can also cause complications of the nasopharyngeal structures. Repeated episodes of vasoconstriction and subsequent ischemia may cause this destruction. We present a patient with perforation of the nasal septum and palate and collapse of the nasal dorsum. The destroyed anatomy was reconstructed by using standard surgical techniques. Palatal destruction is a rare entity and, to our knowledge, this patient is the first reported patient with palatal destruction due to cocaine insufflation.

Precision-Guarded Graph-Text Alignment for Universal Chemical Understanding.

Large Language Models (LLMs) have demonstrated transformative potential in scientific discovery but frequently suffer from "semantic-structure misalignment"─generating syntactically plausible but chemically invalid structures, or failing to capture precise numerical properties. Existing multimodal adaptations often employ naive projection layers that, under mixed-precision training, lead to feature collapse and the loss of fine-grained topological information. In this work, we propose Deep Graph-Text Alignment (DGTA), a precision-first framework designed to unify structural generation and regr

Defective cortex glia plasma membrane structure underlies light-induced epilepsy in cpes mutants.

Seizures induced by visual stimulation (photosensitive epilepsy; PSE) represent a common type of epilepsy in humans, but the molecular mechanisms and genetic drivers underlying PSE remain unknown, and no good genetic animal models have been identified as yet. Here, we show an animal model of PSE, in Drosophila, owing to defective cortex glia. The cortex glial membranes are severely compromised in ceramide phosphoethanolamine synthase (cpes)-null mutants and fail to encapsulate the neuronal cell bodies in the Drosophila neuronal cortex. Expression of human sphingomyelin synthase 1, which synthe

Coronal Fracture of the Lunate in Advanced Kienböck Disease: Reestablishing Midcarpal Congruency to Enable Osteochondral Reconstruction: A Case Report.

CASE: We describe a patient with Bain grade-2b Kienböck disease with a coronal fracture of the distal lunate articular surface with proximal structure collapse. To reestablish midcarpal congruity, we used a bone-anchored suture to repair the coronal split in the lunate in preparation for lunate preservation with osteochondral medial femoral trochlea (MFT) reconstruction. CONCLUSION: In Bain grade-2b Kienböck disease, lunate reconstruction of both the proximal and distal joint surfaces can be performed. Successful reconstruction of the distal articular surface can be achieved using a bone-anch

Cardioembolic Stroke and ST-Segment Elevation Myocardial Infarction (STEMI) Due to Left Atrial Myxoma in an Elderly Pilgrim: A Case Report.

Left atrial myxomas are the most common primary cardiac tumors, but may present deceptively. A 77-year-old woman with type 2 diabetes collapsed during Tawaf (circumambulation of the Kaaba in Makkah) with pulseless electrical activity arrest. Return of spontaneous circulation was achieved, and subsequent evaluation revealed a large left atrial myxoma complicated by inferior ST-segment elevation myocardial infarction (STEMI) and multiple posterior circulation infarcts. Echocardiography demonstrated a mobile septal mass prolapsing into the left ventricle, while brain imaging confirmed bilateral c

Echocardiographic Evaluation of Pericardial Effusion and Cardiac Tamponade.

Pericardial effusion (PEff) is defined by an increase in the physiological amount of fluid within the pericardial space. It can appear following different medical conditions, mainly related to inflammation and cardiac surgery. Cardiac tamponade is a critical condition that occurs after sudden and/or excessive accumulation of fluid in the pericardial space that restricts appropriate filling of the cardiac chambers disturbing normal hemodynamics and ultimately causing hypotension and cardiac arrest. It is, therefore, a life-threatening condition that must be diagnosed as soon as possible for cor

A "rising" transpedicular access to anterior vertebral body: A case report.

INTRODUCTION: The transpedicular access is a common and effective procedure used to reach anterior vertebral body without passing through critical structures. after a transpedicular instrumentation, it is difficult to reach the anterior vertebral body because screws are placed in the way. We assume that an innovative transdiscal route could be used in patients with previous instrumentation, in order to perform a vertebroplasty or biopsy. PRESENTATION OF CASE: We report the case of a 65-years-old woman who came to our Department complaining low back pain. Neurological examination showed a stab

Use of bone graft substitute in the treatment for distal radius fractures in elderly.

INTRODUCTION: Fractures of the distal radius in elderly patients are often associated with metaphyseal defects that can lead to collapse, malunion and therefore decreased function. An alternative approach to simple reduction is to fill the defects with materials that can provide structural support. METHODS: We used synthetic hydroxyapatite (HA) in unstable fractures of the distal radius in thirty-one elderly patients, of which four patients lost to follow-up, leaving twenty-seven patients for this study. All subjects underwent closed reduction with K-wire fixation and HA augmentation. They we

Cassava starch enhances the printability and swallowability of rapeseed protein gels for 3D-printed dysphagia foods.

Plant-based proteins have attracted increasing attention in the development of easy-to-swallow foods and three-dimensional printing (3DP) materials due to their excellent nutritional properties. However, the disordered arrangement of protein molecular chains in rapeseed protein (RP) results in insufficient structural stability and poor self-supporting capacity during 3DP, leading to collapse behaviour. To address this limitation, cassava starch (CS) was incorporated into RP gels at different CS: RP ratios to improve printability and swallowing properties. FT-IR and XRD analyses demonstrated th
