Association Between Level of Arousal and 30-Day Survival in Acutely Ill Older Adults.

OBJECTIVE: To investigate the association between impaired arousal on admission and 30-day mortality in acutely ill older adults. DESIGN: R … Assessment Method (Short-CAM). We used 2 alternative criteria to define impaired arousal: lethargy, stupor, or coma according to the Short-CAM; and a Glasgow Coma Scale (GCS) score of ≤13. Our primary outcome was time-to-death in 30 days, and we used Cox proportional hazards models to explore the association between impaired arousal and decreased surviva …

Hyperosmolar nature of diabetic coma.

Stupor in patients with nonketotic hyperglycemia has been ascribed to hyperosmolarity, but the cause of depressed consciousness in patients with ketoacidosis has been puzzling. In this study, blood pH, serum glucose and sodium concentrations, and serum osmolality were measured in eighty-five consecutive episodes of diabetic ketoacidosis and forty-seven of nonketotic hyperglycemia. In the acidotic patients, as in those with nonketotic hyperglycemia, stupor closely paralleled hyperosmolarity and not the severity of acidemia. Indeed, the mean elevations of serum osmolarity were almost the same in…

[Subacute citrullinemia--diagnosis and course up to 4th year of life].

The article reports on an infant which first in age of nine months and then again in age of 1.6 years had an episode of stupor. Laboratory investigation showed hyperammonemia and high values of transaminases. The differential diagnostic check up led to the diagnosis of citrullinemia in the age of 1.7 years by amino acid chromatography. The diagnosis could be confirmed by measuring enzyme activity in the liver. Therapy is protein-low feeding and supplementation of essential amino acids and arginine. A therapeutic trial with additional sodium benzoate did not show a positive effect. The patient …

Can Pure Thalamic Strokes Lead to Severe Impairment of Arousal?

BACKGROUND: The thalamus has been considered critical for maintaining consciousness, but it is not clear if thalamic strokes can lead to sev … impairment of arousal were included as controls. RESULTS: We identified nine patients with coma or stupor following an acute stroke involving the thalamus. Five of these patients remitted following endovascular therapies but had residual lesions intersecting the thalamus. In the four patients with long-term coma or stupor, the thalamic lesions extended …

Regional acetylcholine metabolism in brain during acute hypoglycemia and recovery.

Insulin-induced hypoglycemia in normothermic rats caused progressive neurological depression and differentially altered regional cerebral acetylcholine metabolism. Reductions of plasma glucose from 7.7 mM (control) to 2.5-1.7 mM (moderate hypoglycemia associated with decreased motor activity) or 1.5 mM (severe hypoglycemia with lethargy progressing to stupor) decreased glucose concentrations in the cerebral cortex, striatum, and hippocampus to less than 10% of control. Moderate hypoglycemia diminished acetylcholine concentrations in cortex and striatum (21% and 45%, respectively) and reduced […
