Biomechanical assessment of Kirschner wires integrated with a novel external fixation device for treatment of pediatric supracondylar humeral fracture: a finite element analysis.

BACKGROUND: Pediatric supracondylar humeral fractures present considerable surgical challenges due to the difficulty of achieving proper fracture alignment and stable fixation while avoiding injury to the ulnar nerve. This study assesses the biomechanical performance of a novel Kirschner wire (K-wire) fixation device (KFD), designed to enhance stability and reduce complications linked to traditional K-wire configurations. METHODS: Using finite element analysis (FEA), we evaluated four fixation strategies for treatment of pediatric supracondylar humeral simple transverse fractures: crossed pin

Application of Self-Made Connection Device in Intractable Intramedullary Device.

BACKGROUND: Intramedullary fixation a standard surgical technique for long bone meta/diaphyseal fractures. There were many difficulties in removal of the intractable intramedullary device. The authors reported a new technique to remove the intractable intramedullary nail by using a self-made connecting device. METHODS: The subject underwent removal of the intramedullary nail using a self-made connecting device, the core components of which were a caudal connecting rod and a sliding hammer in the common intramedullary nail removal device, and the auxiliary device was mainly a clinically common

Cleft Lip and Palate: Le Fort I Distraction Using an Internal Device.

Maxillary hypoplasia is a commonly seen dentofacial anomaly in patients with cleft lip and palate after surgical repair of the cleft anomaly. To facilitate large horizontal movements of the maxilla, distraction osteogenesis is used to slowly stretch the soft tissue envelope with the skeletal advancement preventing tissue recoil and skeletal relapse. Internal distraction devices have the advantage of reducing the amount of physical and psychological stress placed on patients, families, and caregivers. The technique has been successful in producing stable results for large advancements of the fa

Management of severe frontobasilar skull fractures.

Immediate reconstruction of severely comminuted frontobasilar skull fractures involving the frontal sinus can be accomplished safely if the sinus is cranialized and the floor of the anterior cranial fossa is totally reconstructed. Operative time can be reduced if the pieces of the frontal bone are reassembled and stabilized with rigid fixation devices out of the surgical field and thus made ready for reinsertion as a single unit after any necessary neurosurgical procedures are completed. Immediate reconstruction provides postoperative protection for the brain and eliminates the need to return

Early outcomes of the synthes femoral neck system for valgus-impacted femoral neck fractures: a multicenter retrospective study.

INTRODUCTION: Nondisplaced and valgus-impacted femoral neck fractures have traditionally been treated with screw fixation, but reoperation rates remain high. This study reports clinical outcomes following internal fixation of low-energy femoral neck fractures (AO/OTA 31B1.1 and 31B1.2) using a new, fixed-angle sliding device. METHODS: This was a multi-institutional retrospective review at four academic Level I Trauma Centers, one Level II Trauma Center, and one Level III Trauma Center. Patients with AO/OTA 31B1.1 and 31B1.2 fractures who underwent internal fixation using the Femoral Neck Syst

Soft tissue anaplastic large T-cell lymphoma associated with a metallic orthopedic implant: case report and review of the current literature.

On occasion, the placement of orthopedic prosthetic components or stabilization hardware leads to surgical site infections, in some cases presenting at a point in time distant from the surgical procedure. Although infectious complications are the most common etiology for surgical site pain and inflammation, malignancies can also develop around prosthetic devices and metallic implants. When encountered, such malignancies are most often sarcoma, but rarely B-cell non-Hodgkin's lymphoma has also been described. In this article, we describe what we believe to be the first published case of anaplas

Controlled percutaneous gastrostomy: nylon T-fastener for fixation of the anterior gastric wall.

We devised a fixation device using the principles of a garment T-fastener to fix the anterior gastric wall to the abdominal wall for percutaneous gastrostomy. Insertion of large (14-16-F), soft, Foley-type catheters without the problem of buckling of the catheter and/or guide wire into the peritoneum, which has been described by previous methods, is facilitated. Also, fixation of the stomach has prevented leakage of gastric contents into the peritoneum and allows formation of adhesions between the abdominal and gastric walls, similar to surgically placed tubes. We initially used this technique

Femoral neck system vs conventional fixation: Long-term outcomes from a single-center study.

BACKGROUND: The Femoral Neck System (FNS) is a minimally invasive fixation device for femoral neck fractures, but its comparative long-term performance versus conventional fixation remains debated. MATERIALS AND METHODS: In this single-center retrospective study (Jan 2020-Oct 2022) all patients treated surgically for femoral neck fractures were included. Primary outcomes were operative time, peri-operative bleeding, fixation failure, avascular necrosis, reoperation, mortality, and autonomy using Parker score. A multivariable logistic regression assessed risk of postoperative loss of autonomy.

A minimally invasive technique for the reduction of calcaneal fractures using the Endobutton®.

Calcaneal fractures are very common injuries, and closed reduction-percutaneous pinning, open reduction-internal fixation, and primary arthrodesis are the procedures used in the surgical treatment of these injuries. The aim of surgical treatment is to restore the normal biomechanics of the Achilles tendon and rearfoot by anatomic reduction of the articular surfaces. Minimization of the soft tissue complications commonly associated with the open treatment of calcaneal fractures can be achieved using indirect closed reduction and Kirschner wire or screw-assisted minimally invasive percutaneous o

Gamma Nail Combined with One Cannulated Compression Screw Fixation for Treating Pauwels Type III Femoral Neck Fractures in Young and Middle-Aged Adults: Clinical Follow-Up and Biomechanical Studies.

OBJECTIVE: Recently, some clinical studies have reported the use of an intramedullary nailing system for treating unstable femoral neck fractures or femoral neck fractures combined with femoral shaft fractures in young adults, and the results have indicated certain advantages. However, no study has investigated the mechanical properties of this method. We aimed to evaluate the mechanical stability and clinical efficacy of the Gamma nail combined with one cannulated compression screw (CCS) fixation for treating Pauwels type III femoral neck fracture in young and middle-aged adults. Intense granulomatous inflammatory lesions associated with absorbable internal fixation devices made of polyglycolide in ankle fractures.

Since absorbable internal fracture fixation devices made of polyglycolide have been increasingly used clinically, a peculiar type of complication has emerged. After an initially uneventful course, a local nonbacterial inflammatory reaction appears two to four months after the operation, resulting in a copiously discharging sinus on the skin. A series of 286 patients with unimalleolar or bimalleolar fractures were treated by open reduction and internal fixation using cylindrical rods made of polyglycolide. Among these there occurred 18 nonbacterial inflammatory tissue responses (6.3% of the tot

Laparoscopic-extracorporeal surgery performed with a fixation device for adnexal masses complicating pregnancy: Report of two cases.

The potential complications associated with an adnexal mass discovered during early pregnancy call for surgical treatment. Ideally, surgery is performed after gestational week 12, but uterine expansion after the first trimester makes surgery difficult. We report two pregnancies complicated by adnexal masses for which we used an organ fixation device for safe performance of single-site umbilical laparoscopic surgery. Pelvic magnetic resonance imaging depicted a dichorionic, diamniotic twin pregnancy and 60-mm right adnexal mass in the first patient and bilateral adnexae in the second. All three

[Ipsilateral femoral neck fracture after fixation of intertrochanteric fracture by InterTAN intramedullary nail: A case report].

Hip fractures are common in elderly patients and are associated with significant morbidity and mortality, often referred to as the "last fracture of life". These fractures frequently result in a loss of functional independence. Evidence suggests that early surgical intervention can reduce mortality. The selection of treatment modality should take into account factors such as the type of fracture, the patient' s age, and overall health status. This case report discusses an 88-year-old female patient who sustained an unstable intertrochanteric fracture of the left femur following a fall. She und

Suction-fixing surgical device for assisting liver manipulation with laparoscopic forceps.

PURPOSE: Noninvasiveness and stability are significant issues in laparoscopic liver resection. Inappropriate grasping force can cause damage or serious bleeding to the liver. In addition, instability of grasping can result unsafe operations or wavered cutting. We propose a surgical device to improve stability of liver manipulation. METHODS: A proposed device adheres to the liver surface with suction fixation, then tunes its stiffness to being hard and shapes like as a bulge on the liver surface to be grasped with laparoscopic forceps. It consists of two soft beams, a chamber sponge, membrane
