Children with spastic hemiplegia often present with upper limb muscle imbalance. The purpose of this paper was to determine whether reconstructive surgery improved their functional ability. 17 children under the age of 16 years with spastic hemiplegia underwent reconstruction that included tendon transfers, tendon lengthenings and thumb metacarpophalangeal fusion. They were assessed pre-operatively and at an average follow-up period of 2.6 years. Children's abilities were classified according to House's functional rating scale. Tendon transfers improved functional grading by two grades, from g

In sixty-five knees followed for an average of 4.5 years after treatment of subluxation or dislocation of the patella by the slot-block method of medial patellar-tendon transfer, the results were satisfactory in fifty-four (83%) and unsatisfactory in eleven (17%). Recurrent subluxation or dislocation occurred in three (5%) of the knees and was attributed to insufficient displacement. Chondromalacia of the patella became worse after transfer in three of the sixty-two knees in which the distal realignment was technically correct. Two knees (3%) required patellectomy because of patellofemoral sym

BACKGROUND: Tendon transfers are commonly used to restore finger extension in patients with radial nerve palsy, but quantitative comparisons of different techniques remain limited. Achieving the correct tension continues to be a surgical art, guided more by experience than by objective standards. In-silico biomechanical simulation may serve as a valuable tool to evaluate and optimize tendon transfer strategies in a more reproducible and quantitative manner.
METHODS: We used OpenSim software to simulate four tendon transfers targeting the extensor digitorum communis (EDC): flexor carpi radiali

CASE: A 76-year-old woman presented with bilateral hip pain and hip abductor weakness following years of trochanteric bursa steroid injections. MRI showed injury of the hip abductor tendons bilaterally with muscle atrophy, and she underwent staged bilateral hip abductor tendon repair with gluteus maximus transfer, resulting in improvement in pain and function.
CONCLUSION: This is among the first reported cases of bilateral hip abductor tendon repairs with gluteus maximus transfers in a patient with native hips. Patients presenting with hip abductor insufficiency and muscle atrophy may benefit

Ankle and foot tendon transfer involves moving a tendon from its anatomic location to another location to reinforce a weakened muscle-tendon unit. An ideal donor tendon should have appropriate strength, excursion, phase, and tensioning. Imaging plays an important role in treatment selection and pre- and postoperative evaluation. Radiographs enable assessment of hardware position, bone tunnels, and foot alignment. Weight-bearing views or CT provides a more precise evaluation of alignment, potentially uncovering flexible deformities. MRI depicts the status of the donor and transfer sites and bot

INTRODUCTION: By combining nerve and tendon transfer procedures, a more versatile hand function can be expected. Here we report the long-term outcomes of novel, individualized reconstruction strategies using combined nerve and tendon transfer procedures (CNaTT) to restore prehension and grasp in two patients with tetraplegia.
CASE PRESENTATION: Two women, 45 years of age, underwent bilateral nerve transfer according to the Bertelli S-PIN (supinator to posterior interosseous nerve transfer) procedure. The grip reconstruction included tendon transfers using brachioradialis to flexor pollicis lo

Foot drop has various etiologies, such as neurologic (peripheral nerve injury, neuropathy, lumbar radiculopathy, and cerebral lesions) or muscular (extensor muscle injury and compartment syndrome) causes. Tendon transfers can aid in the restoration of dorsiflexion in the treatment of foot drop. The tibialis posterior tendon is the primary tendon used in these surgeries for transfer. In this present study, the flexor hallucis longus tendon is transferred to the dorsum of the foot because of following knee dislocation that caused a nonfunctioning posterior tibial tendon. At the sixth month posto

Iatrogenic peripheral nerve injuries can result from numerous medical procedures, particularly transection, stretching, compression, injections, heat, radiation, and the use of anticoagulant agents. Late diagnosis may lead to atrophy of the motor endplate and result in poor outcomes. We report a case in which the posterior tibial nerve was accidentally sectioned as the posterior tibial tendon for transfer to the anterior tibial tendon in the reconstruction of drop foot. This iatrogenic complication ultimately required foot amputation. Physicians must be aware of the anatomy of the posterior ti

Cerebral Palsy (CP) is a static disorder of movement and posture secondary to an insult to the developing central nervous system. The peripheral manifestations and functional impairments of this condition vary in severity from mild-to-profound. In hemiplegic CP, 1 side of the body is affected more than the other. Spastic hemiplegia is the most common type and that for which upper extremity surgery is most indicated. Treatment options range from physical therapy and splinting to botulinum toxin A injections (Botox) to tendon transfers to arthrodeses. This article will discuss the indications, p

BACKGROUND: Most techniques for posterior tibial tendon transfer attach the posterior tibial tendon somewhere at the dorsum of the foot. Inadequate tendon length and difficulties in securing the tendon to bone or tendon can complicate these procedures. Performing the tenodesis proximal to the ankle joint can prevent these problems. The present study is the first to describe the results of posterior tibial tendon transfer through the interosseous membrane attaching only the posterior tibial tendon to the extensor tendons proximal to the ankle joint.
METHODS: We examined 13 feet in 12 patients

BACKGROUND: Adult acquired flatfoot is a common condition that leads to significant morbidity. Along with bony procedures to operatively treat this condition, transfer of the flexor digitorum longus (FDL) tendon to the medial cuneiform or navicular is routinely performed. The goal of this tendon transfer is to increase the capacity of the FDL to invert the hindfoot and control the transverse tarsal joints. However, it is not known whether this biomechanical goal is met or whether one transfer site produces a larger mechanical advantage compared to another site. The purpose of this study was to

This study is the review of 16 hands in the same number of patients (15 men, 1 female) in whom ring finger flexor digitorum superficialis opponensplasty was performed. The patients were operated on during the period of 6 years. The lack of thumb opposition was a result of traumatic nerve leasions (5 median, 11 median and ulnar). The operation was performed 7.2 months (on average) after the nerve reconstruction. The outcome was assessed by scoring the finger to which the thumb could obtain a pinch grip (Kapandji's test), measuring the gap between thumb and little finger MCP joints, the patient'

Three patients, a woman aged 33 years, a man aged 42 and a woman aged 66, had a loss of hand function. This was due to denervation of a peripheral nerve, in essence the radial, ulnar and median nerves respectively. In the first two cases this was caused by trauma and in the third by compression neuropathy. The problems were loss of wrist and finger extension (a dropping hand) in the first, loss of intrinsic muscle function and thumb adduction in the second and loss of opposition in the third patient. Tendon transfers were performed by diverting a tendon insertion of a still functional muscle t

BACKGROUND: Paralysis of the serratus anterior muscle can be functionally disabling. As a result of the scapular winging associated with such paralysis, the scapula does not remain apposed to the thorax when the upper extremity is elevated forward at the shoulder. This produces functional disability associated with pain and loss of a stable base for movement of the upper extremity.
METHODS: We reviewed the results of transfer of the pectoralis major tendon with the addition of a fascial graft in sixteen patients who had paralysis of the serratus anterior. The average age of the patients at th

This is a retrospective study of the modified Robert Jones tendon transfer in 24 patients and 28 feet. The main indication for the procedure was to correct symptoms related to clawing of the hallux in seven patients, relieve pressure under the head of the first metatarsal in 13 patients, and treat for both symptoms in three patients. One patient had diffuse ache in the arch. Fifteen patients had idiopathic cavus feet, of whom five had associated spina bifida occulta. The remainder had a variety of different etiologies including cerebral palsy, head injury, brain abscess, and spinal cord tether
