Experience in the control of tetanus neonatorum (TN) in a rural area is presented. TN was reduced by training of dais, increasing the coverage of tetanus toxoid to pregnant women and distribution of presterilized delivery kits to pregnant ladies for use by birth attendants. The problems of untrained birth attendants, effectiveness of tetanus toxoid coverage and place of delivery are discussed.

Prior to the Second World War, toxoid immunizations for both tetanus and diphtheria had been developed but were not widely used in adults. Starting in 1941, however, the U.S. Army began extensive immunization with tetanus toxoid. We developed a highly sensitive and reproducible enzyme-linked immunosorbent assay (ELISA) and a qualitative immunochromatography (IC) test to specifically measure antibodies against tetanus toxoid. A cluster survey on neonatal tetanus (NNT) mortality was conducted in Kano Metropolis, Northern Nigeria, to investigate epidemiological factors associated with NNT mortality. A 5-year-old boy was evaluated for a history of an anaphylactic reaction to a DPT immunization at 18 months of age. Cutaneous tests and RAST disclosed high titers of IgE antibody to tetanus toxoid vaccine. Variable sensitivity to different vaccine commercial preparations was demonstrated. Immunization was achieved using a 9-step graded dosing schedule with the tetanus toxoid vaccine to which the patient was least sensitive.
