In this article the author reviews outcomes of thoracic disc surgery reported in the literature. Care was taken to include only series in which none or a few patients underwent laminectomy for the treatment of thoracic disc lesions. The author found that thoracic discectomy may be performed in an efficacious and safe manner via the transthoracic, lateral extracavitary, transpedicular, or endoscopic approach. In the vast majority of patients, preoperative symptoms of pain (radiculopathy or axial loading pain) and myelopathy were improved after surgery. Measurements of operative time, blood loss

Postoperative paraplegia caused by ischemic injury of the spinal cord is the most disabling complication of thoracoabdominal surgery, particularly when repair of the descending thoracic aorta is involved. We describe the case of a 59-year-old man who underwent emergency surgery for placement of a Dacron prosthesis to repair a ruptured descending thoracic aorta aneurysm, using an aortic cross-clamping technique plus aortic-femoral partial bypass with normothermia and an ischemic time of 165 minutes. The early postoperative course included complete spinal syndrome with motor and sensory loss bel

Indocyanine green (ICG) is a fluorescent dye used for near-infrared (NIR) intraoperative imaging. In thoracic surgery, it is used to delineate intersegmental planes during anatomical segmentectomy, localizing small, nonpalpable lung lesions, assessing gastric conduit perfusion before esophageal anastomosis, and identifying the thoracic duct. A review of the latest literature (2020-2025) indicates that ICG significantly increases the precision of segmentectomy (allowing visualization of the intersegmental plane) and the effectiveness of lung nodule localization (> 95%), as well as enabling obje…

Splenic injury after thoracic surgery is rare and has not been reported following robotic-assisted thoracic surgery (RATS). We describe a 52-year-old woman who underwent RATS left upper lobectomy. After discharge, she returned with abdominal pain and hypotension. Computed tomography revealed hemoperitoneum with signs of splenic injury. Emergent laparoscopic splenectomy confirmed splenic rupture. This case highlights that unrecognized intraabdominal trauma can occur during RATS; surgeons should exercise caution near the diaphragm and remain vigilant post-discharge.

This review documents the importance of postoperative interventions that accelerate the functional recovery of the thoracic surgical patient. Enhanced recovery after surgery (ERAS) pathways aim to mitigate the harmful surgical stress response. Improvements to the entire patient pathway, by removing unnecessary care elements while introducing evidence-based interventions, have synergistic effects. At the same time, some key care elements appear to be more important than others, including early removal of chest tubes, early mobilization, and the limited use of opioids. These care elements are al

Perioperative stroke and neurological injuries in general are complications that can occur during and after cardiac surgery, particularly in aortic surgery that involves the aortic arch. The overall incidence of early and delayed stroke is about 1% according to recent meta-analyses. This incidence depends on interindividual risk factor profile and type of surgery. In order to reduce cerebrovascular complications during cardiac surgery, a number of preventative measures can be taken, including the evaluation of atherosclerotic plaques, the site of cannulation and neuroprotection strategies. One of the main tasks in every anesthetist's routine clinical practice is securing the airway. This also includes techniques for lung isolation and one-lung ventilation in thoracic surgery and in intensive care medicine. The anesthesiologist has various methods available to achieve one-lung ventilation. This article presents the most commonly used methods for lung isolation. These include the double lumen tube, the bronchus blockers by Arndt and Cohen, the EZ blocker, the Uniblocker, the Univent tube and the VivaSight-DL™. The effects of the one-lung ventilation are not described in detail and…

Background Recent evidence surrounding the use of venovenous extracorporeal membrane oxygenation in treating acute respiratory failure has led to the expansion of extracorporeal membrane oxygenation services worldwide. The high rate of complications related to venovenous extracorporeal membrane oxygenation often requires intervention by specialist thoracic surgeons. This study aimed to investigate the role of specialist thoracic surgeons within the multidisciplinary team managing these high-risk patients. Methods We retrospectively reviewed 90 patients who received venovenous extracorporeal me

INTRODUCTION: Robot-assisted surgery emerged and evolved in order to increase the surgical precision and due to the need to overcome the drawbacks of conventional minimally invasive surgery. In thoracic surgery the first reported use of a robotic device was in a series of 12 patients with different lung pathologies with the assistance of the DaVinci Robotic Surgical System in 2002. The DaVinci system has been used for various procedures in the field of thoracic surgery since then. While its advantages for the resection of early stage thymoma have been well documented, its role in the treatment…

Despite significant improvements in perioperative care, major surgery is still associated with major complications. Enhanced recovery after surgery was introduced by the National Health Service in the UK with the aim of improving patient outcomes and reducing length of stay in hospital. The degree of applicability differs between surgical specialties, and in thoracic surgery it has not been developed until recently. We have therefore reviewed recent literature specific to thoracic surgery, and will discuss key elements of the design, implementation and monitoring of an enhanced recovery (ER) p…

Non-intubated thoracic surgery (NITS) entails procedures performed through regional anesthesia methods in awake or mildly sedated, spontaneously ventilating patients. The rationale is the avoidance of side-effects of intubated general anesthesia and maintenance of more physiologic muscular, neurologic, and cardiopulmonary status in order to reduce the procedure-related traumas, fasten recovery and optimize outcomes. Preliminary reports including some randomized studies have suggested optimal feasibility of several surgical procedures including management of pleural effusion, of spontaneous pne…

AIM: Epidural analgesia is considered to be the best method of pain relief after major surgery despite its side-effects, which include hypotension, respiratory depression, urinary retention, incomplete or failed block, and, in rare cases, paraplegia. Paravertebral block is an alternative technique that may offer a comparable analgesic effect and a better side-effect profile. This study measured postoperative pain and respiratory function in patients randomized to receive either paravertebral block or epidural analgesia for pain control after thoracic surgery.

BACKGROUND: Acquired elevation of the diaphragm is mostly the result of phrenic nerve paralysis, some of thoracic and abdominal patological states, and also some of neuromuscular diseases. Surgical treatment is rarely performed and is indicated when lung compression produces disabilitating dyspnea, and includes plication of diaphragm. The goal of this case report has been to show completely documented diagnostic procedures and surgical treatment one of rare pathological condition.
