Successful free flap transfer following venous thrombectomy in recipient vessel.

We report the case of a 53-year-old male patient who suffered a high velocity multiple trauma with bilateral open tibial fractures. At definitive orthopaedic and plastic surgical reconstruction 5 days post initial trauma, he was found peroperatively to have an existing deep venous thrombosis in his popliteal vein on one side. He underwent venous thrombectomy and had subsequent successful latissimus dorsi flap transfer using the unblocked popliteal vein as a recipient vessel.

Video-assisted thoracoscopic left ventricular thrombectomy and coronary artery revascularization.

Advances in video-assisted techniques in the fields of general and thoracic surgery have led to the application of this technology in experimental and clinical cardiac surgery. Visualization of intracardiac anatomy and pathology allows procedures to be performed without violation of ventricular muscle, thus preserving myocardial function. We present the case of a patient with a recent myocardial infarction, found to have a large left ventricular thrombus on echocardiogram. By applying thoracoscopic surgical technology we successfully removed the thrombus during emergency coronary artery revasc

Percutaneous recanalization of acutely thrombosed vessels by hydrodynamic thrombectomy (Hydrolyser).

A hydrodynamic thrombectomy catheter was prospectively evaluated for the treatment of recently thrombosed vessels. A total of 52 consecutive patients (42 males and 10 females; mean age 64 +/- 15 years) presenting with acute or subacute occlusion of dialysis shunts (n = 25), peripheral bypass (n = 14) or native arteries (n = 15) were treated with the Hydrolyser (Cordis Europa NV, Roden, The Netherlands). Mean occlusion time was 4 days (range 1-17 days) and mean thrombus length 19 +/- 11 cm. The Hydrolyser was effective and fast in removing thrombus, regardless of the thrombus length. No major c

Distal embolization during mechanical thrombolysis: rotational thrombectomy vs. balloon angioplasty.

To determine the incidence and extent of distal embolization during percutaneous transluminal balloon angioplasty (PTA) and rotational thrombectomy (PRT), we collected, filtered, and weighed the distal effluent of acute thrombotically occluded canine arteries following mechanical thrombolysis. PRT (n = 11) and PTA (n = 10) were equally effective in recanalizing occluded vessels (91% vs. 90%) and reduced percent diameter stenosis to a similar degree (97 +/- 6% to 8 +/- 11% and 100 +/- 0% to 17 +/- 23%, respectively). Distal embolization following mechanical intervention was observed in 10 of 10

Percutaneous balloon occlusion of surgical arteriovenous fistulae following venous thrombectomy.

We describe a percutaneous method of balloon occlusion of surgically created femoral arteriovenous fistulae (AVF) after thrombectomy for acute iliofemoral venous thrombosis. The technique was successful in permanent obliteration of the AVF in 25 of 27 patients. Complications were few, minor, and limited to the developmental period of the procedure. No patient required surgical intervention. The procedure provides an opportunity to angiographically evaluate the results of previous thrombectomy.

[Current Evidence of Mechanical Thrombectomy for Acute Cerebral Artery Occlusion].

Following previous negative randomized controlled trials (RCTs), the validity of mechanical thrombectomy (MT) for large-vessel occlusion (LVO) of the anterior circulation was established by multiple positive RCTs in 2015. The success of these RCTs was achieved by introducing a stent retriever, which enabled a safer procedure and higher revascularization rate. Since then, novel evidence has been accumulated by several emerging RCTs to expand the indication of MT. First, the time limit for MT was extended based on RCTs demonstrating the efficacy of MT in patients with a longer penumbra on radiol

Balloon-assisted retrieval of detached microwire tip during mechanical thrombectomy.

Mechanical thrombectomy is the standard of care for acute ischemic stroke due to large vessel occlusion.1 The reported intraprocedural complication rate is approximately 8%, including emboli to new territory, perforations and dissections.2 3 Rarely, technical device failures such as microwire tip detachment can occur during neurointerventional procedures. Previously reported cases of guidewire detachment have utilized microsnares or stent retrievers for retrieval.4 5 In this technical video 1, we describe one such complication of microwire tip detachment and its successful retrieval. Stent retriever size and outcomes after anterior circulation occlusion thrombectomy.

BACKGROUND: The impact of stent retriever size on mechanical thrombectomy (MT) outcomes remains uncertain. We aim to clarify the influence of stent retriever size on MT outcomes by analyzing data from two national prospective registries. METHODS: A retrospective analysis was performed on data from the French and German MT registries including consecutive patients with anterior circulation large vessel occlusion who underwent Solitaire stent retriever MT with or without additional aspiration. Efficacy outcomes were successful reperfusion and complete reperfusion. Safety outcomes included any i

Thrombectomy for medium-sized cerebral vessel occlusion: Size does matter.

INTRODUCTION: Randomised controlled trials comparing endovascular thrombectomy (EVT) to medical treatment in patients with medium vessel occlusion (MeVO) suggested neutrality or futility of EVT. We studied whether the size difference between thrombectomy device and the occluded vessel influenced MeVO outcomes. PATIENTS AND METHODS: This was a retrospective single-centre observational study comprising EVT-treated patients with occlusion of the M2 branch of the middle cerebral artery on digital subtraction angiography. The diameter of the occluded M2 was measured and compared to the manufacture

Tenecteplase versus Alteplase and First-Pass Reperfusion in Endovascular Thrombectomy.

BACKGROUND AND PURPOSE: It is unknown whether thrombolysis with IV tenecteplase before endovascular thrombectomy (EVT) influences the probability of first-pass reperfusion when compared with IV alteplase. Therefore, we evaluated the effect of IV thrombolytic choice on this outcome. MATERIALS AND METHODS: We investigated the effects of thrombolytic agent (IV tenecteplase or IV alteplase) on first-pass reperfusion in EVT patients from the Alteplase Compared to Tenecteplase in Patients with Acute Ischemic Stroke (AcT) trial. The primary outcome was first-pass reperfusion, defined as expanded Thr

Clinical outcomes of venous tumor thrombectomy in renal cell carcinoma.

OBJECTIVE: Renal cell carcinoma (RCC) has a unique feature in which tumor thrombus extends through the vein into the inferior vena cava. RCC with venous tumor thrombus is classified based on the level of thrombus. Standard treatment involves a combination of radical nephrectomy and tumor thrombectomy, despite the technical challenges associated with this approach. We aimed to evaluate the clinical outcomes based on the tumor thrombus level. METHODS: We conducted a retrospective analysis of patients who underwent tumor resection and thrombectomy for RCC with venous tumor thrombus at a single c

Endovascular thrombectomy in young patients with acute vertebrobasilar artery occlusion.

BACKGROUND AND PURPOSE: Data on endovascular thrombectomy (EVT) for acute vertebrobasilar artery occlusion (VBAO) in young adults are limited. We compared clinical characteristics and outcomes after EVT between young and older patients. METHODS: Using a multicenter retrospective registry, we analyzed patients undergoing EVT for acute VBAO. Patients were dichotomized by age (18-44 years vs ≥ 45 years). Primary outcomes were functional independence (modified Rankin Scale score, mRS 0-2) at 90 days and 1 year. Secondary outcomes included successful reperfusion, 24-hour/discharge National Institu

Microsurgical thrombectomy: where the ancient art meets the new era.

Mechanical thrombectomy (MT) is the leading treatment for acute large vessel occlusion (LVO). However, surgical thrombectomy (ST) may have a role in well selected LVO patients where MT failed to re-establish flow, the endovascular route is inaccessible, or where MT is a financially prohibitive or absent option (developing and poor countries). We compared the efficacy and efficiency between ST and MT, and described our operative experience and its potential application in the developing world. Clinical outcomes, procedural times, and efficacy of treatment were compared between the MT and ST of

Incidence of Orbital Infarction Syndrome Following Anterior Circulation Endovascular Thrombectomy.

BACKGROUND: Orbital infarction syndrome (OIS) is a rare entity defined as acute ischemia of intraorbital structures. Its occurrence requires interruption of the rich collateral supply to the orbit from both the ophthalmic artery (OA) and branches of the external carotid artery. Three case reports have recently been published of OIS occurring after endovascular thrombectomy (EVT). The goal of this study was to determine the true incidence of post-EVT OIS and to identify associated imaging findings on computed tomography angiography and digital subtraction angiography (DSA). METHODS: A retrospe
