[Reconstructive laser surgical treatment for posttraumatic traction ciliochoroidal detachment].

To treat patients with eye injuries remains a topical problem due to the high incidence of this type of pathology and the severity of clinical outcomes. Ciliochoroidal detachment (CCD) occurring in 1-9% of the victims is one of the most common complications of mechanical eye injury. The purpose of the study was to evaluate the efficiency of pathogenetically substantiated technology for a combined laser surgical reparative intervention in posttraumatic traction CCD. The results of treatment were analyzed in 70 patients with posttraumatic traction detachment of the choroid and ciliary body. Ther

Vitreopapillary traction: cost-effective diagnosis by optical coherence tomography.

CASE REPORTS: A prospective, noncomparative, observational case series. Three patients, aged 76 to 81, all referred to subspecialty services for evaluation of optic disc elevation, and all were found to have optic disc (vitreopapillary) traction as verified by optical coherence tomography. COMMENTS: Vitreopapillary traction is a recently recognized syndrome characterized by incomplete posterior vitreous detachment. The use of optical coherence tomography is helpful in the diagnosis of this syndrome, preventing many costly, unwarranted evaluations.

Tomographic features and surgical outcomes of vitreomacular traction syndrome.

PURPOSE: To study the relation between preoperative macular changes and surgical outcomes in vitreomacular traction syndrome. DESIGN: Prospective study. METHODS: We prospectively examined 14 eyes of 13 patients (aged 48 to 82 years; mean 66.1) with vitreomacular traction syndrome using optical coherence tomography (OCT) before and after vitreous surgery. RESULTS: OCT demonstrated two types of partial posterior vitreous detachment: incomplete V-shaped detachment in 10 eyes (group 1) and partial detachment temporal to the fovea but attached nasally in 4 eyes (group 2). Preoperative OCT showed

Deep skin slough following skin traction for hip fractures.

INTRODUCTION: Elderly patients who fracture their hips are susceptible to complications associated with bed rest before surgery. In some institutions a skin traction device is used in order to relieve pain before surgery. The authors followed ten patients who developed severe (deep) skin slough and evaluated the relationship to the skin traction device. The need for use of a skin traction device is debated and the need for multidisciplinary treatment if this complication develops is emphasised. PATIENTS AND METHODS: All patients who had serious skin slough between January and December 2000 we

Management of traction wire fracture complicating mechanical basket lithotripsy.

BACKGROUND AND STUDY AIMS: The application of basket catheters has become the main method of removing calculi from the biliary and pancreatic duct. However, larger or impacted stones have to be crushed and fragmented by mechanical lithotriptors before removal is possible. Sometimes, fracture of the traction wire occurs as a severe and fraught complication. We describe a precautionary measure which helps to manage this complication. PATIENTS AND METHODS: In a series of 569 consecutive patients suffering from bile or pancreatic duct stones we found 60 (10.5%) who required mechanical lithotripsy

Vitreomacular traction syndrome confused with pseudophakic cystoid macular edema.

Vitreomacular traction syndrome causes visual loss as a consequence of persistent traction on the fovea from an attached posterior hyaloid. The diagnosis can be challenging because the attachment may be subtle. Cystoid macular edema secondary to cataract extraction is well known and can occur in the absence of any surgical complication. Herein, a patient is described who had vitreomacular traction syndrome diagnosed after uneventful cataract surgery. Pars plana vitrectomy with detachment of the posterior hyaloid restored vision to an acceptable level. Careful examination and clinical suspicion

"Chignon alopecia": a distinctive type of nonmarginal traction alopecia.

Localized occipital alopecia may result from traction of the hair necessary to form and maintain a bun (chignon) in the occipital region. Marginal alopecia in the patient whose hair is drawn firmly back is more familiar, although traction may lead to loss of hair at any site of the scalp depending on the hairdressing techniques used. Three women were seen with localized alopecia of the occipital scalp associated with the wearing of a chignon. They had previously been uniformly misdiagnosed as having alopecia areata. "Chignon alopecia" should be ruled out when the diagnosis of persistent alopec

Zygomatico-maxillary fractures. Late results after traction-hook reduction.

A simple, closed method for zygomatico-maxillary fracture treatment is described i.e. replacement by the use of a traction bone-hook. After X-ray and clinical examination had excluded undisplaced fractures, blow out fractures and comminuted fractures, the postreduction stability was assessed peroperatively and again one week after. Sophisticated preoperative fracture-classifications predicting postoperative stability appeared redundant. In the present study, this procedure could be performed in 55.4% of the total fractures which needed to be reduced. At re-evaluation, two to eight years later,

Effect of silicone oil on experimental traction retinal detachment.

The use of silicone oil as a retinal tamponade in the treatment of proliferative vitreoretinopathy is often complicated by membrane reproliferation. We studied the development of traction retinal detachments following the intravitreal injection of 15,000 retinal pigment epithelial cells in silicone-, perfluoropropane-, and fluid-filled rabbit eyes. By 28 days a higher proportion of silicone-filled eyes (83%) had severe proliferative vitreoretinopathy than either the perfluoropropane-filled (30%) or fluid-filled (10%) eyes. An in vitro proliferation assay using the vitreous samples showed that

Prognosis of traction retinal detachment in proliferative diabetic retiopathy.

Considering the complications which may arise when traction retinal detachment (TRD) is treated surgically, the correct assessment of its course is important. The evolution of TRD in proliferative diabetic retinopathy is described and its pathogenesis discussed. In order to predict the course of a specific clinical situation we have to assess the activity of retinopathy and the localization and extension of vitreoretinal adhesions. Some guide lines for proper evaluation and proper management of the often complicated clinical picture of TRD in proliferative diabetic retinopathy are presented.

Genu recurvatum: a complication of prolonged femoral skeletal traction.

Genu recurvatum is a rare complication of prolonged skeletal traction. The literature reports very few cases, the majority secondary to wire traction applied to the tibia in the treatment of femoral shaft fractures. A case of genu recurvatum in a 12-year-old girl as a complication of prolonged femoral skeletal traction was treated at the Kobe University Hospital. A proximal open wedge tibial osteotomy using iliac bone grafts was the surgical treatment initiated at the time of diagnosis. Follow-up after 1 year showed recurrence of the genu recurvatum. Early surgical intervention, prolonged cast

Treatment effects of Fränkel, activator and extraoral traction appliances.

The effects of the Fränkel appliance on the treatment of Class II, Division 1 malocclusion were compared with the activator, high-pull headgear, cervical traction and a control group. A sample of 16 Fränkel cases treated in the mixed dentition was tested in four craniofacial areas, with findings as follows: a. Maxillae, no significant horizontal effect b. Mandibular length, significantly increased c. Lower face height, widely variable effects. d. Less upper molar eruption and more lower molar eruption. Although the findings of this study should be interpreted as tentative, they do show the Frä

Vitrectomy for diabetic traction retinal detachment involving the macula.

We analyzed the surgical results for 197 eyes that underwent vitrectomy for diabetic traction detachment of the macula. The retina was successfully reattached in 130 of the 197 cases (66%), although the final level of vision depended on the amount of permanent macular damage from the previous detachment and on the effects of the retinal vascular disease. At the final examinations, 112 eyes (57%) had improved visual acuities, 68 eyes (35%) were worse, and 17 eyes (9%) were unchanged. Thirteen eyes (7%) had final visual acuities of 20/40 or better, 103 eyes (52%) had final visual acuities of 20/

Spontaneous release of retinal traction due to subretinal strands.

Spontaneous release of a taut, fibrotic subretinal strand was observed in a patient with rhegmatogenous retinal detachment. This strand had been elevating the retina, and its release led to partial reattachment. Whereas spontaneous resolution of preretinal membrane contraction in retinal detachment has been reported, this is the first report of spontaneous release of such a subretinal fibrotic strand. The manner in which the strand released raised the question of whether efforts should be made to rupture such strands with transcleral pressure prior to considering retinotomy.

The natural history of diabetic extramacular traction retinal detachment.

There is a difference of opinion concerning the advisability of surgery for extramacular fraction retinal detachment (EMTRD) in diabetic patients before development of macular detachment. Ninety-nine eyes with EMTRD of 84 diabetic patients observed without surgery had a detachment rate of 13.8% in one year. This rate compares favorably with the reported 30% to 47% failure rate for diabetic eyes subjected to vitrectomy.

Clinical results in vitrectomy for diabetic traction retinal detachment.

A series of 75 eyes underwent pars plana vitrectomy for progressive diabetic traction retinal detachment of the macula. In all cases, the media was sufficiently clear to allow preoperative macular examination. The overall visual improvement was higher in phakic eyes (80%) than aphakic eyes (72%) and the rate of neovascular glaucoma was less in phakic eyes. The difference was not statistically significant, however. The final visual acuity corresponded to the degree of preoperative visual deterioration. Thus, surgery during the rapidly evolving proliferative and retracting process did not advers
