The authors have studied 53 patients with essential tremor, focusing its clinical and epidemiological aspects. There were familial history in 37.70% of all cases, prevailing in females (56.60%) and white people (69.80%); nevertheless this difference can not be assured from the statistical point of view due to lack of population data. We agreed that the main incidence of this syndrome occurred beyond the 5th decade, specially during the 6th and 7th decades. Topographically, we could observe that the hand tremor predominanted, with an incidence of 96.20% of the total number of cases, followed by…

We reviewed the database of the Dystonia Clinic at the University of Kansas Medical Center for patients with dystonia and tremor. Of 296 patients with idiopathic dystonia, 24 had dystonic tremor, 20 with cervical dystonia had an isolated head-nodding tremor, two patients with writer's cramp had ipsilateral hand tremor, and two patients with generalized dystonia had arm tremor. Eight patients, all with cervical dystonia, had essential tremor that preceded the onset of their dystonia.

We report on four patients with unilateral tremor stemming from cerebrovascular accidents. In two patients with proven lesions of the thalamus, the tremor was irregular and, in addition, there was dystonic posturing of the affected arm. Tremor and dystonic posturing had appeared after the stroke. In the other two patients tremor had occurred immediately at the onset of the stroke and lasted only a few days. The tremors were of small amplitude and high frequency, and lesions could not be found on CT or MRI in these two patients. None of our patients showed signs of the so-called rubral tremor.

Classification of essential tremor was attempted using tremor frequency; tremor duration; family history of tremor; responsiveness to alcohol, propranolol, and primidone; muscle contraction pattern; and long-latency reflexes. Sixty-one patients were evaluated. The majority of patients had a tremor frequency less than 7.0 Hz, a positive family history, and a positive response to alcohol. Approximately 46% of patients had a beneficial response with propranolol and 71% with primidone. Tremor frequency was inversely correlated with age and directly correlated with an antagonist pattern of muscle c…

A review of current knowledge of essential tremor and its treatment is presented. Its prevalence in Africa is not known but it is quite common in those over 40 years in most communities of the world. The condition is characterised by slowly progressive postural tremor of the arms and head without an identifiable organic cause. It is often misdiagnosed as Parkinson's disease. Current treatment is with beta-adrenoceptor blocking drugs, but this treatment is not completely satisfactory. Further studies are required on its pathophysiological mechanisms and new therapeutic directions.

We studied physiologic and essential hand tremor using inertial loading; hand acceleration and forearm EMG data were analyzed by auto- and cross-spectral analysis. Early essential tremor was qualitatively similar to the 8- to 12-Hz component of physiologic tremor, suggesting that this tremor component is a forme fruste of essential tremor. Advanced essential tremor had a frequency of 4 to 8 Hz. Patients with tremor frequencies in both ranges were observed in each of 10 families. In antagonistic forearm muscles, both synchronous and alternating tremor bursts were observed in 11 of 44 patients. …

We examined 100 alcoholics who had had no alcohol for more than 21 days, 100 controls, and 50 patients with essential tremor. Three percent of the controls and 47% of the alcoholics had a postural tremor. Alcoholic tremor was never severe, and functional disability occurred in only 17% of patients. There was no relation to age or duration of drinking, and only 1% of the alcoholics had a family history of tremor compared with 46% in essential tremor. Tremor frequency was significantly greater in the alcoholics than in essential tremor. Propranolol therapy decreased tremor more in the alcoholics…

In a prospective study, the prevalence of pathologic drinking in essential tremor did not differ significantly from other tremor disorders or chronic neurologic diseases without tremor. Parkinsonian patients had a significantly lower prevalence of alcohol use. These data suggest that patients with essential tremor are not at higher risk of chronic alcoholism than other patients with neurologic disease.
