Trigger zones in trigeminal neuralgia: clinical features, pathophysiological mechanisms, and therapeutic strategies.

Trigeminal neuralgia (TN) is a neuropathic pain disorder characterized by recurrent, unilateral, electric shock-like facial pain, with trigger zones representing one of its most distinctive clinical features. These localized hypersensitive regions can be reliably activated by innocuous mechanical stimuli such as light touch, speech, or chewing, and their distribution provides important guidance for localizing the affected trigeminal branch and directing treatment decisions. Current evidence suggests that trigger zone formation involves peripheral nerve hyperexcitability driven by upregulation

Fu's subcutaneous needling for intractable hiccups: A case report.

RATIONALE: Intractable hiccups pose a significant impact on patients' quality of life and mental well-being, yet effective nonpharmacological interventions remain limited. Fu's subcutaneous needling (FSN), a novel acupuncture technique targeting myofascial trigger points (MTrPs), has not been previously reported in treating intractable hiccups. PATIENT CONCERNS: The patient developed intractable hiccups (30-40 episodes/min) following treatment for allergic cough, unresponsive to medications and conventional acupuncture. DIAGNOSES: Intractable hiccups. INTERVENTIONS: FSN was applied to MTrPs…

Management strategies for anterior cutaneous nerve entrapment syndrome: a scoping review.

PURPOSE: Chronic Abdominal Wall Pain (CAWP) covers a broad spectrum of abdominal wall-related disorders, with Anterior Cutaneous Nerve Entrapment Syndrome (ACNES) being the most prevalent. Treatment strategies in published literature are limited and often not clearly outlined. This scoping review aims to outline current available literature about the management of ACNES, focusing on trigger point injections (TPI), pulsed radiofrequency (PRF) and surgical neurectomy. METHODS: A systematic search in PubMed and EMBASE was performed until December 2024, in compliance with PRISMA guidelines. Artic

The Neurophysiological Effects of Dry Needling: An Update of a Narrative Review.

The interest in dry needling as a treatment option for myofascial pain has flourished for the last decades and will probably continue to do so, because multiple clinical effects can be attributed to this technique. However, evidence about the underlying physiological mechanisms of its effects is still underrepresented in scientific research and caution must be taken in generalizing results from acupuncture or animal research. This review offers an overview of the possible mechanisms involved in the pathophysiology of myofascial trigger points, that is, the formation of the taut band, the prese…

Neural Interconnections of Extracranial Nerves in Headache Surgery: Anatomical Landmarks and Clinical Implications: A Review of the Literature.

Headache disorders (HDs) remain a nationwide challenge for the US health care system, affecting nearly 60% of the US population. Surgical deactivation of the peripheral trigger site, also referred to as headache surgery, represents an effective treatment for patients with refractory HD pain or nonresponse to pharmacologic regimens. Research stemming from other surgical specialties has underlined the clinical relevance of neural interconnections in refining diagnostic algorithms, adapting surgical techniques, and improving overall patient outcomes. Different HD trigger points have been identifi…

Influence of clinical experience on accuracy and safety of obliquus capitus inferior dry needling in unembalmed cadavers.

BACKGROUND: Suboccipital myofascial trigger points are common in tension-type headaches. OBJECTIVES: Compare the influence of clinical experience on the accuracy and safety of dry needle placement on the C2 laminar arch using a cranial-medial and caudal-medial technique to target obliquus capitus inferior in unembalmed cadavers. METHODS: Three physical therapists inserted three 50 mm dry needles, per technique, individually toward the C2 laminar arch targeting the obliquus capitus inferior. Ultrasound video of each trial was recorded, and an investigator trained in ultrasound interpretation …

Diagnosis and management of somatosensory tinnitus: review article.

Tinnitus is the perception of sound in the absence of an acoustic external stimulus. It affects 10-17% of the world's population and it a complex symptom with multiple causes, which is influenced by pathways other than the auditory one. Recently, it has been observed that tinnitus may be provoked or modulated by stimulation arising from the somatosensorial system, as well as from the somatomotor and visual-motor systems. This specific subgroup -somatosensory tinnitus - is present in 65% of cases, even though it tends to be underdiagnosed. As a consequence, it is necessary to establish evaluati

Chronic Pain Syndromes: Myofascial Pain Syndrome.

Myofascial pain syndrome (MPS) is a regional musculoskeletal pain disorder characterized by trigger points within the muscle or fascia. There are no universally accepted diagnostic criteria. Diagnosis currently is based on a physical examination finding of at least one localized trigger point that, when palpated, recreates the pain at the site or produces pain away from the site in a referral pattern. MPS is thought to be related to sustained muscle contraction from under- or overuse. This syndrome commonly coexists with other chronic pain conditions, including fibromyalgia. The difference bet…

The Role of Manual Therapies in the Treatment of Headache Disorders.

PURPOSE OF REVIEW: A comprehensive headache treatment plan typically requires both medication and non-medication treatment strategies. Manual therapies offer another therapeutic approach to headache treatment. This article reviews the evidence for manual therapies in the treatment of headache disorders. RECENT FINDINGS: Current evidence shows potential benefit from myofascial trigger point injections, myofascial release, and massage for the treatment of various headache types. There is also evidence for strain counterstrain technique, ischemic compression, and spinal manipulative therapies fo

From Histoanatomy to Sonography in Myofascial Pain Syndrome: A EURO-MUSCULUS/USPRM Approach.

Myofascial pain is a common clinical condition, whereby accurate physical examination is usually considered as the cornerstone to identify/diagnose the "trigger point complex," that is, the characteristic finding of this syndrome. Considering the emerging role of ultrasound examination as the natural extension of physical assessment for musculoskeletal disorders, we briefly revise the histological/anatomical features of trigger points and propose a standardized, multistep sonographic approach to myofascial pain. We also imply that the integrated clinical-ultrasound evaluation could be consider…

Scoping review and interpretation of myofascial pain/fibromyalgia syndrome: An attempt to assemble a medical puzzle.

BACKGROUND: Myofascial Pain Syndrome (MPS) is a common, overlooked, and underdiagnosed condition and has significant burden. MPS is often di … d. Fascia can adapt to various states by reversibly changing biomechanical and physical properties. Trigger points, tension, and pain are a hallmark of MPS. Myofibroblasts play a role in sustained myofascial tension. Tension can propagate in fascia, possibly supporting a tensegrity framework. Movement and mechanical interventions treat and prevent MPS, while liv …

Impact of physical therapists' instructions on function and the perception of post-dry needling soreness in mechanical cervical pain; a randomized clinical trial.

BACKGROUND: To investigate the impact of physical therapists' instructions on the perception of post dry needling (DN) soreness and function in patients with mechanical neck pain. METHODS: Seventy-five patients with neck pain were randomly assigned to three groups: "positive" group (n = 25) received positive verbal input; "negative" group (n = 25) received negative verbal input, and control group (n = 25) did not receive any input about post-needling soreness. All three groups received DN of a trigger point in the upper trapezius muscle. The subjective pain experience, pressure pain threshold

New perspectives on dry needling following a medical model: are we screening our patients sufficiently?

Myofascial trigger points are not an isolated neuromusculoskeletal phenomenon and have been implicated in systemic, visceral, and metabolic pathology, as a side effect of some medications and in the presence of psychological risk factors. This complexity can complicate adequate screening of patients prior to choosing dry needling as a treatment intervention. Regardless of whether clinicians practice in a direct access setting, they should be cognizant of medical conditions, comorbidities, and risk factors that will influence clinical decisions for dry-needling appropriateness, technique chosen…
