What's new in MRI of peripheral nerve entrapment?

New developments in clinical peripheral nerve imaging with MRI over the past few years, primarily those related to nerve entrapment syndromes, are reviewed. The basic principles of peripheral nerve imaging are described briefly. Relevant current or forthcoming technical innovations are described, and then recent work describing novel findings, organized by anatomic location (brachial plexus, upper extremity, and lower extremity), is reviewed. The review concludes with a summary and suggestions of areas in which future clinical research would be particularly helpful.

The cubital tunnel syndrome caused by intraneural ganglion cyst of the ulnar nerve at the elbow: a case report.

BACKGROUND: Cubital tunnel syndrome is common nerve compression syndrome among peripheral nerve compression diseases. However, the syndrome caused by intraneural ganglion cysts has been rarely reported. Medical approaches, like ultrasound-guided aspiration and open surgical treatment remain to be discussed. CASE PRESENTATION: A 57-year-old woman presented with occasional pain, numbness and paralysis in her left hand and a palpable, painless mass in the ulnar side of her left elbow. Ultrasound-guided aspiration of the mass was performed to decompress the ulnar nerve. The patient experienced an

Mid-motion deformation of median nerve during finger flexion: a new insight into the dynamic aetiology of carpal tunnel syndrome.

Carpal tunnel syndrome (CTS) exists in a spectrum of severity and symptoms with a dynamic component. We aim to study dynamic nerve-tendon interrelationships in normal and mild CTS wrists during a fist motion, with dynamic ultrasound. We observed that in normal wrists, the nerve arcs in an ulnar-volar direction and changes from a circular shape to a flat oval during motion. In CTS candidates, however, the curvature and distance of the nerve's path are reduced, while nerve shape remains relatively constant. In all candidates, the nerve is compressed against the flexor retinaculum, with the nerve

Adverse effects of GH self administration on peripheral nerve. A case report.

To our knowledge there is no evidence of multientrapment neuropathy related to growth hormone (GH) administration. We observed the case of a 45-year-old bodybuilder complaining of paresthesias and dysesthesias of the hands and feet after self-administration of high-dose of recombinant GH therapy. The patient complained of transitory symptoms after each cycle of GH therapy. The electrodiagnosis showed: 1) moderate right carpal tunnel syndrome (CTS) and minimal left CTS; 2) mild right ulnar entrapment at elbow; 3) a reduction of motor nerve conduction velocity of the left ulnar nerve across elbo

Quantification of In Situ Ulnar Nerve Strain and Constraint Tension in the Forearm.

BACKGROUND: Surgical interventions for ulnar neuropathy are typically identified as releases or decompressions. With the expectation that ulnar nerve strain contributes to cubital tunnel syndrome and with the basic hypothesis that nerve tension will most significantly increase in positions of highest elbow flexion, wrist extension, and classic sites of compression, we initiated a new experimental technique to quantify strain and tension in the ulnar nerve. METHODS: In five fresh-frozen cadaveric upper extremities from the spine to the wrist, we percutaneously placed small radiopaque spheres i

Tremor Induced by Focal Peripheral Nerve Entrapment: A Case Series.

Little is known about tremors caused by peripheral nerve entrapment. We report two cases of tremors caused by peripheral nerve compressions. Two patients presented with intentional tremors combined with peripheral nerve compression symptoms on their affected hand. Based on the clinical findings and evaluations, the first patient was diagnosed with double-crush compression of the ulnar nerve at the cubital tunnel and Guyon canal, and the second patient was diagnosed with lacertus syndrome. The first patient underwent surgical release of the cubital tunnel and Guyon canal in two stages. The seco

Ulnar nerve entrapment complicating radial head excision.

INTRODUCTION: Several mechanisms are involved in ischemia or mechanical compression of ulnar nerve at the elbow. PRESENTATION OF CASE: We hereby present the case of a road accident victim, who received a radial head excision for an isolated fracture of the radial head and complicated by onset of cubital tunnel syndrome. This outcome could be the consequence of an iatrogenic valgus of the elbow due to excision of the radial head. Hitherto the surgical treatment of choice it is gradually been abandoned due to development of radial head implant arthroplasty. However, this management option is st

Management of Recalcitrant Cubital Tunnel Syndrome.

Cubital tunnel syndrome is a common upper extremity compressive neuropathy. Recalcitrant cubital tunnel syndrome poses diagnostic and treatment challenges. Potential etiologies of persistent or recurrent symptoms after surgical treatment include an inaccurate preoperative diagnosis, incomplete nerve decompression, iatrogenic injury, postsurgical perineural adhesions, irreversible nerve pathology, or conditions associated with secondary nerve compression. Confirmation of recalcitrant ulnar nerve pathology relies on a thorough history to consider symptoms and chronology, careful examination to q

Early surgical management for heterotopic ossification about the elbow presenting as limited range of motion associated with ulnar neuropathy.

BACKGROUND: The formation of heterotopic ossification (HO) about the elbow after traumatic injury has been well documented in the literature. The optimal treatment, however, for ectopic bone associated with restricted range of motion and ulnar nerve entrapment syndrome has not been established. METHODS: Seven elbows with HO in 7 patients admitted to Chang Gung Memorial Hospital from April 1998 to January 1999 presented with limited range of motion and associated ulnar nerve neuropathy. All of these patients received early surgical excision of HO combined with release of the encased ulnar nerv

Tardy ulnar nerve palsy caused by cubitus varus deformity.

Fifteen patients with tardy ulnar nerve palsy caused by cubitus varus deformity were studied. All patients had a history of previous fracture of the humerus during childhood. The mean interval between fracture and onset of symptoms was 15 years. The severity of the palsy was classified as McGowan's grade I in 12 patients, grade II in 2 patients, and grade III in 1 patient. The mean carrying angle was -2 degrees before surgery. X-ray films showed a shallow ulnar nerve groove, a dysplastic humeral trochlea, medial shift of the ulna, and deformity of the medial epicondyle. The ulnar nerve was exp

Is the anconeus epitrochlearis muscle a predictor for ulnar nerve compression?

BACKGROUND: The role of the anconeus epitrochlearis (AE) in cubital tunnel syndrome, either as protector or potential compressor of the ulnar nerve, as well as its prevalence in both symptomatic and asymptomatic patients is still unclear. This study aimed to assess the prevalence of the AE in a large cohort using 3-dimensional imaging and to investigate any association of the AE with preoperative or postoperative features of patients undergoing cubital tunnel surgery. METHODS: From a database of 1240 elbow magnetic resonance imagings, all patients with an AE were retrospectively screened for

Neuropathic arthropathy of the elbow. A report of five cases.

BACKGROUND: Neuropathic arthropathy of the elbow is rare and characterized by a painless but unstable articulation. The functional capacity of patients with this condition has not been reviewed in detail. METHODS: Five male patients, with an average age of fifty-one years, were treated for neuropathic arthropathy of the elbow. The underlying conditions associated with the arthropathy included syringomyelia, insulin-dependent diabetes mellitus, end-stage renal failure, and two cases of polyneuropathy of unknown cause. Four patients sought medical attention after a specific traumatic event. Per

Compression of the ulnar nerve at the elbow in association with synovial cysts.

Compression neuropathies around joints in association with synovial cyst formation are rare events. A case of a posttraumatic ulnar nerve palsy in a patient with osteoarthritis of the elbow is presented. At operative exploration of the cubital tunnel, the ulnar nerve was found to be compressed by a synovial cyst. Cyst formation should be considered in determining the etiology of compression neuropathies in patients with posttraumatic and degenerative processes occurring around the joints.

The hand diagram: A novel outcome measure following supercharged end-to-side anterior interosseous nerve to ulnar nerve transfer in severe compressive ulnar neuropathy.

BACKGROUND: With advances in the surgical management for severe ulnar neuropathy with the introduction of the super charged-end-to-side (SETS) anterior interosseous nerve (AIN) to ulnar nerve transfer, a simple and reliable outcome measure is required. There is currently not "one" standardized outcome measure used to represent and compare results. PURPOSE: To present the abduction hand diagram as a "novel", reproducible, and simple outcome measure for patients with severe ulnar neuropathy. STUDY DESIGN: Retrospective case series. METHODS: Nine patients with severe entrapment/compressive uln

Survey of hand symptoms in pregnancy.

In a study of 1216 pregnancies, 427 (35%) patients reported hand symptoms. Symptoms of the same quality and distribution were reported by 40 (30%) of 132 control subjects within the previous year, and although invariably mild, these symptoms suggest that pregnancy may aggravate a pre-existing condition. Fewer than 20% of the 427 affected patients described a classic median-nerve symptom distribution (carpal tunnel syndrome), while 12% of patients described an ulnar-nerve distribution, which is thought to represent a genuine and previously underestimated occurrence of ulnar-nerve neuropathy in

Effective surgical treatment of cubital tunnel syndrome based on provocative clinical testing without electrodiagnostics.

This study evaluated the effectiveness of identifying surgically correctable ulnar nerve compression at the elbow based on provocative clinical testing alone in patients with cubital tunnel syndrome after failure of conservative treatment. Twenty-four patients were included in the study (mean age, 60 years). Three patients underwent bilateral procedures. Patients complaining of symptoms in the distribution of the ulnar nerve were tested by elicitation of Tinel's sign and combined flexion and pressure testing at the elbow and wrist. Two-point discrimination was determined. After a failed 6-week

The Surgical Approach to a Medial Epicondylectomy in Cubital Tunnel Syndrome.

Cubital tunnel syndrome, a common neuropathy of the upper limb, presents with sensory and motor symptoms, impacting hand function and grip strength. Surgical interventions, such as medial epicondylectomy (ME), aim to alleviate symptoms by relieving pressure on the ulnar nerve at the elbow. Historical attempts to treat this condition date back to the early 19th century, with procedures evolving over time. Modern techniques, including in situ decompression, transposition, and various forms of epicondylectomy, offer different approaches to address nerve compression. This article examined the evol

Ulnar bursa distention following volar subluxation of the distal radioulnar joint after distal radial fracture: a rare cause of carpal tunnel syndrome.

This report describes an eighty-four-year-old woman with persistent carpal tunnel syndrome attributable to an ulnar bursa distention associated with the subluxation of the distal radioulnar joint after distal radial fracture. During surgery, when the forearm was placed in supination, the ulna head with a sharp osteophyte was found to be displaced into the carpal tunnel through a defect of the ruptured capsule of the wrist joint. This volar subluxation of the ulnar head had caused distention of the ulnar bursa, causing compression of the median nerve, which resulted in carpal tunnel syndrome. I

Delayed Formation of Traumatic Ulnar Artery Pseudoaneurysm Presenting With Ulnar Nerve Palsy: A Case Report.

Ulnar artery pseudoaneurysm (UAP) is a rare occurrence after penetrating injury to the distal upper extremity and may lead to complications such as rupture, sensorimotor dysfunction, and compartment syndrome. We present the case of a 57-year-old man who developed delayed UAP after suffering a penetrating injury to the right forearm. UAP was diagnosed a week after the injury using CT angiography (CTA), which was indicated due to the onset of ulnar nerve palsy (mildly reduced hand-grip strength and fourth- and fifth-digit hypoesthesia and numbness) and growing swelling and tenderness of the righ
