[Epidemiological aspects of the female urinary incontinence].

Urinary incontinence is defined as an involuntary loss of urine, which makes social and hygienic problem. It is a symptom with different causes. According to the typical clinical manifestation it is classified as stress, urge, reflex and paradox urinary incontinence. Loss of small amount of urine related to the increase of intraabdominal pressure (during coughing, sneezing or running) is characteristic for stress urinary incontinence. Sudden and uncontrollable voiding with loss of greater amount of urine is typical for urge incontinence. Reflex incontinence means that urinary bladder is emptyi…

Stress urinary incontinence in the gynecological practice.

This review discusses the prevalence, risk factors, the impact on the quality of life and healthcare-seeking behavior of women suffering from urinary incontinence (UI) and stress urinary incontinence (SUI) in particular. UI is a common problem, affecting women in all age groups, and has devastating effects on their social, professional and family life. UI may be manifest as stress urinary continence (SUI), i.e. 'the complaint of involuntary leakage on effort or exertion, or on sneezing or coughing'. SUI is the most common form of UI, reported by approximately 50% of the incontinent women. Alte…

Surgical treatment of urinary incontinence in women.

Urinary incontinence affects many Americans, predominantly women. The majority of health care costs are spent on absorbent products, and diagnosis and treatment consume significant additional resources. Many factors interact with urinary incontinence-including medications, health conditions, and other pelvic floor disorders. There has been no meaningful research on prevention of urinary incontinence. The majority of research has focused on treatment, often with medical approaches or surgery. Surgery for urinary incontinence is largely limited to treatment of stress urinary incontinence. Nearly…

[Urinary incontinence in the man--pathophysiology and diagnosis].

About 7-8% of men experience unvoluntary urinary leak. Incidence in patients older than 60 years is 2-3 fold increased. Urge incontinence is the mostly present, less frequent are overflow-incontinence/chronic retention and urinary stress incontinence. In men, prostate and bladder dysfunction as well as neurologic diseases are responsible for urinary incontinence. The baseline diagnostics include micturition protocols, the urologic clinical examination, residual urine determination and laboratory analyses of urine and blood. Extended diagnostics proceed to morphological and infectious etiologie…

[Urinary and anal incontinence: a taboo subject].

UNLABELLED: Lamotrigine (Lamictal), a phenyltriazine derivative, is a well established anticonvulsant agent that has shown efficacy in the prevention of mood episodes in adult patients with bipolar I disorder. The mechanism of action of the drug in patients with bipolar disorder may be related to the inhibition of sodium and calcium channels in presynaptic neurons and subsequent stabilisation of the neuronal membrane. Lamotrigine monotherapy significantly delayed time to intervention with additional pharmacotherapy or electroconvulsive therapy for any new mood episode (mania, hypomania, depres

Urinary incontinence. No need to be wet and upset.

Over 17 million people in the United States suffer from urinary incontinence. UI limits functional and social activities and is a common cause of anxiety, social withdrawal, and depression. Primary care providers should take a proactive approach in searching for the presence of, and then investigating the reasons for, incontinence. Usually the diagnosis is apparent from the data obtained from a good history, physical examination, measurement of post-voiding residual urine, and urinalysis. A voiding diary can quantify the magnitude of the problem and response to treatment. Conservative treatmen…

Coping strategies for women with urinary incontinence.

Incontinence is a common and complex condition affecting women of all ages, social and cultural backgrounds. 'Coping strategies' play an important role for managing incontinence and include changes in lifestyle, protection, aids, and catheters, whether they are indwelling or intermittent. By the time women seek medical advice for urinary incontinence they have usually tried various tactics to either limit the leakage or protect themselves from it. The most common tactics to limit leakage include fluid restriction, regular toileting, exercises and weight reduction. With medical advice, women ca…

Bladder training for urinary incontinence in adults.

BACKGROUND: Bladder training is widely used for the treatment of urinary incontinence. It is generally used for the treatment of people with urge incontinence or detrusor instability, although it is also thought that it might be of use for people with mixed incontinence or stress incontinence. OBJECTIVES: To assess the effects of bladder training for the treatment of urinary incontinence. SEARCH STRATEGY: We searched the Cochrane Incontinence Group trials register up to July 1999. Date of the most recent search: July 1999. SELECTION CRITERIA: Randomised or quasi-randomised trials of bladder…

Historical aspects of the treatment of urinary incontinence.

Rabies virus (RABV) is a neglected tropical pathogen in Latin America predominantly transmitted to mammals by the common vampire bat (Desmodus rotundus). Transmission of RABV among D. rotundus individuals and colonies is a function of individual dispersal between colonies, patterns of which can be inferred from population genetic structure. Nevertheless, a baseline assessment of population genetic structure among D. rotundus individuals has been lacking for some areas of South America, including Colombia, where RABV has impacted some areas more heavily than others. To assess individual dispers

A unique cause of unconscious urinary incontinence.

The authors present a case of a healthy 55-year-old woman with a 1-year history of vaginal malodor and perineal dampness. A pyridium (phenazopyridine) pad test revealed a well-defined stain area no greater than 3 cm. Cystoscopic examination revealed multiple multilobulated polypoid neoplasms, which we feel probably represent pedunculated hemangiomas. Removal of the neoplasms cured the dampness, malodor and recurrent vaginal infections. We submit this case as a unique cause of extraurethral incontinence.

Periurethral injection for the treatment of urinary incontinence.

Periurethral injection of substrates that compress, support, or narrow the bladder neck for the treatment of stress incontinence is not new. Several injectable compounds have been used in a small number of patients during the 1940s and through the 1960s; however, the results were not very successful and often led to significant complications. More recently, two major materials have been developed that seem to be useful in treating stress incontinence by periurethral injection: Polytef paste and GAX collagen. Other injectables include autologous fat tissue and silicone microimplants. The most s

[Treatment of cystoceles with urinary stress incontinence].

Thirteen women were operated for cystocele associated with urinary stress incontinence. Cystoceles were grade I in 6 cases, grade II in 6 cases and grade III in 1 case. Five patients underwent retropubic colposuspension according to Burch (3 cases) and M.M.K. (2 cases). Eight patients were treated by RAZ's technique associated with anterior colporaphy in order to reduce the cystocele. In this latter group, only one recurrence was observed and was treated by repeat colporaphy with good results. Colposuspension associated with anterior colporaphy is a reliable method for the treatment of cystoce

Diagnosis and management of female urinary incontinence.

Urinary incontinence affects an estimated 13 million Americans of which 85% are women. It is an embarrassing and lifestyle limiting condition for which effective treatment is available. Health care providers should be alert to the signs and symptoms of UI and pursue its etiology. Those patients who fail medical therapy, in whom the etiology for the incontinence is unclear, or those patients with concomitant cystocele, enterocele, or rectocele, should be referred to an incontinence specialist.

Clinical predictors of urinary incontinence in women.

OBJECTIVE: Our purpose was to characterize historic and clinical parameters in incontinent women to determine the predictive value for urodynamic diagnoses. STUDY DESIGN: The analysis includes 535 consecutive women with final diagnoses of genuine stress incontinence, detrusor instability, or both. Evaluations included a standardized history, examination, urinary diary, quantitation test, and urodynamics. The analysis used one-way analysis of variance, chi 2 analysis with Yates' correction, and Fisher's exact test. RESULTS: A total of 351 (66%) women were diagnosed with genuine stress inconti

[Urinary incontinence in patients with Parkinson syndrome].

Disturbances of micturition are very common in Parkinson's disease. Hence, we assessed 50 patients (22 female, 28 male, average age: 59.8 years) by means of the Gaudenz score. In 31 cases (62%) normal findings were obtained. Urge incontinence was diagnosed in 13 cases (26%) and stress incontinence in 5 patients (10%). One patient had both urge and stress incontinence. All patients with stress incontinence were female.

An algorithm for the management of urinary stress incontinence.

An algorithm for the management of urinary incontinence in the female is presented. The performance of history, physical examination, and simple office bladder testing are reviewed. Patients are categorized into a complex group which require multi-channel urodynamic testing or a non-complex group which may proceed to surgical intervention without such testing. Indications for collagen injection and pubo-vaginal sling procedure are discussed.
