BACKGROUND: Marginal ulcer (MU) remains a serious complication after Roux-en-Y gastric bypass (RYGB). This can be a life-threatening problem, even years after RYGB. Patients can present with pain or even with hemorrhage or perforation. There is no agreed-upon standard in prevention or treatment, although most perforated ulcers are treated with an omental patch. The gastric secretion inhibition, the second phase mainly, after the extragastric vagotomy (EGV) conduction, was proved in experiment. The inhibition of carbacholine and more profound (up to 92%) of pentagastrin secretion was noted. In 42 patients with duodenal ulcer disease the EGV was performed.

Short-term and long-term results of surgical treatment of 487 patients with duodenal ulcer were studied. In 337 patients (69.2%) truncal vagotomy was performed; in 94 patients (19.3%)--combined vagotomy; in 56 patients (11.5%)--selective vagotomy. With no lethal outcomes, the frequency of general surgical complications was 5.6% which did not depend on the type of vagotomy. According to the Visick criteria long-term excellent and good results were noted after truncal vagotomy in 90.4% of the patients, after combined vagotomy--in 88% and after selective proximal vagotomy--in 70% of the patients.…

Highly selective vagotomy (HSV) is the elective treatment of duodenal ulceration, however, it is little used in our hospitals. Amylin injected IP reduced food intake for 2-4 h in subdiaphragmatically vagotomized and in sham-vagotomized rats. The magnitude of the anorectic effect of 0.5 microgram/kg amylin was similar to that of 2.5 micrograms/kg amylin but the duration of the effect was somewhat shorter with the low dose. A 10-year experience with the use of vagotomy with the organ-preserving intervention in the treatment of patients with a bleeding pyloroduodenal ulcer who were treated for urgent and emergency indications is presented. The postoperative mortality was 8.1%. An excellent and good long-term result was noted in 88% of the patients, an unsatisfactory one--in 12%.

The author suggests a multifactor program for decoding the variant of the course of the postoperative period in patients with duodenal ulcer. It was tested in 199 patients. Three groups of patients with a favourable prognosis, an uncertain prognosis, and with a high risk of recurrent ulcer were distinguished. An individual therapeutic program with consideration for the risk group allows the incidence of postoperative recurrence of ulcer to be reduced by more than three times.

The results of highly selective vagotomy in 174 Indian patients have been analysed. Compared to other procedures on the stomach, HSV has a definite advantage both on long term as well as on short term basis. HSV has therefore become the procedure of choice in the treatment of duodenal ulcer disease provided the expertise is available locally. HSV has also been used now in the treatment of ulcer complications and benign gastric ulcer disease.

Proximal gastric vagotomy (PGV) consists of denervation of the body and fundus of the stomach, the antral nerve supply being left intact. A comparative analysis based upon studying results of clinico-endoscopic examinations of 376 patients with duodenal ulcers after different kinds of vagotomies evidences high efficiency of fibrogastroduodenoscopy in diagnosis of functional and morphological lesions after surgical procedures. Results of operations are dependent not only on the correct selection of surgical methods, adequate vagotomy and effective drainage, but also on a disturbance of the barrier function of the pylorus.
