Varicoceles: a coaxial coil occlusion system.

A modified transfemoral technique for testicular venography and occlusion of the testicular vein (TV) was used to treat 30 patients with a total of 43 varicoceles (17 left-sided and 13 bilateral). Occlusion was satisfactory in all left varicoceles and 90% of right varicoceles. This coaxial technique facilitates subselective catheterization of the TV and its collaterals and enables coils to be placed deep within these vessels, either alone or in combination with detachable balloons.

[Pathogenesis of symptomatic varicocele in kidney tumor].

On the basis of two case reports the pathogenesis of symptomatic varicocele in cases of kidney tumour due to flow impairment in the testicular vein is doubted. A reflux of venous blood due to collateral circulation is referred to as a possible cause.

The recurrent varicocele--a poorly recognized problem.

A varicocele is most likely the result of an incompetent vein with retrograde flow of blood into the scrotum. Surgical ligation of this incompetent system is the present-day procedure of choice. In our series of 225 varicocelectomies, there have been three definite recurrences, due to either recollateralization or failure to ligate all branches of this venous plexus.

Testis temperature, infertility, and the varicocele paradox.

The author presents arguments which favor elevated temperature of the testis and adnexae as an important factor in patients with poor semen seen in varicocele and "idiopathic" infertility. This appears to be primarily the result of disturbed thermoregulation due to internal spermatic vein retrograde blood flow. Elevated testis temperature without varicocele may be present in as high as 54% of all patients with "idiopathic" infertility. The prevailing theory in the United States is that the varicocele's bilateral effect is due to a substance noxious to spermatogeneis which is carried to both te…

[Pathogenesis and therapy of varicocele (author's transl)].

During the last 13 years 519 patients were operated upon for varicocele in the refertilisation center Hamburg. Out of 209 cases examined thereafter, 174 showed an improved spermiogram, and only in this group 40 children were fathered. Pathogenesis, diagnosis and surgical treatment are presented. A persisting varicocele (in our clientele in 1 per cent) was corrected in 18 patients. The reasons for recurrent surgery were overlooked branches of the V. spermatica interna and not yet described collaterals to neighbouring venous systems. For optimal surgical planning the latter are best demonstrated…

Testicular biopsies in 101 cases of varicocele.

Testicular biopsies were done on 101 patients who underwent ligation of the internal spermatic vein as primary treatment for infertility. Retrospective analysis included evaluation of tubular thickening, Leydig cell hyperplasia, premature sloughing, maturation arrest and decreased spermatogenesis. A comparison was made of biopsy profiles, sperm counts and pregnancies in an attempt to determine further the value of testicular biopsy in the subfertile man. The pregnancy rate was 40 per cent over-all, with 32 per cent of the patients with tubular thickening reporting pregnancies. There was only 1

Progressive testicular atrophy in the varicocele patient.

We examined 82 healthy volunteers, 61 subfertile men with varicoceles and 27 subfertile men without varicoceles for testicular size and semen quality. In 13 volunteers we found unsuspected varioceles and a left testicle significantly smaller than the other 69 volunteers. The size of the right testicle and the sperm density in these patient groups were similar. Both testicles of subfertile male subjects with varicoceles were significantly smaller than the testicles of subfertile male subjects without varicoceles. However, the sperm densities of these 2 groups were similar. Finally, although sub…

Sperm metabolomic profiles of varicocele-associated asthenozoospermia.

Varicocele is a leading cause of male infertility, with asthenozoospermia being a common phenotype. The metabolic basis of varicocele-induced asthenozoospermia remains poorly understood, necessitating a comprehensive metabolic profiling study. This GC-MS-based metabolomic study investigated sperm metabolic profiles across three cohorts: healthy controls (HC, n = 30), varicocele patients with normal motility (VC, n = 30), and varicocele-associated asthenozoospermia cases (VA, n = 30). Multivariate analyses (Principal Component Analysis, Partial Least Squares Discriminant Analysis, and Orthogona…

Oxidative Stress and Varicocele-Associated Male Infertility.

Despite being regarded as one of the most common causes of male subfertility, the pathophysiology of varicocele remains largely unknown. Recently, oxidative stress (OS) is proposed to be the mediator in how varicocele may negatively impact fertility. The imbalance of reactive oxygen species (ROS) and seminal antioxidants results in damage to sperm DNA and lipid membrane. There is evidence demonstrating higher OS level in men with varicocele which is also positively correlated with clinical grading of varicocele. Moreover, a number of studies have revealed the negative correlation between OS an…

Adrenal incidentalomas are a known entity that have been increasing in commonality with the advent of improved diagnostic and imaging techni … at the patient was entirely asymptomatic aside from complaints related to scrotal swelling due to a varicocele, for which this presentation of pheochromocytoma has been scarcely reported in the literature. Second, the patient had a pathology proven diagnosis of pheochromocytoma, while lacking both symptoms and biochemical findings to support the diagnosis, m …
