Endovascular repair of a double-lumen dissecting aneurysm.

Treating dissections and dissecting aneurysms requires maintenance of flow through the true lumen and exclusion of the false lumen from the circulation. A dissecting aneurysm of the vertebral artery presented with both a true and false lumen within the aneurysmal sac. Stenting of the true lumen followed by coil embolization of both lumens was performed. Management options and decision-making are discussed for this unique situation.

Blood withdrawn through a cannula valve connector does not result in clinically significant haemolysis.

OBJECTIVE: To assess the impact of cannula valve connectors on haemolysis of blood samples drawn from newly inserted cannulae. METHODS: In a semi-blinded, randomized study paired blood samples, with and without cannula valve connector, were obtained from patients within the ED and tested for haemolysis, defined as haemolysis index of greater than 120 mg/dL. Patients were randomized as to which sample was collected first. Cannula size was standardized and vacutainer systems provided consistent draw pressures. Time taken for the tube to fill was recorded as a measure of blood flow. RESULTS: Tw

Comparison of isolated venous approach with the standard approach in children undergoing patent ductus arteriosus device closure.

BACKGROUND: Transcatheter device closure is a safe procedure recommended in children with patent ductus arteriosus (PDA). While the standard procedure uses arterial and venous femoral access, it poses risk of vascular complications especially in young infants. Isolated venous approach has been tried in a few studies and was found to be non-inferior to the standard technique. In this prospective observational study, we have compared the two vascular approaches of PDA device closure in pediatric patients and have also studied the feasibility of this approach in young children with weight < 6 kg.

Wireless, non-invasive, high-resolution thrill sensor for continuous vascular access monitoring of hemodialysis patients.

Hemodialysis is the primary life-sustaining therapy for patients with end-stage renal disease (ESRD). Diagnosing and monitoring the health condition of vascular access (VA) for hemodialysis involves the use of ultrasonic imaging and angiography. These infrequent tests cannot, however, identify signs of mild stenosis and sudden onset to facilitate early intervention. Hence, approximately 70% of hemodialysis patients experience VA failure each year, rendering dialysis difficult and posing life-threatening risks. Here, we report wireless, non-invasive thrill sensors incorporating high-resolution

Quantitative and qualitative evaluation of residual deposits in implanted intravenous ports informs maintenance strategies.

This study aimed to evaluate the distribution and composition of intraluminal deposits in implanted intravenous ports removed from oncology patients, with a focus on assessing the effectiveness of current nursing maintenance protocols. Ports removed after completion of cancer treatment were analyzed. Each catheter was segmented into proximal, middle, and distal thirds for regional analysis. Total protein concentrations were measured using the bicinchoninic acid (BCA) assay. Thrombosis-related proteins-fibrin, fibrinogen, thrombin, and P-selectin-were identified by Western blot analysis. A tota

Long-term vascular access for infants with moderate to severe osteogenesis imperfecta.

PURPOSE: Osteogenesis imperfecta (OI) is a genetic disorder that causes skeletal fragility. For the most fragile infants and young children with OI, intravenous (IV) bisphosphonate administration is essential, but IV access attempts often cause fractures. Port-a-caths help prevent these events, but some surgeons are hesitant to insert these devices in these infants due to lack of data on their safety. METHODS: Retrospective study of pediatric patients with OI who underwent port-a-cath placement from 1999 to 2018; incidence of complications such as infection and thrombosis and need for reopera

Patched Closure of Dural Outlet of External Ventricular Drainage Catheter: Better Way to Prevent Cerebrospinal Fluid Leakage.

Postoperative continuous bloody cerebral spinal fluid drainage with external ventricular drainage catheter could decrease the incidence of inflammation. But the drainage catheter left in place will interrupt the watertight dura matter closure. To prevent the related cerebrospinal fluid leakage and subgaleal pseudomeningocele after the catheter removal, we developed a patched closure method to seal the dural outlet of the drainage catheter with a favorable clinical outcome.

A Novel Method: Super-selective Adrenal Venous Sampling.

Primary aldosteronism (PA) and subclinical Cushing's syndrome (SCS) are conditions in which the adrenal glands autonomously produce excessive amounts of aldosterone and cortisol, respectively. The conventional adrenal venous sampling (cAVS) method collects blood samples from both adrenal central veins and is useful for identifying the laterality of excess hormone production in a unilateral lesion(s), as documented in PA cases. In cAVS, plasma cortisol concentrations (PCCs) are used to normalize plasma aldosterone concentrations (PACs). A novel "super-selective" adrenal venous sampling (ssAVS)

The outcomes of percutaneous versus open placement of peritoneal dialysis catheters.

BACKGROUND: Peritoneal dialysis catheters (PDCs) can be inserted by a percutaneous, open surgical, or laparoscopic approach. Considerable controversy surrounds the mode of catheter placement and its impact on technical success. We compared the complications and survival characteristics of PDCs that were inserted by the open approach versus those inserted percutaneously. METHODS: We reviewed the outcomes of 167 patients who received PDCs between September 2009 and February 2012. Of these, 89 were placed percutaneously and 78 were placed by open surgical techniques. The mechanical complication

Jugular vascular access port implantation for frequent, long-term blood sampling in cats: methodology, assessment, and comparison with jugular catheters.

Long-term, frequent venous access for diagnostic, therapeutic, or research purposes in cats is problematic. Frequent blood sampling over extended periods is necessary for some therapeutic regimes and often required for clinical research in veterinary science. In this paper, we describe the implantation of vascular access ports (VAPs) and assess their use for repeated blood sampling over 16 weeks and 38 weeks, as well as the use of jugular catheters for one week. The VAP placement procedure was well-tolerated with few minor complications (minor swelling, contusion, or superficial dermatitis fro

ProGlide-AngioSeal versus ProGlide-FemoSeal for vascular access hemostasis posttranscatheter aortic valve implantation.

BACKGROUND: The hybrid strategy combining plug-based and suture-based vascular closure devices (VCD) was introduced as a promising technique for vascular access hemostasis after transcatheter aortic valve implantation (TAVI) with satisfactory outcomes. However, data comparing two plug-based VCDs each in the combination with a suture-based VCD, namely ProGlide/AngioSeal (P/AS) with ProGlide/FemoSeal (P/FS) VCDs, is still lacking. AIMS: To compare the 30-day outcome of the hybrid strategy using P/AS versus P/FS for vascular access site closure after TAVI. METHODS: A retrospective single-center

The novel use of the Haemodialysis reliable outflow graft (HeRo®) in intestinal failure patients with end-stage vascular access.

Intestinal failure (IF) patients are dependent on central venous access to receive parenteral nutrition. Longstanding central venous catheters are associated with life-threatening complications including infections and thromboses resulting in multiple line exchanges and the development ofprogressive central venous stenosis or occlusion. The Haemodialysis Reliable Outflow (HeRO) graft is an arterio-venous device that has been successfully used in haemodialysis patients with 'end-stage vascular access'. We describe a case series of HeRO graft use in patients with IF and end-stage vascular access

Dermal Abrasion Experienced as an Adverse Effect of the EZ-IO(®).

BACKGROUND: In 2007, an update was released to the pediatric and neonatal septic shock guidelines, which emphasized early use of therapies, specifically, first-hour fluid resuscitation and inotrope therapy. This has led to increased use of intraosseous (IO) access as a source of vascular access. Previously, IO access could be obtained only via a manual IO placement. New semi-automatic devices, such as EZ-IO(®) (Vidacare, Shavano Park, TX), allow for safer and quicker IO access. Data support the use of semi-automatic devices during the acute resuscitation period. CASE REPORT: The patient was a

First-in-human evaluation of a hand-held automated venipuncture device for rapid venous blood draws.

Obtaining venous access for blood sampling or intravenous (IV) fluid delivery is an essential first step in patient care. However, success rates rely heavily on clinician experience and patient physiology. Difficulties in obtaining venous access result in missed sticks and injury to patients, and typically require alternative access pathways and additional personnel that lengthen procedure times, thereby creating unnecessary costs to healthcare facilities. Here, we present the first-in-human assessment of an automated robotic venipuncture device designed to safely perform blood draws on periph
