An infectious cause has long been suspected for most forms of vasculitis. In most cases vascular damage has been ascribed to immune-mediated mechanisms rather than direct microbial toxicity. Evidence cited in this review provides further support for a causal role of infection in the vasculitides by strengthening the link between group A streptococci and hepatitis B virus with polyarteritis nodosa and Kawasaki syndrome. It is anticipated that the application of new molecular technology will provide definitive evidence for the role of infection in the etiopathogenesis of the vasculitides.

We have studied 495 sera that were referred to us from patients suspected on clinical and/or histological grounds to have a small vessel vasculitis. These sera were tested for antibodies against neutrophil cytoplasm antigens (anti-neutrophil cytoplasm antibodies, ANCA) using assays based on neutrophil acid extract, myeloperoxidase and elastase. Such antibodies are commonly found in Wegener's granulomatosis (WG) and microscopic polyarteritis (MPA), and sometimes in other small vessel vasculitides. One hundred and twenty-six of these sera (25%) were positive in the acid extract ELISA, 68 (14%) i…

Vasculitis of arteries and veins is an important feature of Behçet's disease. When it occurs in systemic or pulmonary arteries or in large veins it is life threatening. Retinal vasculitis is the hallmark of eye disease, and much of the central nervous system disease is linked to small artery occlusions. Treatment with immunosuppressive drugs has improved the prognosis for sight and life.

Relapsing polychondritis is a systemic disease of unknown etiology with predominate manifestations of multiorgan cartilaginous inflammation. Although relapsing polychondritis occurs predominantly as a separately defined clinical complex, a significant number of patients may suffer from another rheumatic disease. Vasculitic syndromes are the most commonly observed disorders associated with relapsing polychondritis.

Vasculitis is the predominant feature of a large number of diseases in childhood. Cerebral vasculitis are quite common and sometime misinterpreted. They can be to express the direct vascular damage caused by the bacterial, viral, mycotic and protozoal agents; otherwise they can be to arise from some type of immunopathology including the presence of circulating immune complexes, or the prevalent defect of the T-Cells, or else still not well-know. The histological anatomy will to display the leukocytoclastic vasculitis or the granulomatous arteritis, and often will to make clear the pathogenetic…

Two cases of young patients with an acute abdomen caused by a necrotizing angiitis of the cecum and ascending colon are reported. The absence of any systemic disease and a history of medical treatment in both cases led to the diagnosis of hypersensitivity vasculitis by a serum sickness-like reaction. An extensive review of the literature is also presented.

The vasculitides consist of a spectrum of clinical syndromes having in common necrotizing inflammation of the vascular system. Typical classification systems are based on both size of vessel involved and clinical gestalt. The author favors a pragmatic classification. Symptoms are manifestations of systemic inflammation and of organ ischemia secondary to inflammatory vascular occlusion. Multicenter studies suggest that the most common vasculitides do not satisfy criteria for one of the specific nosological entities and would be classified as "other" vasculitides. For those syndromes fitting a c

A great number of vasculitis in childhood is associated with Rheumatic disease. Vascular lesions involve small vessels of various organs. These are described in systemic lupus erythematosus, juvenile rheumatoid arthritis, dermatomyositis, scleroderma, mixed connective tissue disease, Behçet syndrome, Sjögren syndrome.

Pulmonary vasculitis is usually caused by one of three disorders: (1) Wegener's granulomatosis (WG); (2) Churg-Strauss syndrome (CSS), or allergic angiitis and granulomatosis; or (3) a nonspecific small vessel systemic necrotizing vasculitis (SNV), or microscopic polyarteritis. WG, the most common cause of lung vasculitis, has features of a granulomatous vasculitis of the upper airway and lung and widespread small vessel vasculitis involving the kidneys and other organs. The features of pulmonary WG overlap with those of malignancy and infectious granulomatous lung disease; accurate diagnosis …
