Placement of a Greenfield filter in the superior vena cava.

A 75-year-old man with a Greenfield filter in the inferior vena cava for previous pulmonary emboli had recurrent pulmonary emboli. He was unable to take anticoagulants due to gastric ulcer disease. Contrast studies revealed a large thrombus in the left brachiocephalic vein. A Greenfield filter was placed in the superior vena cava through the right internal jugular vein using the femoral insertion apparatus.

Superior vena cava and central venous reconstruction.

Partial or complete obstruction of the superior vena cava and its major tributaries occassionally results in incapacitating venous hypertension of the upper extremities and/or head and neck. Factors intrinsic and extrinsic to the central veins play a role in the pathogenesis. The more common causes include mechanically and chemically induced intimal injury with resultant fibrosis, sclerosis, or thrombosis and neoplastic masses with external compression or direct extension in the central venous structures. Medical therapy is indicated in the acute situation and generally allows the time necessa

Left superior vena cava with atrial displacement.

Two cadavers were dissected showing persistence of left-sided superior vena cava with atrial displacement and inversion of the venous drainage from the body wall (azygos system of veins). The presence of atrial displacement in relation to a persistent left superior vena cava observed in this study lends support to the view that haemodynamic factors influence the morphogenesis and development of the heart. The development of an asymmetrical venous pattern from a symmetrical venous drainage may be influenced by the same factors which influence the arterial end of the primitive heart tube.

[Current indications for inferior vena cava interruption].

Clinical indications of vena cava interruption are reviewed. During the last few years pulmonary embolism frequency remained high and many new percutaneous vena caval filters became available. These facts probably explain the increasing use of these filters reaching about 10,000 filters each year in France. Existing data show that: embolic risk with antithrombotic agents is less than 5%, probably not far greater than embolic risk with cava filters (about 2%); complications encountered with the filters are caval thrombosis in 8%, and more or less than 4% other major complications; there is no c

Retrohepatic vena cava reconstruction with polytetrafluoroethylene graft.

The case of an 18-month-old male who underwent a right hepatic lobectomy for hepatoblastoma with extensive involvement of the retrohepatic vena cava is presented. The retrohepatic vena cava was replaced with an expanded polytetrafluoroethylene graft. This graft was proved patent by ultrasonography 2 years after operation. The child has no evidence of recurrent hepatoblastoma. The results of grafting the retrohepatic vena cava have been notoriously poor. It is widely believed that a prosthetic vena cava graft cannot be expected to remain patent. Our experience with polytetrafluoroethylene and p

[Pathology of the inferior vena cava and liver transplantation].

Pathology of the inferior vena cava is not frequently encountered in the context of liver transplantation. Such a pathology was observed in 7.9% of our recipients, in the pre- intra- and post-operative period. Pre-existing anomalies of the IVC consisted the absence of the retrohepatic vena cava in 7 children with biliary atresia; technical adjustments were quite simple. During the operative period, dissection of the supra-hepatic vena cava was made very difficult in 2 patients with hepatic alveolar echinococcosis complicated by secondary biliary cirrhosis it was necessary to make a trans-diaph

Congenital obstruction of the caudal vena cava in a dog.

A 16-week-old female Rottweiler was examined because of labored breathing and abdominal distention. Ascites, hepatomegaly, and pleural effusion were detected during physical and radiographic examinations. Selective angiography revealed a stenotic lesion at the junction of the caudal vena cava and the right atrium. A fibrous diaphragm with a pin-hole orifice was observed at surgery. Clinical signs of disease resolved after successful removal of the membranous obstruction.

A contemporary perspective on superior vena cava syndrome.

The superior vena cava (SVC) syndrome is usually associated with advanced malignancy and has a dismal prognosis. In order to analyze the impact of newer diagnostic and therapeutic modalities, we retrospectively examined the last 45 consecutive cases of SVC syndrome treated over a 12-year period. The underlying causes were advanced lung cancer (57%), mediastinal tumors (20%), and metastatic solid malignancy (5%). Forty-two patients (93%) were treated with external beam radiotherapy and/or chemotherapy with a mean patients survival of 3 months; 11 of 42 patients (26%) were treated without histol

Echocardiography in planned interruption of the inferior vena cava.

Interruption of the inferior vena cava by the transvenous placement of a filter or umbrella effectively prevents pulmonary embolism by acting primarily as a physical barrier to emboli. Such a device will be effective only if the site of thrombosis is distal to the planned placement site. We have presented two cases in which a preoperative echocardiogram revealed a right atrial embolus, thereby mandating either embolectomy, fibrinolytic therapy, or continued anticoagulation in addition to the filter placement. These cases suggest that an echocardiogram should be included in the evaluation prece

Intracardiac causes of superior vena cava obstruction.

Most causes of superior vena cava (SVC) obstruction are extracardiac. In rare instances, an intracardiac process may obstruct the venous return from the SVC. This is illustrated by a report of a patient with a congenital coronary artery fistula to the right atrium obstructing the SVC return. We propose a classification and clinical approach to the differential diagnosis of SVC obstruction.

Interruption of inferior vena cava with anomalous continuations.

A case of interruption of the inferior vena cava which has two continuational pathways is reported. One is the hemiazygos and azygos continuation and the other is an unusual intrathoracic continuation, phrenic and pericardial vein draining through the highest intercostal vein into the left brachiocephalic vein.

Etiologic considerations in superior vena cava syndrome.

The Mayo Clinic experience with superior vena cava obstruction during the last 20 years was reviewed. The diagnosis of superior vena cava obstruction is often made at the bedside. Typical symptoms include suffusion, dyspnea, cough, and, less commonly, pain, syncope, dysphagia, and hemoptysis. The most important physical findings are the increased collateral veins covering the anterior chest wall and the dilated neck veins with edema of the face, arms, and chest. The chest x-ray film usually shows widening of the superior mediastinum. Of our 86 cases of superior vena cava obstruction, 67 (78%)

Uninterrupted inferior vena cava with azygos continuation.

Azygos continuation of the inferior vena cava is diagnosed in infants in the presence of obstruction to flow in the inferior vena cava due to infrahepatic interruption. Recently we studied in an infant complex congenital heart disease in which there was azygos continuation of the inferior vena cava without infrahepatic interruption or any other obstructive lesion of this vessel. The blood from the lower part of the body drained into the right atrium by two wide patent veins: the inferior vena cava and the azygos system. The angiocardiographic observations of this condition in an infant are rep
